Introduction A radiological evaluation is essential in endodontics, for diagnostic purposes, planning and execution of the treatment, and evaluation of the success of therapy. The periapical radiography is nowadays the main radiographic investigations used but presents some limits as 3D anatomic alteration, geometric compression, and possible anatomical structures overlapping that can obscure the area of interest. CBCT (cone beam computed tomography) in endodontics allows a detailed assessment of the teeth and surrounding alveolar anatomy for endodontic diagnosis, treatment planning, and follow-up. Objective The purpose of this study was to evaluate the accuracy of CBCT in comparison with conventional intraoral radiographs used in endodontic procedures. Materials and Methods Statistical analysis was performed on 101 patients with previous endodontic treatments with the relative radiographic documentation (preoperative, postoperative, and follow-up intraoral X-ray) that had underwent at CBCT screening for surgical reasons. The CBCT scans were evaluated independently by two operators and compared with the corresponding periapical images. Results Our analysis shows that the two radiological investigations statistically agree in 100% of cases in the group of patients without any endodontic sign. In the group of patients with an endodontic pathology, detected with CBCT, endodontic under extended treatments (30.6%), MB2 canals in nontreated maxillary molars (20.7%), second canals in nontreated mandibular incisors (9%), root fractures (2.7%), and root resorption (2.7%) were not always visible in intraoral X-ray. Otherwise, positivity in the intraoral X-ray was always confirmed in CBCT. A radiolucent area was detected in CBCT exam in 46%, while the intraoral X-ray exam was positive only in 18%. Conclusions Our study shows that some important radiological signs acquired using CBCT are not always visible in periapical X-ray. Furthermore, CBCT is considered as a II level exam and could be used to solve diagnostic questions, essential to a proper management of the endodontic problems.
This case study concerns a patient who had lost all of his teeth, needing a rehabilitation with total prosthesis, who went to the dentist looking for help to overcome psycho-physical trauma and to overcome functional and social problems related to being a prosthesis wearer. Tooth loss occurs most in old age, even if it is not a direct consequence of aging. The rehabilitation of oral functions allows the patient to speak, chew, smile and feel confident in his own aesthetics and therefore improve, even a lot, his well-being in social relations. It is very important in oral rehabilitations to evaluate their type and therapeutic timing. This study stems from the idealization of a new protocol to simplify the supported oral rehabilitations. In this manuscript, a patient was considered and shown according to a complete photographical documentation all the phases. Rehabilitation included the use of Osstem (Osstem, Seoul, Korea) and equator type abutments (Rhein83, Bologna, Italy). This manuscript claims to represent the first of a whole series of cases demonstrating the utility of this protocol.
In recent years, a major research goal of companies has been to create mechanical components suitable for rehabilitation that are safer and more reliable. Evaluating their biomechanical features could be a way to improve them. The purpose of this study was to evaluate the different biomechanical features of low-profile retentive systems (Rhein®). Two different attachment systems were tested: OT Equator® Smart Box and Locator® R-TX. Once a machine was created for the simulation of the connection and disconnection of the attacks in a combined manner, it was possible to evaluate these parameters over time. Attachments were mounted in two different configurations of the divergence angle: 10° and 50°. The drop retention force proved to be stable over time. The Locator® R-TX attachment experienced a more rapid decrement of the retention force than the OT Equator® Smart Box. Both tested systems experienced a high drop in retention; this drop tended to stabilize after 1.5 years of use, and it was correlated with the divergence angle. The OT Equator® Smart Box system underwent this loss of retention more gradually than the Locator® R-TX. This study demonstrates preliminary results from a bioengineering and biomechanical point of view, providing useful information for the continuous improvement of these devices and, therefore, for the quality of patients' oral health.
The developments in the field of rehabilitation are proceeding hand in hand with those of cybernetics, with the result of obtaining increasingly performing prostheses and rehabilitations for patients. The purpose of this work is to make a brief exposition of new technologies regarding composites materials that are used in the prosthetic and rehabilitative fields. Data collection took place on scientific databases, limited to a collection of data for the last five years, in order to present news on the innovative and actual materials. The results show that some of the most commonly used last materials are glass fibers and carbon fibers. Even in the robotics field, materials of this type are beginning to be used, thanks above all to the mechanical performances they offer. Surely these new materials, which offer characteristics similar to those in humans, could favor both the rehabilitation times of our patients, and also a better quality of life.
