Thrombotic thrombocytopenic purpura (TTP) is a multisystemic disorder characterized by microangiopathic hemolytic anemia and thrombocytopenia, which may be accompanied by fever, renal, or neurologic abnormalities. Cases are divided into acute idiopathic TTP and secondary TTP. Autoimmune diseases, especially systemic lupus erythematosus, in association with TTP have been described so far in many patients. In contrast, TTP occurring in a patient with mixed connected tissue disease (MCTD) is extremely rare and has only been described in nine patients. We describe the case of a 42-year-old female with MCTD who developed thrombocytopenia, microangiopathic hemolytic anemia, fever, and neurological symptoms. The patient had a good clinical evolution with infusion of high volume of fresh frozen plasma, steroid therapy, and support in an intensive care unit. Although the occurrence of TTP is rare in MCTD patients, it is important to recognize TTP as a cause of thrombocytopenia and hemolytic anemia in any patient with autoimmune diseases. Prompt institution of treatment remains the cornerstone of treatment of TTP even if plasma exchange is not available like what frequently happens in developing countries.
Introdução: Os linfomas constituem um grupo de neoplasias originárias do tecido linfóide, sendo o linfoma de células do manto um subtipo de Linfoma não-Hodgkin (LNH) derivado dos linfócitos B pequenos “virgens” da zona do manto. Corresponde a 6% de todos os LNH.Objetivo: Descrever um caso de linfoma não-Hodgkin de células do manto.Método: Revisão de prontuário médico.Relato de Caso: J.N.A, 62 anos, sexo masculino, com história de adenomegalias cervicais indolores à esquerda, seguidas pelo aparecimento de adenomegalias axilares à direita e esquerda; inguinais bilaterais; supra-claviculares e aumento indolor da bolsa escrotal. Relata sudorese noturna, acompanhada de astenia e emagrecimento. Ao exame: Linfonodos firmes, aderidos a planos profundos, de consistência elástica. Abdômen: globoso, depressível, indolor à palpação superficial e profunda, apresentando hepatoesplenomegalia. Radiografia de tórax apresentando adenomegalia mediastinal bilateral. O diagnóstico foi realizado por biopsia do linfonodo cervical: linfoma não-Hodgkin de células do manto, apresentando imunohistoquimica com CD20 e Ciclina D1 positivos. Biopsia da medula óssea ilíaca direita e à esquerda: Infiltração difusa por Linfoma não-Hodgkin de pequenas células, compatível com linfoma de células do manto,correspondendo a 90% da celularidade total. Feito pulsoterapia com corticóide e após, inicio da quimioterapia com esquema CHOP. No momento em acompanhamento multidisciplinar.Conclusão: Este caso retrata um linfoma agressivo, com doença disseminada (estágio IV de Ann Harbor) e índice de prognóstico (IPI) com escore elevado, indicando um pior prognóstico. Nestes casos os tratamentos têm sido insatisfatórios, sendo a remissão completa atingida por uma minoria, com sobrevida média entre 3 e 5 anos.
Introdução A Política Nacional de Atenção à Urgência e Emergência (PNAU), instituída em setembro de 2003, tem a finalidade de intermediar a atenção básica e os serviços de média e alta complexidade, e como parte desta política, o SAMU - 192, auxilia na constituição do atendimento na rede de urgência e emergência à população 24 horas por dia. O enfermeiro tem sua presença obrigatória no Atendimento Pré-Hospitalar (APH) definida na Resolução nº 375 de 22 de março de 2011(²), do Conselho Federal de Enfermagem. Diante disso, a Liga Acadêmica de Trauma e Emergência Profº Irany Salomão–LATE, da Universidade Estadual de Santa Cruz, em parceria com o SAMU-Ilhéus, viabilizaram um estágio extracurricular para os ligantes, em vista da necessidade de ampliar a visibilidade e promover conhecimento e vivências de assistência de enfermagem no ambiente pré-hospitalar. Objetivo: Pretende-se relatar a experiência de acadêmicos de enfermagem nas atividades desenvolvidas no estágio extracurricular no SAMU-Ilhéus. Métodos: O estágio ocorreu no período de dezembro 2019 a fevereiro de 2020, com plantões de 12 horas semanais. Os acadêmicos receberam treinamento teórico sobre o atendimento básico e avançado, com os seguintes temas: atendimento inicial ao politraumatizado, Suporte Básico de Vida, medidas avançadas de ressuscitação cardiopulmonar, crises convulsivas, entre outros. Resultados: Durante as vivências, foram presenciados casos de obstrução de vias aéreas, perfuração por arma de fogo, perfuração por arma branca, crises convulsivas e metabólicas, acidente vascular encefálico, traumas por acidentes de trânsito, paradas cardiorrespiratórias e outros. Como resultado, os acadêmicos perceberam a importância a qual os profissionais enfermeiros precisam estar capacitados para o atendimento pré-hospitalar, tema pouco abordado durante a formação acadêmica. Os estudantes, uma vez formados precisam estar capacitados, e isso despertou nos discentes a avidez de aprofundar seus estudos sobre Atendimento Pré-Hospitalar, dos quais foram preparados para atuar em situações de urgência e emergência e estimulados a valorizar o serviço. Além dos ligantes desenvolverem habilidades e confiança, também adquiriram novos saberes para contribuir com a sociedade, enquanto cidadãos, ao passar para população informações sobre o serviço realizado pelo SAMU, tornando-o mais conhecido e valorizado. Conclusão: Dessa forma, o estágio no serviço sob a forma de plantões foi bastante favorável e serviu para o melhor entendimento do conteúdo teórico estudado e para o treinamento prático, e evidenciou que a parceria da LATE-UESC com o serviço do SAMU foi muito significativa e bem-sucedida no aprendizado dos educandos de enfermagem sobre a rede urgências e emergências no Atendimento Pré-Hospitalar.
