con tratamiento ortodóncico previo y ausencia de primeros premolares inferiores. El motivo principal de consulta es la apariencia facial poco atractiva debido a la posición hacia adelante de la mandíbula y la mordida abierta anterior. A la revisión clínica extraoral, en la vista frontal se observa una cara leptoprosopa, áreas paranasales colapsadas y labios incompetentes. En las proporciones faciales verticales, reveló el tercio inferior aumentado. La vista lateral y oblicua mostró mayor proyección del mentón, perfi l cóncavo y una longitud mentocervical aumentada. Intraoralmente, la relación molar clase III, mordida abierta anterior de 2 mm y mordida cruzada anterior completa de 1 mm. Ligera discrepancia en la línea media dental. El tratamiento ortodóncico se lleva a cabo en tres fases: prequirúrgica, quirúrgica y postquirúrgica. En la fase prequirúrgica se descompensó dentalmente al paciente y se preparó para la cirugía. En la fase quirúrgica se realizó una segmentaria de maxilar de lateral a lateral para realizar un movimiento de rotación sagital proyectando la zona nasogeniana y mejorando la inclinación de los incisivos anteriores en combinación con retroceso mandibular mediante osteotomía sagital bilateral. En la fase postquirúrgica se lograron relaciones oclusales ideales, en términos de clase canina, clase molar, resalte, sobremordida, y la coincidencia de las líneas medias dentales así como un perfi l más armónico.
La impactación es definida como una falla en la erupción dental causada por un obstáculo físico. Las alteraciones en la erupción de los segundos molares permanentes se han reportado como bastante raras. Objetivo: Determinar la prevalencia de impactación de segundos molares permanentes, en pacientes que solicitaron atención al Servicio de Ortodoncia durante el periodo de agosto de 2015-junio de 2017. Material y métodos: Se revisaron 2,968 radiografías panorámicas y se identifi caron 31 pacientes que presentaban segundos molares permanentes impactados, de las cuales se obtuvo una muestra de 41 segundos molares impactados. Con la información obtenida se determinó la prevalencia por edad, sexo, biotipo facial y clase esquelética, de igual forma se recabó información que permitió identifi car la frecuencia en relación con cada uno de los cuadrantes. Resultados: La prevalencia de segundos molares permanentes impactados es del 1.4%; la distribución por sexo demuestra que la mayor prevalencia corresponde al sexo femenino (53.7%) y para el masculino la prevalencia fue del 46.3%. Conclusiones: La relación con el biotipo facial y la clase esquelética infl uye directamente en la prevalencia de impactación al igual que los factores locales como el desarrollo y posición del tercer molar, el espacio entre la superfi cie distal del primer molar y el borde anterior de la rama mandibular, así como el desarrollo y tipo de crecimiento de la mandibular.
<p> </p><p><span> </span><strong>Introducción: </strong>La estética de la sonrisa es importante en la autopercepción de cualquier persona; sin embargo, evaluar la estética de la misma se vuelve subjetivo. En odontología se han establecido ciertos parámetros para una valoración más objetiva; uno de éstos es el ángulo de inclinación de los incisivos superiores. <strong>Objetivo: </strong>Determinar si la inclinación incisal superior influye en la percepción estética de la sonrisa, en una vista de perfil, por parte de un grupo de expertos en el área de Ortodoncia, rehabilitación oral y de gente común. <strong>Material y métodos: </strong>Se seleccionó a una estudiante de postgrado de la UNAM de 30 años de edad, originaria de México; presentaba perfil recto y armónico, líneas medias coincidentes, clase I esqueletal, molar y canina y una inclinación del incisivo superior en norma. Se tomó una fotografía de perfil y se realizaron siete modificaciones en la angulación de los incisivos superiores de cada fotografía. Se realizaron 90 encuestas en la CDMX. En el análisis estadístico se ejecutaron las pruebas de ANOVA, y Fisher & Tuckey. <strong>Resultados: </strong>Existió una diferencia estadísticamente significativa entre los tres grupos en la percepción estética de la sonrisa de acuerdo con el grado de inclinación incisal. No hubo soporte científico para asignar a un grado de inclinación específico como mejor. <strong>Conclusiones: </strong>La profesión del encuestado determina la apreciación de la sonrisa y la inclinación del incisivo superior influye en la apreciación estética de una sonrisa vista de perfil.</p>
