Antecedentes: el Status Epilepticus (SE) es un evento agudo y constituye una emergencia neurológica que requiere tratamiento rápido y eficaz con el fin de evitar daño neuronal. Es frecuente que después de un período de SE, hayan alteraciones estructurales y funcionales en redes neuronales que culminan con la aparición de crisis convulsivas espontáneas. Dentro de las estructuras que se afectan está el sistema límbico y en particular la amígdala del lóbulo temporal y el hipocampo.Otra estructura que se afecta después del SE es el tálamo que es considerado modulador de las crisis convulsivas, y está involucrado en la fisiopatología de crisis convulsivas tipo ausencias.Objetivo: estudiar que los núcleos talámicos están afectados después de inducir SE por la administración de pilocarpina en ratas sometidas a diferentes tiempos de duración del SE.Material y métodos: valorar los núcleos talámicos con presencia de focos hemorrágicos macroscópicos, después de 1, 2, 3 y 4 hs PosSE.Resultados: los 5 núcleos talámicos que tuvieron focos hemorrágicos independientemente del tiempo de duración del SE, fue el posterior, el reticular, el ventrolateral, el ventroposterolateral y el ventroposteromedial. Observamos también que los animales mostraron deficit motor que se correlacionó con la duración del SE, es decir el defict motor fue mayor en los animales que tuvieron 4 hs de SE.Discusión y conclusión: Los focos hemorrágicos observados pueden corresponder a microsangrados por ruptura de vasos sanguíneos de pequeño calibre, que posiblemente condicionen el déficit motor y diversos cambios en la red neuronal.
En esta revisión planteamos la necesidad que tienen los estudiantes en general y en particular los de bioingeniería y medicina de obtener información básica sobre la electrofisiología del tejido excitable. Es importante establecer algunos coceptos históricos de cómo evolucionó y se estructuró el marco teórico de la electricidad y de la electrónica que manejamos en la actualidad. Esta información nos sirve de base para entender el desarrollo tecnológico de la electrónica que en particular se usa para el diseño y construcción de dispositivos biomédicos. Presentamos conceptos generales del circuito eléctrico equivalente a la membrana celular del tejido excitable y de manera funcional establecemos el comportamiento del voltaje y de la corriente en un sistema biológico.
La lesión medular cervical traumática (LMCT) es una de las situaciones de mayor impacto personal y social que produce modificaciones en la vida del paciente y su familia; viendose afectada la posibilidad de subsistencia y actividad en la sociedad. Se presenta con mayor frecuencia en hombres que en mujeres en la etapa productiva de la vida. En México la causa más frecuente; son los accidentes automovilísticos. Se trata de un tema médico del área de la neurología y dado el impacto que tiene en el lesionado y su familia corresponde al área social. En este estudio el tema se aborda desde esas dos perspectivas con el objetivo de contribuir al conocimiento médico social y de proponer acciones útiles para las personas interesadas en el asunto. La atención médica debe proporcionarse por parte del equipo de salud, y cada uno de sus integrantes tiene un papel clave y bien definido de acuerdo con el momento en que se encuentre el lesionado. Desde el punto de vista social, la familia es esencial durante todos los procesos que pasa el lesionado. Las amistades directas e indirectas del lesionado tienen un papel importante, ya que lo acompañan en diferentes maneras y momentos, junto con la familia contribuyen al bienestar personal y emocional. La rehabilitación es la herramienta más importante de la que dispone el lesionado y su familia. Su objetivo esencial es lograr la reintegración social en las condiciones más ventajosas, se lleva a cabo de manera integral y se orienta al desarrollo de la fuerza muscular, para obtener la mayor funcionalidad de los músculos que intervienen en la ejecución de los diferentes movimientos con los cuales el cuadripléjico recobrara independencia.
La alteración de la barrera hematoencefálica (BHE) participa de manera importante en la epileptogénesis. La epilepsia del lóbulo temporal (ELT) es una de las formas más comunes de epilepsia, se caracteriza por lesión de varias zonas del hipocampo. En esta revisión se analiza la participación de los posibles mecanismos o alteraciones de la BHE en el proceso de la epileptogénesis que observamos en el modelo de epilepsia inducido por la administración sistémica de pilocarpina, que reproduce las características clínicas y neuropatológicas principales de la ELT del humano. La valoración de la alteración de la BHE y la extravasación de la albumina sérica en diferentes estados patológicos ha sido estudiada mediante el uso del azul de Evans (AE), por la ventaja de su bajo costo y fácil valoración, ya que puede visualizarse macroscópicamente. En este laboratorio hemos evaluado las posibles alteraciones de la BHE en el modelo de epilepsia experimental por la administración de pilocarpina y de AE en diferentes tiempos pos status epilepticus (SE). Considerando que en la ELT la lesión significativa es la gliosis del lóbulo temporal, también hemos estimado la aparición de la gliosis reactiva mediante valoración de la proteína ácida fibrilar glial (GFAP) con el establecimiento y evolución del proceso convulsivo. Con esta revisión concluimos que existe fuerte relación entre los cambios producidos en la BHE, astrogliosis reactiva y aumento de la excitabilidad neuronal.
