Introdução: O transplante de órgãos é uma modalidade terapêutica reconhecida e bem instituída que promove aumento na expectativa de vida na maioria dos casos. O processo envolve múltiplas etapas, iniciando-se com a identificação de um potencial doador e a subsequente notificação à Central Estadual de Transplantes (CET). Nesse contexto, a autorização familiar é peça-chave e indispensável à efetivação da doação no país. No Brasil, em 2020, foram realizados 7.372 transplantes de órgãos sólidos, número 20% menor do que o correspondente ao ano de 2019 (9.217). Em Goiás, a queda foi ainda mais expressiva: de 30% no número de órgãos sólidos transplantados. Objetivos: Analisar as causas de não concretização da doação de órgãos e tecidos em Goiás, no ano de 2020. Material e métodos: Trata-se de um estudo transversal e descritivo acerca dos índices de não concretização da doação de órgãos em Goiás. Os dados foram obtidos a partir da série histórica de doação de órgãos, publicada pela Central Estadual de Transplantes de Goiás, e do Registro Brasileiro de Transplantes (RBT), referentes ao período de janeiro a dezembro de 2020. Resultados: Após a análise dos dados, evidenciou-se que apenas 23,18% das notificações de morte encefálica (ME) evoluíram para uma doação no ano de 2020 no estado de Goiás. Nesse contexto, os resultados atestam a negativa familiar como a maior causa da não efetivação da doação de órgãos, uma vez que essa representou 44,7% dos motivos de não concretização da doação. A segunda razão mais prevalente foi a contraindicação clínica (27,06%), seguida pelo não diagnóstico de morte encefálica (21,42%). No Brasil, segundo dados da Associação Brasileira de Transplante de Órgãos (ABTO), a porcentagem de recusa familiar das entrevistas em 2020 foi de 37%. Conclusão: A partir dessa análise, constata-se a importância da recusa familiar para a manutenção do baixo percentual de doadores efetivos. A alta porcentagem da negativa é, em grande parte, resultado do desconhecimento a respeito do processo de doação. Urge, portanto, a necessidade de desenvolver e apoiar estratégias de comunicação em massa, bem como direcionadas para a população em idade escolar.
