El concepto que tenemos los oftalmólogos de la retinopatía diabética es en gran parte intuitivo.Sánchez Salario dice en una monografía que sobre Retinopatías Vasculares presentó a la Sociedad Española de Oftalmología (Vitoria, 1971): "Lo patognomónica no son las lesiones, sino la agrupación de ellas".Sin embargo, a pesar de esta falta de especificidad, merece la pena estudiar los signos que componen el cuadro de la retinopatía diabética.Unicamente con fines didácticos establecemos entre ellos un orden, sin que se presuponga nada respecto a su aparición, cronología, e incluso a su valor pronóstico, ya que aunque en la mayoría de los cuadros oftalmoscópicos las lesiones suelen asociarse, no son en absoluto infrecuentes las llamadas formas puras, en las que una sola lesión está presente o domina el campo.Las lesiones "elementales" de la retinopatía diabética son:-Las alteraciones del árbol venoso. -Los microaneurismas.-Las hemorragias retinianas.-Los exudados retinianos y -La proliferación o neoformación, ya sea de vasos (neoformación vascular), de tejido conectivo (neoformación tisular) o más frecuente mixta. Alteraciones del árbol venosoEn líneas generales, podemos clasificar estas alteraciones en dos grandes grupos con diferenciación cronolóe:ica.~l. Alteraciones precoces. 2. Alteraciones tardías. Las alteraciones precoces consideradas por los autores como signos prodrómicos de la auténtica retinopatía diabética, son sujeto de fuertes polémicas, ya que su identificación se realiza casi siempre de forma retrospectiva.Sujetos diabéticos, en general jóvenes, que más tarde desarrollan otros signos, presentan en los estadios iniciales una distensión o turgencia de las venas y sus ramas principales, con más frecuencia que en la pobla-
