Contexte : Le cancer de la vésicule biliaire, relativement fréquent en Algérie, est réputé pour son pronostic sombre. La survie à cinq ans tous stades confondus est infé-rieure à 5 %. Depuis 12 ans, nous avons adopté une attitude chirurgicale résolument agressive vis-à-vis de ce cancer. Cette chirurgie radicale qui consiste en bisegmentectomie IV-V + lymphadénectomie pour les stades invasifs est élargie selon l'extension tumorale rencontrée aux organes de voisinage. Une chimiothérapie adjuvante y est associée en postopératoire. Cent trente patients ont ainsi bénéficié d'une exérèse à visée radicale. L'hépatectomie a été élargie à un ou plusieurs organes de voisinage chez 58 patients (44,5 %). L'atteinte ganglionnaire a été retrouvée chez 67 patients (51,5 %). Nous déplorons une lourde morbimortalité respectivement de 28,5 et 18,5 %. Soixante-six patients (62,3 %) parmi les survivants à l'intervention ont bénéficié d'une chimiothérapie systémique. La survie globale à cinq ans est de 16 % avec une médiane de 17 mois. Conclusion : Ces résultats nous incitent à poursuivre dans cette voie, mais des efforts sont à fournir pour diminuer la mortalité postopératoire en premier lieu en améliorant la prise en charge des patients ictériques et en sélectionnant mieux ceux avec atteinte multiviscérale. Five-year survival for all stages is less than 5%. For the past 12 years, we have adopted an aggressive surgical approach against this carcinoma. Procedure: This radical surgical approach includes a bisegmentectomy for stages IV-Vand extensive lymphadenectomy for invasive stages. This resection can be extended to all other organs which may be involved in the tumour (44.5%). Systemic adjuvant chemotherapy was administered postoperatively. Nodal infiltration was noted in 67 cases (51.5%). Results: Morbidity and mortality were 28.5% and 18.5%, respectively. Overall survival at 5 years was 16%, with a median of 17 months. Conclusions: These results (median and overall survival) encourage us to continue in this way, but a lot of effort must be made to reduce postoperative mortality and improve the management of patients with jaundice and those with the involvement of multiple organs.
