P armak ucu yaralanmaları çocuklarda sıktır, çeşitli çalışmalarda çocuk acil servis başvurularından %1.8-2'sini el yaralanmalarının oluşturduğu bildirilmiştir ve bunların %21-46'sı parmak ucu yaralanmalarıdır. [1][2][3] Erişkin hastaların parmak ucu yaralanmaları değerlendirildiğinde daha çok iş kazaları etyolojik neden olmakla birlikte çocuk hastalarda bu nedenler daha çok ev ve dış ortamda gerçekleşen kazalar olarak karşımıza çıkar. Başka bir çalışmada da tüm acil başvurularının %2'sini kapıya sıkışma ile olan parmak ucu yaralanmaları oluşturmaktadır. [4] Amerika'da yapılan ülke geneli bir çalışmada parmak amputasyonlarının tüm pediatrik travmatik amputasyonların %91,6'sını oluşturduğu bildirilmiştir. [5] Ülkemizde çocuk popülasyonunun hızlı artışına rağmen bu tarz yaralanmaların epidemiyolojisine ilişkin yeterli bilgi bulunmamaktadır. Bu yaralanmalar yapılan aktivitelere ve bölgelere bağlı değişiklik gösterdiği için Amaç: Bu çalışmada 4 yıllık süreçte parmak ucu yaralanması nedeniyle acil serviste değerlendirerek tedavi ettiğimiz 354 pediatrik olgu etiyoloji, demografik bilgiler, tedavi ve komplikasyonlar açısından incelenmiştir. Çalışmanın amacı çocuk hastalarda sık görülen parmak ucu yaralanmalarına dikkat çekmek ve önleyici stratejiler geliştirilmesine yardımcı olmaktır. Yöntem: Çalışmaya dahil edilen 354 hastalanın 191 tanesi erkek çocuk 163 tanesi kız çocuktu. Yaşları 6 ay ile 17 yaş arasında değişmekteydi. Bu hastalar cinsiyete, yaralanan el ve parmaklara, yaralanma mekanizmasına, yaralanma bölgesine, seçilen tedavi yöntemlerine ve komplikasyonlara gore sınıflandırılarak incelendi. Ameliyatların tamamında lokal anestezi altında çalışıldı. Hastaların tamamına acil serviste girişim yapılarak hastalar aynı gün taburcu edildi. Bulgular: Çalışmamızda incelenen hasta grubunda erkek çocukların (%54) kız çocuklara göre (%46) daha fazla yaralndığı görüldü. Sağ elin (%65.3), sol ele (%34.7) göre çok daha fazla yaralandığı görüldü. En fazla yaralanmanın 136(%38.4) hastada orta parmakta olduğu, bunu 120(%33.9) hastada yüzük parmağın takip ettiği gözlendi. Bu yaralanmaların en sık görüldüğü yaş 5 olarak belirlendi. En sık yaralanma tipi %83.3 oranla ezilme tipi yaralanma olarak görülmüşken, bunun da en sık nedeninin kapıya sıkışma tarzı yaralanmalar olduğu görüldü. Bu yaralanmaların çoğuna cerrahi müdahale gerekmişken, uygun hastalarda yara bakımı ile sekonder iyileşme ile tedavi yapıldı. Tartışma: Çocukluk çağında oldukça el yaralanmaları oldukça sıktır ve parmak ucu yaralanmaları bunların arasında çoğunluğu oluşturur. Çocukluk çağında olan bu yaralanmalar ileri dönemde bu hastalarda önemli fonksiyonel, estetik ve psikolojik sekellere yol açmaktadır. Bu yaralanmaların etiyolojisi, dağılımı ve mekanizmaları hakkında bilgi sahibi olmak, bu konuda önleyici tedbirlerin geliştirilmesine imkan verecektir. A vulsion and degloving injuries are caused by a vast array of mechanisms, machines and situations, in which the hand is injured with low energy shearing forces in a rigid position.[1] These types of injuries have generally been viewed as among the most problematic of injuries to manage due to their extensive nature of associated damage. During degloving injuries, blood vessels having great elasticity, are disembodied and separated with irreversible injury along the entire length of involvement and so are the tendons and nerves. Furthermore in circumferential degloving injuries, the distal flap goes through a state of abrupt ischemia followed by reverse perfusion forming after the reattachment of the flaps back to its' origin. Arnez et al, recently reclassified degloving injuries according to the Objectives: Degloving hand injuries have generally been viewed as among the most difficult of injuries to manage due to the extensive nature of associated damage. The traditional approach to the circumferentially degloved segment of problematic flap viability has been to resuture the flap and to wait and see. However, the waiting period or the specific hemorheological protocol remains uncertain. The purpose of this study is to acknowledge if Sivelestat, known to ameliorate ischemia reperfusion injury, enhances the survival of avulsed flaps in a hind limb degloving model of rats and to compare Sivelestat's effects to Pentoxifylline. Methods: Total flap area (cm 2 ), area of necrosis in the flap (cm 2 ), and the ratio between the necrotic and total areas (percentage) were determined. Angiogenesis among the groups were documented with CD31, anti-PECAM stainings. TUNEL assay was performed to allow the visualization of cell nuclei containing fragmented DNA, a typical feature of apoptosis. Results: This study demonstrated that, Sivelestat administered at 10 mg/kg/hour dosage will inhibit the ischemia reperfusion injury more pertinently than Pentoxifylline which exerts only hemorheological effects. Conclusion:The anti-inflammatory effects of Sivelestat, will be beneficial for decreasing the early complications of degloving injury, such as inflammation, sepsis, and edema, better than Pentoxifylline which exerts only hemorheological effects.
Objective: Prominent ear is a congenital ear deformity that is commonly seen in society and has psychosocial effects. A prominent ear deformity can be due to antihelical fold underdevelopment, concave structure overdevelopment, and lobule overgrowth. Material and Method: In total, 108 patients who underwent autoplasty at our clinic between October 2010 and October 2015 were retrospectively evaluated. These patients were comprised of 63 women and 45 men between the ages of five and 42 years (average age, 18.4 years). Among the 108 patients, 105 had a bilateral prominent ear and three had a unilateral prominent ear. Results: Cephaloauricular angle measurements of patients before the operation ranged from 46 to 57-degree. The median cephaloauricular angle was 51-degree. The recorded upper pole-to-mastoid distance ranged from 15 to 27 mm. The median upper pole-to-mastoid distance was 20 mm. After the operation, the median upper pole-to-mastoid distance was 12 mm. The postoperative median cephaloauricular angle was 27-degree. The average follow-up duration was 18.4 months (range, 3-24 months). Two patients developed hematoma in the early postoperative period. In the late postoperative period, one patient had suture exposition, one had recurrence, six had cold intolerance, one had a bad scar, and one had a telephone ear deformity. Conclusion: Today, autoplasty is one of the most commonly practiced aesthetic operations performed by plastic surgeons. Favorable results can be achieved in remedying the anatomical problems when proper problem-oriented surgical treatments are used. In particular, in early aged patients, it is known that good cosmetic results and important psychological improvement are provided by these procedures.
