Az egészségügyi ellátások során kialakuló nem várt események adatainak gyűjtését szolgálja a WHO és az Egészségügyi Minisztérium kezdeményezésére létrehozott NEVES jelentési rendszer. Az ide beérkezett, megtörtént események részletes adatait tartalmazó jelentéseket hét témakörben dolgoztuk fel az általunk kidolgozott, aggregált jelentési adatokra támaszkodó módszertani útmutató alapján. A dolgozat ismerteti ezen módszertan legfontosabb lépéseit és eszköztárát. Ez segíti az intézmények saját adataikra támaszkodó tanulását, a betegbiztonsági intézkedések megfogalmazását és végrehajtását, valamint bevezetőként szolgál a tanulmányok bemutatását célzó cikksorozathoz.
Az egészségügyi ellátás során bekövetkező betegesések megelőzése mind a betegellátás biztonságának javítása, mind pedig az egészségügyi szolgáltatók költségeinek csökkentése szempontjából fontos fejlesztési lehetőség. Dolgozatunk célja, hogy bemutassuk a NEVES jelentési rendszerbe érkezett adatok elemzésére alapozott kutatási eredményeket, amelyek rávilágítanak az egészségügyi szolgáltatóknál bekövetkezett betegesések általános okaira és azok megelőzési lehetőségeire. A NEVES jelentési rendszer indulása óta az adatfeldolgozás megkezdéségig 15.077 betegesési esemény adatait jelentették magyar egészségügyi szolgáltatók anonim, önkéntes módon. Ezeket az adatokat statisztikailag elemeztük, és minőségfejlesztési technikákat is alkalmazva kerestük a bekövetkezésükben szerepet játszó okokat. A vonatkozó szakirodalmak áttekintésével és a szakértői egyeztetésekkel szerzett további ismereteinket is felhasználva rendszereztük a kutatás során azonosított okokat és a megelőzési lehetőségeket. Az okokat 12 fő csoportba soroltuk, amelyek közül kiemelendők a kockázatfelmérés hiánya, a prevenciós intézkedések elmaradása, a kommunikáció hiányosságai és az infrastrukturális problémák. Fontos felhívni a figyelmet arra, hogy ezeket az okokat rendszerben érdemes kezelni. Az eredményes megelőzés érdekében az adott intézményi környezetben minden lehetséges okot fel kell tárni, és minél több ok megszüntetésére törekedve komplex intézkedéseket hozni. Dolgozatunkban felvázoljuk, hogy a betegesések bekövetkezését előidéző okok azonosításakor milyen konkrét intézkedések segíthetik a megoldást.
Bevezetés: A pszichiátriai betegek körében agresszív viselkedés a leggyakrabban az akut pszichiátriai osztályos kezelés során fordul elő, jelentős betegbiztonsági és dolgozói biztonsági kockázatot okozva. Célkitűzés: A Jahn Ferenc Dél-pesti Kórház akut pszichiátriai részlegén, a fizikai agresszió előfordulási gyakoriságának csökkentésére irányuló projekt részeként, kockázatbecslő eszköz napi rutinban történő használatának bevezetése az agresszív betegek korai azonosítására. Módszerek: Az agresszív magatartás kockázatának felmérésére szolgáló eszköz kiválasztása irodalomkutatás alapján történt. A V-RISK-10 magyar fordításának elkészítése után minden, az osztályra sürgősséggel felvett betegnél a felvevő orvos kitöltötte a kérdőívet. A kérdőív predikciós értékének felmérésére az első 50 felvett kérdőív esetében visszamenőleg megvizsgáltuk, hogy a betegek felvételét követő egy hétben hány alkalommal és milyen időtartamban volt szükség korlátozó intézkedés alkalmazására veszélyeztető magatartás miatt. Eredmények: A V-RISK-10 skálán elért átlagpontszám 6,78 ± 3,36 pont volt (a skálán minimum 0, maximum 20 pont kapható). A betegek korlátozás alatt töltött összes ideje 6,96 ± 17,21 óra volt. A V-RISK-10 skála összpontszáma közepes erősségű, szignifikáns korrelációt mutatott a korlátozás alatt töltött összesített óraszámmal (r = 0,447; p = 0,001). Megbeszélés: Az eredmények alapján a V-RISK-10 skála alkalmas a felvételt követő egy hétben megjelenő, fizikai korlátozást igénylő agresszív magatartás előrejelzésére. Az előzményben szereplő erőszakos magatartásnak, szerhasználatnak és a már diagnosztizált pszichiátriai betegségnek, az aktuálisan észlelhető