As exostoses são crescimento ósseos benignos, de origem desconhecida, que acometem a mandíbula e a maxila, sendo as alterações esqueléticas vistas como variantes assintomáticas normais mais comuns em adultos. Objetivos: Descrever a cirurgia de exostose maxilar na etapa pré-protética de um paciente do sexo masculino, 70 anos, edêntulo total. Metodologia: Trata-se de um estudo de abordagem descritiva do tipo relato de caso, realizado na clínica odontológica do curso de Odontologia da FAESA - Centro Universitário, no município de Vitória-ES. O planejamento incluiu preparo pré-operatório com antibiótico profilático seguido de protocolo verbal de redução de ansiedade. Foi realizado alveoloplastia nas regiões com remanescentes ósseos com broca específica para osso e instrumental de forma manual. Conclusão: A alveoloplastia é uma opção terapêutica viável sempre que tenha indicação e com adequado planejamento, uma vez que, tornou possível ao paciente, o uso das próteses totais de maneira satisfatória, devolvendo função, estética e qualidade de vida.
