Unter irritablem Kolon versteht man ein Symptomenbild von intermittierend auftretenden, mehr oder weniger ausgeprägten abdominellen Beschwerden, die vorwiegend vom Dickdarm ausgehen und für die keine faßbare organische Ursache vorhanden ist. Das Leiden ist häufig, die Prognose ad sanationem dubiös, quoad vitam gut.Das Syndrom des irritablen Kolons ist schon seit Anfang des vergangenen Jahrhunderts bekannt, obwohl es differentialdiagnostisch nur unzulänglich umrissen werden konnte. Die erste Beschreibung soll Jan Howship 1820 (31) gegeben haben. Da Costa prägte 1871 dafür die Bezeichnung »membranöse Enteritis« (12). Weitere Namen wie Darmneurose (9), spastische Obstipation (17) oder neurogene muköse Kolitis (4) folgten. Von Spriggs (1930) wurde wahrscheinlich erstmals die Bezeichnung «irritables Kolon« verwandt (54). Sie setzte sich fast allgemein durch, obwohl die Vielfalt des Beschwerdekomplexes beim irritablen Kolon durchaus nicht nur einem «Reiz« gleichkommt (20). Man weiß heute noch nicht sicher, inwieweit der Dünndarm bei dem Beschwerdebild zusätzlich beteiligt ist, weswegen auch die Bezeichnung »irritabler Darm« vorgeschlagen wurde (14).