tünetek közül a gyanakvásnak volt a leginkább prediktív szerepe. Következtetés: A V-RISK-10 skála a rutin-betegellátásban könnyen alkalmazható, az agresszív magatartást közepes erősséggel előre jelző kockázatfelmérő eszköznek bizonyult, mellyel már az osztályos felvételkor azonosíthatók az agresszió szempontjából magas kockázatú betegek, akiknél szoros betegkövetéssel, az agresszív magatartás elkerülését szolgáló technikák időben történő alkalmazásával az agresszív magatartás előfordulása csökkenthető. A nyomási fekélyek előfordulása gyakori probléma a fekvőbeteg ellátásban és jelentős terhet képviselnek mind a beteg, mind az ellátók számára, habár kialakulásuk sok esetben megelőzhető lenne. Dolgozatunk célja, hogy bemutassuk a NEVES jelentési rendszerbe érkezett adatok elemzéséből kiindulva a nyomási fekélyek kialakulásának általános okait és az azokhoz kapcsolódó megelőzési lehetőségeket, valamint ajánlásokat fogalmazzunk meg elkerülésükre. A NEVES rendszerbe az adatfeldolgozás megkezdéségig 2.579 nyomási fekély adatait jelentették. Ezeket az adatokat statisztikailag elemeztük, és minőségfejlesztési technikákat is alkalmazva kerestük a nyomási fekélyek kialakulásában szerepet játszó okokat. A témában végzett szakirodalmi kutatásból és szakértői egyeztetésekből nyert információkat is tartalmazó kutatást felhasználva, valamint a szakirodalmakat áttekintve rendszereztük az okokat és megelőzési lehetőségeket. A kutatás során feltárt, a nyomási fekély kialakulásához vezető általános okokat 12 csoportba soroltuk. Az elemzések eredményei közül kiemelendő, hogy a kockázatfelmérés és a megelőzést célzó protokoll megléte ellenére nagy arányban észlelik előrehaladott stádiumban a nyomási fekélyeket, ami rendszerszintű problémák jelenlétére utal. A megelőzésben kiemelt szerepet játszik a helyesen elvégzett kockázatfelmérés, a megfelelő prevenciós eszköz helyes alkalmazása és a munkatársak naprakész ismerete. Ez utóbbiban nagy jelentősége van annak, hogy a szakmai evidenciák hogyan és mennyi idő elteltével kerülnek át a napi gyakorlatba. A nyomási fekély kialakulásának hátterében álló okok nagyon sokrétűek lehetnek. Ahhoz, hogy felfedjük egy adott intézményben az ott előforduló rendszerszintű problémákat, elemezni szükséges a bekövetkezett eseményeket, ehhez pedig adatokat kell gyűjteni. Az intézményi működésből adódó problémák megelőzése érdekében oki kutatást kell végezni, amelynek eredményei alapján megtett célzott intézkedésekkel csökkenthető az események előfordulása.
Bevezetés: Az éles, hegyes eszközök okozta sérülések mindennapi kockázatként vannak jelen az egészségügyben dolgozók életében. A megelőzésük mind a sérülések elkerülése érdekében, mind pedig a fertőződés kockázata miatt fontos. Célkitűzés: Közleményünkben megvizsgáljuk az éles, hegyes eszközök okozta sérülések megelőzését célzó oktatások eredményességének fejlesztési lehetőségeit a kutatási eredmények és a publikált szakirodalmak tükrében. Módszer: A témában készült, 2020-ban megjelent kutatás, valamint az azóta publikált szakirodalmak alapján fogalmazunk meg javaslatokat az oktatások rendszerére és módszerére, valamint az oktatandó tananyagra vonatkozóan. Eredmények: Az oktatás az ismeretek bővítésén és a viselkedés megváltoztatásán keresztül eredményesen alkalmazható a sérülések megelőzésében. A hazai kutatás eredményei azt támasztják alá, hogy a téma oktatása annak tartalmát és gyakoriságát illetően is fejlesztendő. Megbeszélés: A jogszabálynak megfelelő tartalommal felépített oktatási anyag hangsúlyait célszerű az intézményben azonosított problémákhoz igazítani. A képzés megfelelő gyakorisága fontos eleme az eredményességnek. A javasolt képzési forma a helyszínspecifikus gyakorlati oktatás. Következtetés: Az éles, hegyes eszközök okozta sérülések kockázatát önmagában egyetlen módszer sem tudja nullára csökkenteni. A megelőző intézkedéseknek azonban nélkülözhetetlen eleme a megfelelő oktatás.
