La invaginación intestinal en el adulto es una patología poco frecuente, por lo general secundaria a una tumoración que actúa de cabeza invaginante.
Caso clínicoSe trata de una mujer de 69 años sin alergias medicamentosas conocidas, con antecedentes de apendicectomía en la infancia y gastritis tratada con inhibidores de la bomba de protones, que acude a Urgencias por presentar epigastralgias de un mes y medio de evolución cuya intensidad ha aumentado en los últimos días, acompañadas de náuseas y vómitos biliosos ocasionales. El dolor era de tipo cólico en hemicinturón y la paciente, afebril, refería estreñimiento más evidente en las últimas semanas con una última deposición normal en el momento del ingreso. En la exploración física el abdomen era blando y depresible, doloroso a la palpación sobre todo en epigastrio, sin defensa ni peritonismo. La analítica mostraba 10.100 leucocitos con un 77% de neutrófi-los y un 15% de linfocitos. En las radiografías simples de abdomen se observaba distensión de asas de intestino delgado de predominio centroabdominal con niveles en la proyección en bipedestación, patrón radiológico sugestivo de oclusión mecáni-ca. A continuación y con el fin de descartar patología tumoral del cecoascendente, se practicó un enema opaco que mostró la ausencia de patología colónica. La incompetencia de la válvula ileocecal permitió la opacificación del íleon pelviano en el que se observó un defecto de repleción ovalado persistente con componente de invaginación sugestivo de patología tumoral (Fig. 1). En la laparotomía se halló una tumoración invaginada que se resecó junto a 20 cm de intestino delgado realizándose una anastomosis terminoterminal. La paciente evolucionó de forma correcta y fue dada de alta a la semana de la intervención. El estudio anatomopatológico de la pieza quirúrgica identificó un pólipo fibroide inflamatorio (PFI) de 3 cm de diámetro que ocupaba la totalidad de la luz del segmento intestinal resecado.
DiscusiónLa oclusión del intestino delgado en el adulto suele deberse a la presencia de adherencias en pacientes con antecedentes de cirugía abdominal y a la existencia de hernias intestinales si no existen intervenciones previas. La invaginación intestinal como causa de oclusión es poco frecuente y su etiología suele ser tumoral.El PFI es una tumoración rara y existen unos trescientos casos descritos en la literatura.
