Objetivo: Describir el proceso para la formulación e implementación de una estrategia de contingencia de un servicio público de Ortopedia y Traumatología frente a la pandemia de COVID-19. Materiales y Métodos: Estudio descriptivo del proceso para la formulación e implementación de una estrategia de contingencia de la pandemia de COVID-19 en un Servicio de Ortopedia y Traumatología del sistema público de salud de la Ciudad Autónoma de Buenos Aires, entre el 12 de marzo y el 20 de abril de 2020, período en el cual fue declarada la emergencia sanitaria y el alerta amarilla en nuestro medio frente a la posibilidad inminente deun potencial aumento exponencial de casos de COVID-19. A cargo de un Comité de Pandemia de Ortopedia y Traumatología, el proceso fue dividido en tres instancias: planificación, implementación y control. Resultados: Basados en la normativa nacional, 17 artículos internacionales, 13 guías de sociedades científicas y 17 artículos de recomendaciones de la Organización Mundial de la Salud y los Centros para el Control y la Prevención de Enfermedades de los Estados Unidos. Definimos 7 áreas de trabajo: 1) Sala de internación, 2) Quirófano, 3) Consultorios externos, 4) Servicio de Urgencias, 5) Insumos, 6) Cuidado del personal de salud, 7) Docencia e investigación. Conclusión: Nuestro trabajo aporta una propuesta para el proceso de desarrollo de una estrategia de contingencia frente a la pandemia de COVID-19. Introducción: El objetivo principal fue comparar la actividad clínico-quirúrgica de nuestro Servicio de Ortopedia y Traumatología antes de la instauración del plan de contingencia frente a la pandemia de la COVID-19 y durante este, un aspecto relevante para establecer antecedentes que permitan guiar nuestro desempeño ante la segunda ola de casos y futuras contingencias semejantes. Materiales y Métodos: Estudio analítico observacional longitudinal ambispectivo de los pacientes atendidos en el Servicio de Ortopedia y Traumatología de un hospital del sistema público de la Ciudad Autónoma de Buenos Aires, durante el plan de contingencia frente a la pandemia COVID-19. Como variable de resultado primaria se valoró el número total de cirugías realizadas durante el período de estudio. Resultados: La pandemia se asoció con menos cirugías totales (p = 0,002), de Guardia (p = 0,000) y de Planta (p = 0,002). Las cirugías totales se redujeron un 81,5%. Las cirugías de urgencia representaron el 97%, diferencia significativa con el período fuera de la pandemia (p = 0,080). Además, el promedio de cirugías por día quirúrgico disponible fue significativamente menor (p = 0,000). Conclusiones: Creemos que la atención de la enfermedad ortopédica y el trauma musculoesquelético, adaptada a las obligaciones indicadas por el plan nacional y regional de contingencia frente a la pandemia, fue notoriamente menor a nuestro desempeño habitual. Si bien respondimos adecuadamente a las urgencias de nuestra especialidad queda pendiente la posibilidad de estratificar y considerar tipos de pacientes con cuadros pasibles de ser tratados, en forma gradual, durante la pandemia.
<p>Este trabajo relaciona la bibliografía con la experiencia de nuestros cirujanos para afrontar dificultades con un instrumental defectuoso en el tratamiento de fracturas de fémur con osteosíntesis endomedulares y estimar la existencia de un canal de reclamo.</p><p><strong>Objetivos</strong></p><p><strong>1.</strong>Enumerar inconvenientes técnicos presentados en cirugías de fémur.<strong> 3. </strong>Comparar diferentes centros y problemas afrontados. <strong>4. </strong>Estimar canales de reclamo.</p><p><strong>Material y Métodos</strong></p><p><strong>1: </strong>Estudio retrospectivo observacional descriptivo sobre las historias clínicas y archivo radiológico de nuestra institución. <strong>2: </strong>Encuesta on-line enviada a traumatólogos generales. <strong>3:</strong> Consulta con ANMAT, IRAM, Ministerio de Salud de la ciudad y la Nación.</p><p><strong>Resultados: </strong></p><p><strong>1.</strong> 31 pacientes con fracturas de fémur tratados con osteosíntesis endomedulares entre enero de 2008 a agosto de 2013. Se documentaron 19 casos de fallas o defectos del instrumental de colocación en 15 pacientes. Los problemas más frecuentes fueron las guías y las mechas.<strong> 3. </strong>270 respuestas. 19 provincias nacionales. Respuestas de Colombia, Ecuador, Italia, Australia y Bolivia. Se obtuvieron 180 respuestas de centros privados y 90 de centros públicos. <strong>4. </strong>4 vías de reclamo: ANMAT bajo el programa de Tecnovigilancia, IRAM por el incumplimiento de las Normas ISO 9001, Ministerio de Salud por incumplimiento de la resolución 255 y por incumplimiento de la Ley Básica de Salud 153 Art. 12 (ítem k y l) y AAOT, en la subcomisión de Implantes.<strong></strong></p><p><strong>Conclusión</strong></p><p><strong></strong>Queda explicita la diferencia entre hospitales públicos y centros privados donde en estos últimos las estadísticas fueron favorables. El medio laboral solo definió prevalencia de inconvenientes técnicos pero los tipos de inconvenientes fueron los mismos. Existen formas y recursos para denunciar y enfrentar esta problemática.</p><p> </p>
Objectives:To perform a description of tendinopathies as an injury in volleyball high performance.Methods:An observational and prospective study was conducted from 2014-2016 in the senior Argentinian volleyball team. The same was held by two observers. Moreover, 78 athletes were evaluated. We support Dvorak’s claims that an injury is determined by the loss of at least one training session or a match.Results:78 players were exposed to 21812 hours of training and matches. As a result 37 injuries were evaluated in 31 players. Taking into account tendinopathies, it can be said that 34 players consulted 412 times, showing a prevalence of 43.5% of the whole enquires but when we refer to the same pathology as injury the average lowers, presenting 8 lesions in 6 players and showing a prevalence of 7.6% as injuries. Incidence of tendon injuries is 0.32 per 1000 hours of exposure Tendon Injuries: 5 were patellar, 2 supraspinatus, 1 aquiles. 5 Slight, 2 moderate, 1 severe.Conclusion:Clearly, tendinopathy is a common problem in this sport but it is not a common cause of injury. This is demonstrated in prevalence rates whereas 43.5 % just consulted and 7,6 % suffer from real injuries. We think this might be due to several factors such as, advances in medical therapy, preventive protocols and increase in thresholds of pain that high-performance athletes can bear. In our experience this pathology was shown to be the third leading cause of injuries. In 2016 we did not deal with any case of injury for tendinopathy.
