Background: The COVID-19 pandemic has caused devastating diseases worldwide both in children and adults. Subsequently, a serious and novel pediatric condition called children's multisystem inflammatory syndrome (MIS-C) has emerged and it is important to understand the temporal association between MIS-C and COVID-19. In hyper inflammation syndrome following COVID-19, MIS-C, and multi-organ involvement were documented in the pediatric population. The main goal of this study was to assess the relation of the MIS-C with COVID-19.Methods: This single-center case-control study was conducted at Dhaka medical college hospital (DMCH), Dhaka, Bangladesh. Among 1715 studied population, 227 COVID-19 positive pediatric patients were included in this study. Among them 103 had features of MIS-C in the case group and 124 were in the control group without MIS-C.Results: There were 6% MIS-C patients with COVID-19. The prevalence of male patients was observed in this study. The mean age of the patient group with MIS-C was 7.8±3.12 years. The frequencies of fever 93.3 vs 67.7%, p=0.045; conjunctivitis 75.6% vs 53.2%, p=0.039; rash 47.3% vs 16.1%; arterial hypotension 71.3 vs 12.9%, p=0.058 hypoxemia 80.4% vs 57.2%, p=0.049 and other features were significantly higher in MIS-C patients than the patients without MIS-C. More patients with MIS-C had cardiac abnormalities in our study.Conclusions: MIS-C is an emerging clinical entity and this study was focused on the cases of MIS-C in the pediatric population withCOVID-19. In patients with atypical clinical findings and complaints about COVID-19, MIS-C-like illnesses should be considered.
Background: Breast milk is very much essential for proper growth, development and wellbeing of children. They should be breastfed within 1 hour of birth, exclusively breastfed for first six months of life. Breastfeeding difficulties are common. Maternal, infant and social factors are involved in establishment of proper breastfeeding. Objective: To find out the difficulties that a mother face during breastfeeding. Methodology: Cross-sectional observational study was carried out Shaheed Suhrawardy Medical Collage & Hospital (Sh.S.MCH). All children upto 6 months of age and their mother attending in OPD, Breast feeding corner and pediatrics indoor of Sh.S.MCH during study period. Results: Total 19 (38%) infants had difficulty in breast feeding related to maternal factors. Majority (63%) of mother complained of not enough milk, but urine output of infants was ≥6 times within 24 hours in 7 infants of these mothers. Three mothers had stopped breast feeding due to sickness. Total 25 (50%) infants had difficulty in breast feeding related to infant factors. Majority 17 (68%) of them were crying babies, but urine output was ≥6 times in 24 hours in 12 of these infants. Refusal of breast was found in 6 (24%) infants, sickness was cause for stopping breastfeeding found in 2 (8%) infants. Total 13 (26%) infants had difficulty in breast feeding related to social factors. Majority 8 (67%) were working mother and 5 (33%) mothers said that there was lack of familial support. Conclusion: Most of the mothers initiated breast feeding 1 hour after birth. Most of the mothers had given honey as prelacteal feeding to their infants. Most of them were on partial breast feeding. Sixty five percent infants were given formula milk. Difficulties in breast feeding were related to infant factors and maternal factors mostly. More than one factor was present in some infant. EMCJ. A 18 years old boy presented with severe diffuse abdominal pain, constipation, abdominal distention, fever and diagnosed as a case of perforation as free gas noted under both dome of the diaphragm. After resuscitation, exploratory laparotomy was done and diagnosed as a case of multiple spontaneous leaky perforations of jejunum and ileum 25 to 30 cm from duodeno-Jejunal flexure (DJ) up to terminal ileum. Peritoneal toileting and excisional biopsy of the mesenteric lymph node were done. Operative and postoperative period were uneventful. Histopathological examination of the resected lymph node revealed granulomatous inflammation compatible with tuberculosis. Anti-tubercular therapy was started and continued for 6 months. Patient was discharged on the 12 th postoperative day in a good condition. At 3 months of follow up the patient was asymptomatic and thriving well. Free perforation is one of the most feared complications of the intestinal tuberculosis. The terminal ileum is the most common site of perforation, while the majority of (90%) perforations are solitary. [Bangladesh Journal of Infectious Diseases 2016;3(2):52-54]
Peutz-Jeghars Syndrome (PJS), also known as peri-orificial lentiginosis, is a condition of autosomal dominant inheritance. Here, mutation localized at (19p13.3) LKB1/ STK11. It is characterized by presence of mucocutaneous pigmentation and gastrointestinal (GI) hamartomatous polyps. This case of PJS, is a 7 year old girl who came with recurrent vomiting and abdominal pain for 1 year and weight loss for 8 months. She had multiple black pigmentation over lips and buccal mucosa. Endoscopy revealed multiple polyps in stomach and duodenum. Besides supportive management, polyps were removed by surgical intervention. Biopsy of these polyps showed hamartomatous type. Post operative period was uneventful. She recovered well. So far there was no recurrence of pain. She is on regular follow up. J Bangladesh Coll Phys Surg 2019; 37(3): 160-164
