It may be concluded that the LED therapy protocol established for this in vivo study was effective in reducing the severity of oral mucositis, although the oral lesions were not completely prevented.
OBJETIVO: Discutir os aspectos das urgências odontológicas relacionadas aos traumatismos dentários, disponibilizando mais informações para médicos pediatras ou plantonistas de serviços de atendimento de urgências e emergências. FONTES DE DADOS: O levantamento dos dados foi realizado na base de dados Pubmed e Bireme, selecionando os artigos dos últimos 13 anos. As palavras-chave utilizadas foram: traumatismo dentário, dente decíduo e dente permanente. Os critérios de inclusão utilizados foram: artigos em inglês e português sobre incidência, prevalência e etiologia, guias de procedimentos e casos clínicos apenas de traumatismo dentário, sendo excluídos artigos de clareamento de dentes traumatizados, traumas faciais ósseos e casos clínicos de acompanhamento reduzido. SÍNTESE DOS DADOS: Os dados foram descritos de forma concisa para se tornar um guia de fácil leitura e rápido acesso em relação à conduta, necessidade de atendimento imediato e correta escolha de soluções para armazenagem dos dentes e fragmentos. CONCLUSÕES: O conhecimento sobre o assunto, a agilidade no tratamento de urgência e o correto encaminhamento do paciente proporcionam melhor prognóstico.
RESUMOObjetivo: Apresentar revisão de literatura sobre dentes natais e neonatais, abordando características clínicas, fatores etiológicos, medidas terapêuticas e a importância do conhecimento desta anomalia, por odontopediatras e pediatras. Fontes de dados:Foram selecionados os artigos mais relevantes sobre o tema, desde 1950 até 2006, pesquisados no Medline e na Bibliografia Brasileira em Odontologia (BBO), além de livros de pertinentes.Síntese dos dados: Os dentes natais e neonatais consistem em uma anomalia de erupção, sendo caracterizados por seu irrompimento na cavidade oral durante o período intra-uterino ou no primeiro mês de vida respectivamente, podendo fazer parte da dentição decídua normal ou supranumerária. Esses dentes, em geral, apresentam bordos cortantes e podem estar relacionados ao aparecimento de ulcerações na base da língua do bebê e/ou no seio materno, comprometendo a amamentação. A fraca implantação óssea desses dentes favorece sua grande mobilidade, tornando-se, assim, um fator de risco à sua aspiração ou deglutição pela criança. A abordagem terapêutica depende da dentição à qual pertence o dente e dos possíveis problemas que este pode causar à saúde da criança ou da mãe.Conclusões: O conhecimento sobre as características clínicas e os possíveis distúrbios aos quais os dentes natais e neonatais estão relacionados por odontopediatras e pediatras possibilita a interação necessária para o diagnóstico precoce e a abordagem integral da criança.Palavras-chave: dente decíduo; dentes natais; Odontopediatria; Pediatria. AbStRActObjective: To review available data about natal and neonatal teeth, emphasizing clinical characteristics, etiological factors, treatment, and the importance of the knowledge about this anomaly by pediatric dentists and pediatricians.Data sources: The most relevant articles published on the subject were selected, from 1950 to 2006, browsed on Medline, Brazilian Bibliografy of Odontology (BBO) and Dentistry books.Data synthesis: Natal and neonatal teeth are considered eruption anomalies, characterized by eruption into oral cavity during the intrauterine period or during the first month after birth, respectively, being part of the regular deciduous or the supernumerary dentitions. As these teeth usually present cutting edges, they can be related to traumatic injury to newborn's tongue and/or to mother's nipples, compromising breastfeeding. In general, the weak attachment of these teeth to alveolar bone favors a high degree of mobility, which exposes the infant to the risk of aspiration or swallowing them. The management depends on which dentition these teeth belong and/or on complications that may affect either infant's or mother's health.Conclusions: The knowledge about the clinical characteristics and the possible complications related to natal and neonatal teeth by pediatric dentists and pediatricians provides the necessary interaction to early diagnosis and infant general management. Dental trauma is a public health problem because of its high prevalence rates and treatment costs and for affecting mainly younger individuals. Aim: This cross-sectional study aimed to investigate the variables associated with dental trauma and its influence on oral health-related quality of life among 7th-grade students of public elementary schools. Methods: The data was collected with semi-structured and self-administered questionnaires and an intraoral clinical examination (n = 204). The variables collected regarded dental trauma, lip coverage, overjet, oral health-related quality of life (Child Perceptions Questionnaire - CPQ11-14 ISF: 8), and sociodemographic profile. Association tests and the Mann-Whitney test were performed at a 5% significance level. Results: Dental trauma occurred in 12.3% of students, 94.6% had adequate lip coverage, and the mean overjet was 2.7 mm (±2.0). Sex was the only variable with a statistically significant association with dental trauma (p = 0.021). There was no statistically significant difference in the CPQ11-14 ISF: 8 score between students with and without dental trauma (p = 0.136). Conclusions: Dental trauma was significantly associated with sex and did not impact the oral health-related quality of life of the students.
Research on knowledge regarding tooth avulsion and replantation among children and adolescents is still scarce. Objective: This cross-sectional study aimed to evaluate the knowledge about tooth avulsion and replantation and their association with independent variables among 7th-grade students in public schools. Material and methods: Data collection on the knowledge regarding tooth avulsion and replantation (nine questions) and sociodemographic profile was performed through semi-structured and self-administered questionnaires. The presence of trauma in permanent incisors of students (n=251) was obtained in a clinical examination performed by a calibrated examiner in Sertaozinho, Sao Paulo, Brazil. After a descriptive data analysis, association and multiple logistic regression tests were applied at a significance level of 5%. Results: Most of the students were female (61.4%), were residents in suburban areas (89.6%) and did not present dental trauma (87.7%). Students answered correct 42.2% of the questions. The variable “last visit to the dentist” was the only one that showed a statistically significant association with knowledge about tooth avulsion and replantation in the bivariate analysis (p = 0.007) and the logistic regression (p = 0.031). Conclusion: The level of knowledge was low, and the students whose last visit to the dentist was two years or longer showed a greater chance of having less knowledge about tooth avulsion and replantation. Clinical Significance: During routine dental visits, the dental team should educate patients about the procedures to manage different situations of dental trauma.
O padrão das lesões de cárie tem se modificado ao longo das décadas. A determinação de presença ou ausência de lesão baseada somente no exame clínico visual torna o diagnóstico subestimado, uma vez que lesões dentinárias sob esmalte não cavitado, ou minimamente desmineralizado, apresentam-se com maior frequência na clínica odontológica. O objetivo do presente trabalho é relatar o caso clínico de uma criança que apresentava um molar superior com aspecto oclusal duvidoso, devido à presença de hiperplasia pulpar assintomática emergindo do sulco disto-lingual. Foi realizado exame radiográfico periapical, o qual foi determinante para a elaboração do diagnostico de cárie oculta e do plano de tratamento. Como abordagem do caso, optou-se pela remoção do tecido cariado e pela realização de pulpotomia com hidróxido de cálcio. A execução de um exame clínico minucioso associado ao exame radiográfico, em dentes nos quais se torna duvidosa a detecção imediata da presença de cárie dentária oculta, é de extrema importância para dentes cuja presença desse tipo de cárie seja duvidosa. Com essa associação, esse tipo de lesão pode ser diagnosticada o mais precocemente possível, reduzindo a necessidade de intervenções radicais e possibilitando o oferecimento de procedimentos menos invasivos no cuidado à saúde bucal de pacientes pediátricos.
