The electromyographic properties of the cremaster muscle (CM) are quite different from other skeletal muscles. It shows excessive spontaneous discharges, and the motor unit shape and firing frequency of the CM muscle differ from that of limb muscles. In this study, CM of six adult cadavers and six orchiectomy specimens were used to reveal the detailed histology of the muscle and provide an anatomophysiological explanation for these unusual electromyographic properties. Routine histochemical stains revealed the CM was composed of several distinct bundles of smooth and striated muscle fibers within connective tissue. The smooth muscle fibers that were more profuse than previously known and were not arranged in layers, but widely dispersed between striated muscle fibers. Bielschowsky silver staining technique, anti-neurofilament and anti-synaptophysin immunostaining showed the presence of multiple motor end-plates observed as a series of small dots or lines running along the striated muscle fibers and several nerve endings on a single muscle fiber. Myosin immunostaining confirmed the CM is a slow-twitch muscle, and a-actin smooth muscle immunostaining confirmed the presence of a large number of smooth muscle fibers. There were also small multipolar neurons forming nerve plexuses between smooth muscle fibers. Anti-GFAP immunostaining confirmed the presence of glial cells similar to astrocytes. In conclusion, the findings of this detailed anatomical study showed the CM, widely known as a striated muscle, contains a large number of smooth muscle fibers, and the spontaneous electromyographic discharges are due to the presence of multiple motor end-plates and dense innervation.
Anomalies of the brachial plexus and its terminal branches are not uncommon. Communicating branch arising from the musculocutaneous nerve to the median nerve is a frequent variation, whereas the presence of an anastomotic branch arising from the median nerve and joining the musculocutaneous nerve is very rare. During routine dissection of cadaver upper limbs, we observed an anastomotic branch arising from the median nerve running distally to join with the branches of the musculocutaneous nerve in a left upper extremity. The anastomotic branch originated from the median nerve 11.23 cm proximal to the interepicondylar line. After running distally and coursing between the biceps brachii and the brachialis muscles, this anastomotic branch communicated with two branches of the musculocutaneous nerve separately. The presence of the communicating branches between these nerves should be considered during surgical interventions and clinical investigations of the arm.
68erpes zoster (HZ) is the reactivation of dormant varicella zoster virus (VZV) in the dorsal root ganglia. The commonly involved nerve segments are thoracic, lumbar and trigeminal.1,2 It may also spread to involve the nerve roots adjacent to the dorsal root ganglia causing a plexitis or neuritis.3,4 Postherpetic neuralgia is the most common complication of HZ affecting more than 40% of patients. It typically occurs 2-3 weeks after the herpetic rash and usually affects the myotome that corresponds to the dermatomal distribution. HZ plexopathy may be overlooked in the acute period, or the motor paresis may be mistakenly diagnosed as another cervical or lumbar pathology after the vesicular lesions have disappeared. Three zoster plexopathy cases; two brachial and one lumbar were presented in this article. Electrophysiological study (EDX) findings, limb paresis, pain and erupted skin lesions with positive VZV antibodies indicated HZ plexopathy. EDX revealed patchy involvement of brachial and lumbar plexus. Antiviral treatment and physiotherapy improved muscle strength to mild paresis. Objective: Although there is an easily predictable relationship between psychiatric symptoms and myasthenia gravis, surprisingly few studies have evaluated this correlation. We aimed to screen the symptoms of anxiety and depression in generalized myasthenic patients using practical psychiatric scales and attract attention to the importance of these symptoms. Methods:The correlations between the scores and age, gender, disease duration, intensive care unit experience, medications being used and the frequency of admission/hospitalization, Beck Anxiety , and Depression Scale (BAS, and BDS) scores of 30 patients (21 women), between 48-59 years of age were evaluated retrospectively. Results: The BAS and BDS scores were found to be higher than the normal ranges in more than half of the whole patient group (56% and 60%, respectively). One third of all the patients required medical psychiatric treatment. The disease duration (RS: 0.68 and 0.56,p=0.016, for BAS and BDS,respectively), admission rate (RS: 0.66 and 0.46, p<0.001) and frequency of admission/ hospitalization (RS: 0.64 and 0.48, p<0.001 for BAS and BDS,respectively) were statistically significantly related with the BAS and BDS scores. Conclusion:The symptoms of anxiety and depression may easily mask the myasthenic symptoms, especially at the onset of the disease course leading to delayed diagnosis or misdiagnosis of myasthenia gravis. Furthermore, these symptoms may mimic the myasthenic symptoms in the disease course leading to over-treatments. ÖZETAmaç: Miyotonik Distrofi Tip1 (DM1) miyotoni ve kas güçsüzlüğü ile karakterize beraberinde kardiyak ve endokrin bozuklukların da eşlik ettiği otozomal dominant kalıtım gösteren bir ilerleyici,birden çok sistemi tutan bir nöromüsküler hastalıktır. Santral sinir sistemi bulgularının arasında ciddi bilişsel yıkım, gündüz uykululuk hali ve apati ön plandadır. Bu çalışmada kliniğimizde izlenen DM1 hastalarında iletişimdeki kaybın olaya ilişkin potansiyeller (P300) ile gösterilmesi amaçlanmıştır. Gereç ve Yöntem:Kas hastalıkları polikliniğimizde izlemde olan DM1 tanısı konulmuş 13 hasta ile herhangi bir nörolojik yakınması olmayan 8 kontrol bireyi çalışmaya alınmıştır. Hastaların yaş, cinsiyet, eğitim durumu gibi demografik verileri dosya kayıtlarından çıkartılmıştır. Standart iki tonlu işitsel "oddball" paradigması kullanılarak olaya ilişkin uyarılmış potansiyeller (P300) saptanmıştır. Bulgular: Hastalar ile kontrol grubu arasında yaş, cinsiyet ve eğitim durumu bakımından istatistiksel olarak anlamlı fark saptanmamıştır. Buna karşın hasta grubunda iki hastada P300 potansiyeli hiç uyartılamamış, geriye kalan 11 hastada da P300 latanslarının kontrol bireylerine göre oldukça anlamlı derecede geciktiği görülmüştür (p<0,001).Sonuç: Bu ön çalışmamızdaki bulgular, önceki çalışmalarla uyumlu olarak miyotonik distrofili hastalarda iletişim ve bilgi işleme yetilerinde kayıp geliştiğini telkin etmektedir. DM1 hastalarında hastalığın erken evrelerinde dahi hafif düzeyde olsa bile frontal -prefrontal korteks işlevlerinde bozulma geliştiği bilinmektedir.
