OBJETIVO: avaliar o conhecimento e conduta de profissionais de saúde em relação à diarreia em pacientes recebendo nutrição enteral. MÉTODOS: estudo transversal realizado com profissionais de saúde de duas Unidades de Terapia Intensiva de adultos. Os dados foram coletados por meio de um questionário autoaplicável. RESULTADOS: participaram do estudo 37 profissionais, sendo 17 (45,9%) auxiliares ou técnicos em enfermagem, 8 (21,6%) enfermeiros, 8 (21,6%) médicos, 3 (8,1%) nutricionistas e 1 (2,8%) profissão omissa. A definição mais comum encontrada para diarreia foi “três ou mais episódios de evacuações líquidas ou semilíquidas/dia.” As principais causas citadas para diarreia foram a “infecção” e a “dieta”. A atitude/conduta mais observada foi “comunicação à equipe”. A maioria (16; 43,2%) presencia, com muita frequência, dietas interrompidas por causa de diarreia e a “adequação da dieta” e a “capacitação da equipe multiprofissional” foram as soluções mais citadas para se prevenir essa interrupção. A maioria (20; 54%) dos profissionais não participou de nenhum treinamento sobre diarreias, e 33 profissionais (cerca de 90%) considera importante a implementação de um protocolo. CONCLUSÃO: há divergências entre os profissionais de saúde, tanto em seus conhecimentos quanto nas condutas frente ao paciente com diarreia na UTI, o que pode influenciar diretamente no cuidado com o mesmo. A capacitação da equipe multiprofissional e a criação de um protocolo são essenciais.
Introduction: To report the clinical course of a Chagas’s disease patient with severe intestinal failure after resection of the total colon and terminal ileum.Case Report: The patient underwent rectosigmoidectomy (of the sigmoid volvulus, December 2009) and total colectomy plus partial ileectomy (May 2011). Patient evolved with multiple hospitalizations caused by severe diarrhea (up to 23 stools/day), hydroelectrolyte disturbance and acute renal failure, severe protein-energy malnutrition [loss of 34.9% of usual body weight (uBW)], and multiple episodes of sepsis. Were prescribed parenteral nutrition solutions exclusively or concurrently with very small volumes (e.g., 40ml/day) of semi-elemental diet with low fat and high protein. After several weeks predetermined amounts of carbohydrate-rich foods (potatoes, rice, pasta, cassava), vegetables (chayote, carrots), low-fat meat, cream crackers, coconut water and fruit (watermelon, melon, banana) were progressively introduced. After 36 months, the patient was metabolically stable (BW=67.2 kg, weight recovery of 23.6 kg), with hydroelectrolytic balance. Due to the recovery of the functional capacity, the patient was advised to return to his work activities. Conclusions: Chagas’s disease patient submitted to intestinal resection may have severe intestinal failure and protein-energy malnutrition. Specialized nutritional therapy and clinical and laboratory monitoring by a multidisciplinary team, can contribute to better prognostic.
INTRODUÇÃO: A desnutrição proteico-energética é definida como resultado da deficiência calórica e/ou proteica, podendo causar alterações na composição corporal como a sarcopenia, no estado mental, na funcionalidade e consequentemente prejudicar o desfecho clínico. OBJETIVO: Avaliar oferta energética e proteica de pacientes críticos, submetidos à fisioterapia convencional associada ao cicloergômetro ativo (ou unicamente a fisioterapia convencional) e correlacionar com os dados antropométricos, e a escala MRC e Perme Escore. MATERIAL E MÉTODOS: estudo piloto com pacientes críticos internados na UTI, divididos entre grupo controle (GC) e experimental (GE). Todos foram avaliados pela escala MRC e PERME escore e submetidos a medidas antropométricas. Após 8 dias de intervenção fisioterapêutica convencional (GC), ou clicoergômetro ativo associado a abordagem convencional (GE), eles foram reavaliados. O aporte proteico e calórico de todos foram avaliados todos os dias. RESULTADOS: Houve redução significativa de medidas de circunferência de panturrilha no GC e perda não significativa de medidas no GE (p=0,001). O nível de adequação calórico e proteico foi de 73,9% e 69,5% respectivamente. Não houve diferença significativa entre os grupos. CONCLUSÃO: A mobilização com o cicloergômetro parece exercer efeito positivo na preservação da massa muscular em membros inferiores de pacientes críticos. No entanto, estudos que avaliam a medida de massa muscular em membros inferiores de modo mais controlado são necessários para comprovar essa hipótese.
| Introduction:The high prevalence of hospital protein-energy malnutrition is associated with several factors, including implemented dietary conducts. Objective: Analyze nutritional characteristics of the general hospital oral diet (GHOD). Methods and materials: A prospective and descriptive study, with a qualitative and semi-quantitative approach, concerning the GHOD menu, was developed from May 2012 to August 2013. GHOD menu was established for 28 days and consisted of five meals: breakfast, lunch, mid-afternoon snack, dinner and bedtime snack. Weighing of each food served in the meals (lunch/dinner) from the GHOD menu was performed. Composition tables were used to calculate the total energy value (TEV) and the energy distribution percentage of macronutrients of the foods, as well as to quantify the food portions from different food groups. Results: The average energy distribution to TEV (2409.4±152.6 kcal/day) was adequate for proteins (13.4%), carbohydrates (64.7%), and lipids (21.9%). Variable energy supply was identified at breakfast (15.6-20.6%), lunch (26.2-36.6%), dinner (22.2-31.1%), mid-afternoon and bedtime snacks (8.4-15.5%). The overnight fasting period was up to 13h. In the evaluation of nutritional quality of the GHOD meals in accordance with food groups was identified excessive offer for beans, meat/eggs, oils/fats/oilseeds and sugar/sweets, and deficient offer for fruit/juices, legumes/vegetables and milk/ dairy. Conclusions: It is essential to perform quantitative and qualitative nutritional characterization of the GHOD served to inpatients. Reducing the duration of the overnight fast, as well as adjustments in the supply of food groups and in food fractionation, can contribute to better meeting the nutritional needs and prevention of nutritional deficiencies.
