Pixberg u. a.: Vergleichende Bestimmung des effektiven Nierenpiasmastroms mit PAH und o.13J.Hippursäure 1 17 Vergleichende Untersuchungen über die Bestimmung des effektiven Nierenpiasmastroms mit PAH und o-'31J-Hi ppursäure Die neu synthetisierte Substanz o-131J-Hippursäure wurde auf ihre Stabilität sowie auf ihre Eiweiß-und Erythrozytenbindung geprüft. Mit chromatographischen und autoradiographischen Methoden wurde eine 131J-Abspaltung von etwa 10/o ermittelt. Das Hippursäuremolekül erwies sich unter In-vivo-und In-vitro-Bedingungen als stabil. Die Eiweißbindung, geprüft mit der Säulenfraktionierung, lag bei 3°/o, die Erythrozytenbindung bei 8,4°/o. Der Quotient Co.l3lJHippursäure /CPAH betrug 0,85. Eine kompetitive Hemmung der o-131J-Hippursäure durch PAH konnte nicht bestätigt werden. Während die Messung des Glomerulusfiltrates mit der endogenen Kreatinin-Clearance ein fester Bestandteil der Nierendiagnostik ist, fehlt bislang ein entsprechend einfaches Verfahren zur Bestimmung des effektiven Nierenplasmastroms. In der Routinediagnostik kann die p-Aminohippursäure-(PAH-)Clearance wegen des großen zeitlichen Aufwandes sowohl für die Untersuchung als auch für die chemische Analyse nicht durchgeführt werden. Eine erste Vereinfachung liegt in der Verwendung radioaktiver Substanzen für die Clearance-Untersuchung. Die Analysenzeit läßt sich dadurch erheblich verkürzen und die Meßgenauigkeit erhöhen. Für die Bestimmung des effektiven Nierenplasmastroms wurde vorwiegend o-131J-Hippursäure verwandt. Vergleichende Untersuchungen zwischen PAH und o-131J-Hippursäure wurden von mehreren Autoren vorgenommen. Ihre Ergebnisse waren nicht einheitlich. Die Hauptursache für die wechselnden Ergebnisse sah man in der Instabilität des o-131J-Hippursäure-Moleküls. Vor ungefähr zwei Jahren gelang eine neue Synthese der o-131J-Hippursäure, die diese Unsicherheitsfaktoren auf ein Minimum reduzierte. Aus diesem Grunde waren neue Untersuchungen notwendig, um zu Vergleichswerten mit der PAH-Clearance zu kommen. Weiterhin sollte versucht werden, die Ursache für die von früheren Untersuchungen bekannte niedrigere o-131J-Ilippursäure-CIearance im Vergleich zur PAH-Clearance zu klären. Methodik Die Substanz lag in einer spezifischen Aktivität von 0,1 mCi ± 100/o 131J/mg o-Jodhippurat mit einer Konzentration von 1 mg o-Jodhippurat/mi (Farbwerke Hoechst AG.) vor. Die Untersuchungen wurden morgens am nüchternen Patienten durchgeführt. Eine Stunde vor Untersuchungsbeginn erhielten die Patienten 500-700 ml Flüssigkeit. Blut entnahmen wir aus der Vena cubitalis. Die Blase wurde mit Hilfe eines Blasenkatheters und Luftspülungen entleert. Die Dosierung von PAH und o--'1J-Hippursäure erfolgte nach den Angaben von Brod (4). Die Substanzen wurden mit einer Präzisionspumpe (Braun-Perfusor) in einer Flüssigkeitsmenge von 2 ml/mm infundiert.