This study evaluates salivary immunoglobulin A (s-IgA) and interleukin 6 (IL-6) in saliva of children and its correlation to tooth decay severity. Fifty-nine patients were divided into two groups: caries free (A group) and caries active (B group). B group was investigated according to Mount and Monse indices. Mean salivary IgA rate between two groups (A 16.7 ± 4.5 mg/dL vs. B 21.8 ± 12.9 mg/dL) was not significant, while IL-6 rate (A 19.02 ± 5.3 pg/mL vs. B 30.2 ± 11.8 pg/mL) was statistically different. This study revealed that salivary IL-6 levels were significantly higher in children with active caries when compared with the caries-free group, while the s-IgA rate showed no significant differences between the two groups.
BACKGROUND: Instagram statistic has attracted roughly one billion, monthly active users. In 2021, Instagram belonged to the most popular social networks worldwide. It has been considered an effective tool that contributes to the contemporary sharing of information for raising public awareness while providing educational information. The growing presence of Instagram and frequent user engagement has made it a potentially effective platform for patient communication, seeking educational information, product information for consumers, and advertisements in the form of images and videos. OBJECTIVE: To assess and compare the contents of Instagram posts by healthcare professionals (HP) and non-professional healthcare workers (NPHW) on bruxism and to assess public engagement with this content. METHODS: Twelve “hashtag” terms linked to bruxism were searched. HP and NPHW analyzed the content of relevant posts for the presence of domains. Discourse analysis assessed the post quality for themes. We conducted the descriptive and univariate statistical analysis, whereas Inter-rater reliability was tested using Cohen’s kappa. RESULTS: A total of 1184 posts were retrieved, with the majority uploaded by NPHW (622 posts). The posts by HPs were in Text and image(s) format (53%) with the range of 25–1100 Instagram post “Likes.” “Mouthguard” (90) % was the most frequently included domain posted by HP, followed by “treatment plan/pain management, and complaints of clicking or locking of TMJ (84%). Greater number of domains (p= 0.03) were seen in the posts by NPHWs, compared to HP’s having more bruxism-related content. The inter-rater reliability method (0.89) was used for the presence of domains. CONCLUSION: NPHW uses Instagram more frequently to post bruxism-related information than HP. HPs must ascertain that the content posted by NPHW is relevant and the concerns addressed in posts are to the purpose.
Objective:The aim of this retrospective analysis is to describe and to evaluate the middle third horizontal root fractures, long term clinical management results and to estimate the effect of treatments factors upon healing and survival rate.Methods:Our clinical study included 42 patients presenting a middle third horizontal root fracture in permanent dentition. For each patient at t0 the parameters recorded were: diastasis, mobility, sensibility, periodontal inflammation, pulpal pathology, associated fracture and dislocation of the coronal fragment. The follow-up was performed after 6 (t1), 12 (t2) and 36 (t3) months after the trauma, both clinically and radiologically. Clinical examination, vitality tests and a radiological evaluation (periapical x-ray) were performed.Results:At t0 it was observed: diastasis (14, 3%), mobility (28, 6%), thermal sensibility (61, 9%), periodontal inflammation (4, 8%), pulpal pathology (38, 1%) and dislocation of the coronal fragment (47, 6%) of the patients. The treatment plan started with the coronal fragment repositioning and the blockage (splint) with the adjacent teeth in 47, 6% of cases.A root canal treatment was performed at t0 in 52,4% of the fractured teeth. Statistical analysis showed the highest level of significance between pulpal lesions (t0) and associated fractures. The mobility, sensibility and pulpar lesions parameters, showed a reduction in relation to the follow up timing, with a main variation remarkably evident between t0 and t1. The ratio between observation time and the presence of diastasis was statistically significant.Conclusion:The analysis of the clinical results exhibits the high success rate of a conservative approach in the treatment of teeth fractured in the middle third of the root.
The orthodontic tooth movement is the last step of several biological processes that take place after the application of external forces. During this process, dental pulp tissue is subjected to structural and protein expression modifications in order to maintain their integrity and functional morphology. The purpose of the present work was to perform an in vivo study, evaluating protein expression modifications in the human dental pulp of patients that have undergone orthodontic tooth movement due to pre-calibrated light force application for 30 days. Dental pulp samples were extracted from molars and premolars of the control group and after 7 and 30 days of treatment; the samples were then processed for immunofluorescence reactions using antibodies against fibronectin, collagen I and vascular endothelial growth factor (VEGF). Our results show that, after 7 days of treatment, all tested proteins change their pattern expression and will reset after 30 days. These data demonstrate that the dental pulp does not involve any irreversible iatrogenic alterations, supporting the efficacy and safety of using pre-calibrated force application to induce orthodontic tooth movement in clinical practice.
