Abstract. Despite advances in surgery, radio-and chemotherapy, therapeutic approaches for patients with head and neck squamous carcinoma (HNSCC) need to be improved. Immunotherapies eliciting tumor specific immune responses might constitute novel treatment options. We therefore investigated the expression and immunogenicity of two tumor-associated antigens (TAA) the receptor for hyaluronic acid mediated motility (RHAMM) and carboanhydrase IX (G250/CAIX) in HNSCC patients. Twenty-two HNSCC samples were examined for the expression of RHAMM and G250 by Western blotting and immunohistochemistry, 14/22 samples were tested for HLA-A2 expression by flow cytometry. For 8/22 samples single tumor-cell suspensions were generated, and mixed lymphocyte peptide cultures (MLPC) were performed to evaluate the frequencies of cytotoxic T cells specifically recognizing RHAMM and G250 using Tetramer staining/multi-color flow cytometry and enzyme linked immunosorbent spot (ELISPOT) assays. RHAMM and G250 were expressed in 73 and 80% of the HNSCC samples at the protein level. A co-expression of both TAAs could be detected in 60% of the patients. In 4/8 HLA-A2 + patients, 0.06-0.13% of CD8 + effector T cells recognized Tetramers for RHAMM or G250 and secreted IFNÁ and granzyme B in ELISPOT assays. RHAMM and G250 are expressed at high frequency and high protein level in HNSCCs and are recognized by cytotoxic CD8 + effector T cells. Therefore both TAAs constitute interesting targets for T cell based immunotherapies for HNSCC.
Nach einer repräsentativen Untersuchung der Bundesregierung über "Gewalt gegen Frauen in Deutschland"aus dem Jahr 2004 haben 25 % der in Deutschland lebenden Frauen zwischen 16 und 65 Jahren ein-oder mehrmals körperliche, z.T. auch zusätzliche sexuelle Gewalt durch männliche Bezugspersonen erfahren. Etwa 45.000 Frauen suchen mit ihren Kindern jährlich in einem der etwa 400 Frauenhäuser in Deutschland Schutz und Unterkunft. Mitarbeiterinnen von Frauenhäusern und -beratungsstellen kämpfen seit langem um die Enttabuisierung des Themas "Häusliche Gewalt" sowie um die Erweiterung der Handlungsmöglichkeiten. In den Bereichen Polizei, Justiz, Kinderschutz und Ausländerrecht wurden Maßnahmen entwickelt und etabliert, um Frauen und Kinder besser zu schützen. Seit 2002 wurde mit dem Inkrafttreten des Gewaltschutzgesetzes (GewSchG) die rechtliche Position von Opfern häuslicher Gewalt gestärkt. Trotzdem fällt es vielen Frauen äußerst schwer, diese gesetzlichen Möglichkeiten in Anspruch zu nehmen und sich von gewalttätigen Partnern zu trennen. Zum einen sind es geplante Neuerungen, die das GewSchG nahezu konterkarieren: Das betrifft für Migrantinnen das eigenständige Aufenthaltsrecht für nachgezogene Ehegatten, das wieder eingeschränkt werden soll, und das geplante FGG-Reformgesetz (Verfahren in Familiensachen und in Angelegenheiten der Freiwilligen Gerichtsbarkeit). Es betrifft jetzt schon, oft gravierend, die Auswirkungen des Kindschaftsrechtsreformgesetzes, das das Zugriffsrecht durch gewalttätige Väter nach der Trennung auf Mutter und Kind enorm gestärkt hat. Zwar bietet das GewSchG die Möglichkeit, den Gewalttäter übergangsweise der Wohnung zuverweisen; ihm kann zudem jeglicher Kontakt mit der betroffenen Frau untersagt werden. Im krassen Widerspruch dazu stehen aber die Regeln des Kindschaftsrechtes: Danach ist jeder Elternteil zum Umgang mit seinem Kind berechtigt, wenn dies nicht dem Wohl des Kindes widerspricht. Gewalt gegen die Mutter wird von den Gerichten und den Jugendämtern nicht immer als Gewalt gegen das Kind anerkannt. Nach deren Auffassung könne ein Mann, der seine Frau misshandelt hat, trotzdem ein guter Vater sein. Alle Appelle von Frauenschutzorganisationen, das Umgangsrecht zumindest für einige Zeit auszusetzen, damit Mütter und Kinder zur Ruhe kommen können, wurden bisher wenig beachtet. Die Angst vor der Drohung "Ich werde dich überall fi nden!" ist einer der Gründe, warum viele Frauen zu ihrem Misshandler zurückkehren -in der Hoffnung, Schlimmeres verhindern zu können. Frauenhäuser und das Gesetz bieten keine hundertprozentige Sicherheit. Beraterinnen können zusammen mit den Frauen eine Gefahrenprognose erstellen und einen Sicherheitsplan entwickeln, der meist funktioniert, aber eben nicht immer. Die soziale und die psychische Situation spielen eine wichtige Rolle bei der Bewältigung der Trennung. Am häufi gsten genannt werden folgende Befürchtungen: Ein höheres Gefährdungsrisiko für sich und die Kinder bei einer Trennung, da auch die angeordneten Schutzmassnahmen nur bedingt schützen können.
