The Chinese Glioma Cooperative Group (CGCG) Guideline Panel for adult diffuse gliomas provided recommendations for diagnostic and therapeutic procedures. The Panel covered all fields of expertise in neuro-oncology, i.e. neurosurgeons, neurologists, neuropathologists, neuroradiologists, radiation and medical oncologists and clinical trial experts. The task made clearer and more transparent choices about outcomes considered most relevant through searching the references considered most relevant and evaluating their value. The scientific evidence of papers collected from the literature was evaluated and graded based on the Oxford Centre for Evidence-based Medicine Levels of Evidence and recommendations were given accordingly. The recommendations will provide a framework and assurance for the strategy of diagnostic and therapeutic measures to reduce complications from unnecessary treatment and cost. The guideline should serve as an application for all professionals involved in the management of patients with adult diffuse glioma and also as a source of knowledge for insurance companies and other institutions involved in the cost regulation of cancer care in China.
The A disintegrin and metalloproteinase 9 (ADAM9) protein has been suggested to promote carcinoma invasion and appears to be overexpressed in various human cancers. However, its role has rarely been investigated in gliomas and, thus, in the current study we have evaluated ADAM9 expression in gliomas and examined the relevance of its expression in the prognosis of glioma patients. Clinical characteristics, RNA sequence data, and the case follow-ups were reviewed for 303 patients who had histological, confirmed gliomas. The ADAM9 expression between lower-grade glioma (LGG) and glioblastoma (GBM) patients was compared and its association with progression-free survival (PFS) and overall survival (OS) was assessed to evaluate its prognostic value. Our data suggested that GBM patients had significantly higher expression of ADAM9 in comparison to LGG patients (p < 0.001, t-test). In addition, among the LGG patients, aggressive astrocytic tumors displayed significantly higher ADAM9 expression than oligodendroglial tumors (p < 0.001, t-test). Moreover, high ADAM9 expression also correlated with poor clinical outcome (p < 0.001 and p < 0.001, log-rank test, for PFS and OS, respectively) in LGG patients. Further, multivariate analysis suggested ADAM9 expression to be an independent marker of poor survival (p = 0.002 and p = 0.003, for PFS and OS, respectively). These results suggest that ADAM9 mRNA expression is associated with tumor grade and histological type in gliomas and can serve as an independent prognostic factor, specifically in LGG patients.
Background: Seizures are a common symptom in patients with temporal lobe gliomas and may result in brain network alterations. However, brain network changes caused by glioma-related epilepsy (GRE) remain poorly understood. Objective: In this study, we applied graph theory analysis to delineate topological networks with resting-state functional magnetic resonance images (rs-fMRI) and investigated characteristics of functional networks in patients with GRE. Methods: Thirty patients with low-grade gliomas in the left temporal lobe were enrolled and classified into GRE (n = 15) and non-GRE groups. Twenty healthy participants matched for age, sex, and education level were enrolled. All participants had rs-fMRI data. Sensorimotor, visual, default mode, auditory, and right executive control networks were used to construct connection matrices. Topological properties of those sub-networks were investigated. Results: Compared to that in the GRE group, four edges with higher functional connectivity were noted in the non-GRE group. Moreover, 21 edges with higher functional connectivity were identified in the non-GRE group compared to the healthy group. All significant alterations in functional edges belong to the visual network. Increased global efficiency and decreased shortest path lengths were noted in the non-GRE group compared to the GRE and healthy groups. Compared with that in the healthy group, nodal efficiency of three nodes was higher in the GRE and non-GRE groups and the degree centrality of six nodes was altered in the non-GRE group. Conclusion: Temporal lobe gliomas in the left hemisphere and GRE altered visual networks in an opposing manner. These findings provide a novel insight into brain network alterations induced by GRE.
Purpose -The purpose of this paper is to investigate two audit committee characteristicsindependence and expertise of the audit committee -and the property-liability insurers' financial reporting quality, which is proxied by loss reserve error. Design/methodology/approach -The authors' hypotheses are tested using multivariate analysis where the loss reserve error is the dependent variable, and audit committee independence, and four types of audit committee financial expertise (accounting, finance, supervisory, and insurance expertise) are the testing variables. Findings -It is found that accounting, finance, and insurance financial expertise are associated with more accurate loss reserve estimate. In contrast, a supervisory financial expertise and an independence audit committee are not found to be associated with better loss reserve quality.Research limitations/implications -The sample includes publicly-held property-liability insurers. Although the results from publicly-held insurers could provide a good laboratory for such investigation in all insurers, they might be limited due to different organization structures of public vs private insurers. Practical implications -The implications of the study are important for the SEC and NAIC. The results suggest that the requirements on the audit committee financial expertise would be necessary, even in highly regulated industry, such as property-casualty insurance. Originality/value -The paper contributes to the extant literature by studying audit committee characteristics in the insurance industry. It also contributes to the extant literature on audit committee effectiveness by decomposing the financial expertise into four types of financial expertise (accounting, finance, supervisory, or insurance expertise) and investigates which (if any) of these four types of expertise really drives the improvement of loss reserve quality.
