We describe a complicated case of dextro-transposition of the great arteries (d-TGA) that was treated unusually late at age four by Glenn Shunt operation with pulmonary artery banding. She presented fifteen years later with superior vena cava syndrome as a consequence of Glenn shunt failure and was managed conservatively. Glenn Shunt with pulmonary artery banding can be done for Dextro-TGA patients in which arterial switch operation is contraindicated; however, pulmonary artery banding failure can cause a functional obstruction leading to fatal SVC syndrome.
