Objetivo: describir los hallazgos por embrioscopía transcervical en pacientes con aborto retenido, de diferentes semanas de gestación en primer trimestre.Institución: Medifértil, Bogotá, Colombia.Materiales y métodos: estudio descriptivo tipo serie de casos. Pacientes con aborto retenido diagnosticado por ecografía; se describe el cariotipo de la muestra obtenida por legrado obstétrico posterior al procedimiento de embrioscopía.Resultados: el procedimiento fue realizado exitosamente a las seis pacientes, cuatro de las cuales presentaron alteraciones fenotípicas todas con cariotipo normal, un caso presentó trisomía 21 sin alteraciones fenotípicas y un caso en que se identificó degeneración hidrópica, correspondió a una mola parcial.Conclusión: la embrioscopía transcervical previa al legrado obstétrico podría ser una alternativa diagnóstica útil, principalmente en pacientes con pérdida temprana posterior a reproducción asistida y pérdida recurrente de la gestación.
Debido a la rara presentación simultánea entre cáncer y embarazo y a que la afectación de la unidad fetoplacentaria por dicha patología es aún más exótica, la presente revisión busca lograr un mejor entendimiento sobre la afectación placentaria y fetal por patologías malignas que pueden comprometer el embarazo. Se realizó una búsqueda en diferentes bases de datos electrónicas como Pubmed, Ovid, Elsevier, Cochrane y libros de texto impresos. Los hallazgos en la literatura médica son bastante limitados y básicamente se trata de reportes de casos o series de casos, encontrados mediante búsqueda bibliográfica.Desde el primer reporte de metástasis de carcinoma hepático a placenta y feto, descrito en 1866, se han documentado solamente 14 casos de metástasis transplacentaria materno-fetal y poco más de 50 casos de enfermedades malignas maternas con metástasis a placenta.
Objetivos: determinar la prevalencia de la lesión escamosa intraepitelial y la validez diagnóstica del curetaje endocervical en pacientes con citología informada con atipias en células glandulares.Metodología: se llevó a cabo un estudio de corte transversal en pacientes con atipias en células glandulares en la citología, del Hospital Universitario de La Samaritana, de tercer nivel de atención en Bogotá, Colombia, durante el periodo de enero del 2000 a junio del 2006.Criterios de inclusión: información completa de citología con atipias glandulares, colposcopia y curetaje endocervical. Los datos se procesaron en el programa estadístico SPSS, V.15.Resultados: se analizaron 105 mujeres con resultados positivos de citología para atipias glandulares. La prevalencia de lesión cervical intraepitelial fue del 53% en mujeres con AGC, de estas correspondió a LEI de bajo grado el 46,5% y LEI de alto grado el 7%. El curetaje endocervical fue informado en el 86,7% como negativo para la lesión, el 13,3% reportaron lesión intraepitelial de bajo grado y en 22 como muestra insuficiente. La lectura del epitelio exocervical fue negativa en 46,5% e igual proporción para lesión intraepitelial de bajo grado y de alto grado en el 7%. La concordancia entre la biopsia exocervical y el curetaje endocervical mostró un Kappa de 0,07.Conclusión: hay una alta prevalencia de lesión cervical intraepitelial, fue del 53% en mujeres con AGC. Ninguna de las mujeres tuvo resultado positivo para carcinoma escamocelular o adenocarcinoma.
