Introducción: Se definen como hernias inguinoescrotales gigantes a aquellas que se extienden por debajo del punto medio de la cara interna del muslo, con el paciente en posición de pie. Debido a la pérdida de domicilio que se puede presentar en estos casos, durante la cirugía, al reintroducir el contenido herniario a la cavidad abdominal, se puede producir un síndrome compartimental abdominal.Caso clínico: Se describe un caso de un hombre de 50 años de edad con una hernia inguinoescrotal de 15 años de evolución, con contenido intestinal y omental. Se le realizó neumoperitoneo progresivo preoperatorio. La hernioplastia fue llevada a cabo con la colocación de una malla de polipropileno libre de tensión. El paciente tuvo una recuperación sin complicaciones ni recurrencias.Discusión: El manejo quirúrgico de las hernias inguinoescrotales gigantes que han perdido el domicilio, es un desafío para el cirujano. A lo largo de la historia se han descrito diversas técnicas para llevar a cabo su reparación. Entre ellas, en 1940, Goñi Moreno introduce el concepto de neumoperitoneo progresivo preoperatorio. Es importante conocer los benefi cios y riesgos de cada una y saber aplicarlas acorde a cada paciente. El neumoperitoneo progresivo es una opción válida para reparación de las hernias inguinoescrotales gigantes. Permite, previo a la cirugía, un aumento del volumen de la cavidad abdominal y una disección neumática del saco herniario y de su contenido.
Laparoscopic splenectomy by primary hydatid cyst of the spleen Objective: Describe a case report of a hydatid splenic cyst of one year of evolution, treated by laparoscopy. Materials and Methods: A 36-year-old woman with left upper quadrant pain for 1 year. The topographic study showed a 67mm splenic cystic lesion. Laparoscopic splenectomy was performed without incident. Result: Favorable post-operative evolution. Histology compatible with splenic hydatidosis. Discussion: The incidence of splenic hydatidosis is low, even in endemic areas, approaching 1-2%. Surgery is the therapeutic strategy of choice, given the risks of rupture. However, there is no consensus regarding the technique. Conclusion: Laparoscopic splenectomy is a safe procedure supported by the literature.
Hepatocellular carcinoma is the sixth most common neoplasm in the world and the third cause of death in cancer patients. Mortality in patients with this condition is mainly due to cancer cachexia, liver failure, and spontaneous tumor rupture. This report describes the clinical case of a 69-year-old patient who attended the hospital emergency department complaining of sudden onset abdominal pain located in the epigastrium and vomiting with a tendency to hypotension. The patient had to undergo emergency surgery due to the rupture of a previously unknown hepatocellular carcinoma. In addition to the clinical description of the case, the study is accompanied by a discussion on the possibilities of therapeutic management of this type of patient based on the international literature on the subject.
ANTECEDENTES: los liposarcomas son los tumores sarcomatoides de partes blandas más comunes en los adultos. Los sitios más frecuentes de aparición son las extremidades, el retroperitoneo y la región inguinal.CASO CLÍNICO: paciente masculino de 57 años de edad, quien consulta por una masa abdominal, de crecimiento progresivo, de 8 meses de evolución. La tomografía computada objetivó una masa originada a partir del retroperitoneo, que encapsulaba el riñón derecho y desplazaba los órganos abdominales hacia la izquierda. Mediante laparotomía exploradora se logró la exéresis completa del tumor retroperitoneal. El informe anatomopatológico definitivo fue de liposarcoma bien diferenciado, con márgenes quirúrgicos negativos (R0) y tumor de 56 x 35 x 30 cm, de 27 kg. CONCLUSIÓN: la resección completa de los liposarcomas representa la única posibilidad de tratamiento radical, lo que sugiere el factor predictivo más importante de recurrencia y supervivencia.PALABRAS CLAVES: liposarcoma; neoplasias retroperitoneales; sarcoma.
