In the article we deal with the rehabilitation of patients using information technology, especially Internet support. We concentrate on two main areas in the IT support of rehabilitation: one of them is the support for individual therapy, the other one is providing patients with information, which is the basic step in emphasising individual responsibility. In the development of rehabilitation programmes, the knowledge of the IT professional and the therapist, in the IT support of web guidance, medical expertise plays the primary role. The degree of assistance involved in the rehabilitation process depends on the IT knowledge of medical (general practitioner, nursing staff) professionals as well. The necessary knowledge required in healing and development processes is imparted to professionals by a special (full-time) university training. It was a huge challenge for us to teach web-based information organisation skills to doctors and nurses, and it is also a complex task to put forward such an IT viewpoint to information specialists in order to create the foundations of the cooperation between IT and healthcare professionals.
This article is a (possible) conceptual educational model, which introduces data representation, information storage and retrieval possibilities on the Web in a way analogous to the levels of organization of metadata.The model uses the traditional library and information systems as a starting point, referring to the levels and types of information organization, and describes directions of its development. General acquaintance with the dominant organizational levels and types helps to understand the information organization on the internet, the coexistence of both structured and unstructured elements, the closedness and deficiencies of the content of information, and also helps to find possible ways of correcting these deficiencies. One of the main advantages of model-driven approaches is that they, by using the well-known classical systems, make tangible the development of physical and content data organization types and levels of organization of information for medical students that usually do not possess informatics knowledge.The conceptual model presented in details in the article can provide a basis for a general introduction to metadata and to develop curricula equally appropriate for traditional face to face classes, trainings and online courses.
Telemedicine is a young science that integrates innovations of information-technology and telecommunications into medical science. A successful telemedicine procedure should guarantee reduced workload of the healthcare system with well secured and cost-effective processes. Our goal was to collect the development phases of telemedicine projects through existing telecardiology solutions. Subsequent to reviewing international publications we analyzed the past and present situation of blood pressure monitoring, remote diagnostics of electrocardiography, implantable cardioverter defibrillator monitoring and pocket ultrasound devices. In case of new solutions (a) several internationally accepted, confidently reproducible "good practices" are needed for creating (b) guidelines and recommendations of international medical associations. They have to ensure (c) cost-effective work, with well-designed sustainability and (d) patient confidentiality. Improving (e) education for professionals and patients is essential. We recommend to telemedicine developers to use our standards in order to introduce their products more effectively into clinical practice. It is encouraging that current possibilities of telecardiology partly or fully meet the aforementioned criteria. Az utóbbi évtizedek telekommunikációs és informatikai-technológiai fejlődése az egészségügyi ellátásban is jelentős változásokat hozott, és kialakult az orvostudomány új ága, a telemedicina. Az elmúlt 20 évben a telekommunikációs hálózatok folyamatos fejlődése, az internet megjelenése, az okostelefonok térhódítása lehetővé tette a telemedicina ugrásszerű fejlődését, így mára már számos eszköz és alkalmazás áll rendelkezésünkre, ezzel párhuzamosan pedig a szakirodalmi publikációk és hivatkozások száma is exponenciálisan növekszik. A terület egyik legdinamikusabban fejlődő ágává a teledermatológia, azaz a telemedicina bőrgyógyászati alkalmazása vált, mivel a bőrön található elvál-tozások jól láthatóak és hozzáférhetőek, az új eszközökkel pedig komolyabb beavatkozás nélkül is dokumentálható-ak. A módszer Magyarországon eddig még nem honosodott meg, pedig alkalmazásával a betegellátás hatékonyabbá, gyorsabbá és olcsóbbá válhat. A szerzők célja, hogy áttekintsék a bőrgyógyászat távkonzultációs gyakorlatának külföl-di tapasztalatait és legfontosabb eredményeit. The appearance and rise of internet and smart phones induced a rapid progression in telemedicine. Several new applications and mobile devices are published every hour even for medical purposes. Parallel to these changes in the technical fields, medical literature about telemedicine has grown rapidly. Due to its visual nature, dermatology is ideally suited to benefit from this new technology and teledermatology became one of the most dynamically evolving fields of telemedicine by now. Teledermatology is not routinely practiced in Hungary yet, however, it promises the health care system to become better, cheaper and faster, but we have to take notice on the experience and problems faced in teledermatologic applications so far, summarized in this review.
