Estimado director: Hay pocos temas que resulten tan controvertidos como la cirugía bilateral de cataratas. Sin embargo, para los autores firmantes se trata de una práctica aplicada diariamente a nuestra rutina quirúrgica desde hace varios años. Aunque no es éste el lugar indicado para hablar de requisitos, de cómo evitar complicaciones, de seleccionar a los pacientes (escasos) que deben ser intervenidos unilateralmente…, sí debemos decir que cuando hablamos de cirugía bilateral nos referimos a aquélla en la que intervenimos al paciente de ambos ojos el mismo día, en la misma sesión quirúrgica; y las premisas fundamentales para llevarla a cabo son: 1. Sólo debe realizarse por cirujanos realmente experimentados. 2. Sólo debe realizarse en servicios con poca o nula circulación del personal, que además debe ser entrenado para entender lo que estamos haciendo. 3. El material quirúrgico utilizado para cada ojo debe ser totalmente diferente (mesa quirúrgica, guantes, instrumental, ...).Actualmente acumulamos más de 20.000 cirugías bilaterales sin la existencia de complicaciones directamente derivadas de la bilateralidad. Estamos finalizando el mayor estudio prospectivo y randomizado que se haya realizado sobre el tema, comparando las complicaciones intraquirúrgicas y postquirúrgicas, y las ventajas económicas, junto con cuestionarios sobre la calidad de vida (Euroquol y VF14), entre la cirugía bilateral y la unilateral (intervenir ambos ojos en diferentes días) de cataratas, auspiciado por un organismo sanitario oficial. No pretendemos con ello imponer a ningún compañero el camino a seguir. Sólo pretendemos que el resto de la sociedad oftalmológica lo valore como una alternativa más e igual de válida que la práctica unilateral.A pesar de que hemos intentado difundir el tema en los foros nacionales, todavía se nos escucha con la actitud de quien observa algo anecdótico, curioso e incluso peligroso. Nada más lejos de la realidad; aunque la cirugía unilateral puede considerarse como la norma, es decir, la más practicada, cada día más cirujanos se suman a la bilateral. Actualmente está en marcha un movimiento internacional liderado por Steve Arshinoff: Acaba de ser fundada la iSBCS (Sociedad internacional de cirugía bilateral de cataratas), el 1 de septiembre del año en curso, del que los autores de este escrito formamos parte como socios fundadores, y en la que se incluyen numerosos cirujanos de todo el mundo, sumando una experiencia acumulada cercana a las 100.000 intervenciones. El 13 de septiembre fue el primer meeting, en Berlín. Existe una página web de la sociedad (www.isbcs.org), y desde hace años se imparten cursos sobre el tema en los más prestigiosos congresos internacionales (acaba de impartirse uno en la ESCRS en Berlín, el próximo será en la ESCRS de Barcelona, en septiembre, del que somos co-moderadores).
