Introdução: A água é um nutriente essencial para a funcionalidade do organismo. A desidratação é frequente no idoso e pode lhe trazer graves consequências, inclusive levá-lo í morte. Objetivo: Identificar a importância da hidratação em idosos institucionalizados, seus fatores interferentes e estratégias para melhorar a ingesta hídrica dessa população. Metodologia: Revisão bibliográfica integrativa realizada nas bases de dados PubMed, Scielo, Medline, Lilacs e Cochrane, com seleção de 16 artigos, publicados nos últimos 10 anos. Resultados: A água constitui 55% do peso corporal de idosos, sendo sua reposição essencial. Alterações próprias do processo do envelhecimento como diminuição da percepção da sede associadas í diminuição do consumo hídrico por: comprometimento da capacidade cognitiva, dificuldade de deglutição e dependência de cuidadores, são as principais dificuldades enfrentadas no processo de hidratação desses indivíduos, especialmente quando institucionalizados. A oferta fracionada de líquidos sob várias formas (sopas, caldos, chás e águas saborizadas) e de alimentos ricos em água (leite desnatado, determinadas frutas, verduras e legumes), são estratégias que podem contribuir para uma melhora da ingestão hídrica nessa população. Conclusão: A desidratação é uma etiologia frequente de hospitalizações e morbimortalidade em idosos, tendo a nutrição importante papel na adequação do consumo hídrico dessa população.
