According to current treatment guidelines for Complex PTSD (cPTSD), psychotherapy for adults with cPTSD should start with a "stabilization phase." This phase, focusing on teaching self-regulation strategies, was designed to ensure that an individual would be better able to tolerate trauma-focused treatment. The purpose of this paper is to critically evaluate the research underlying these treatment guidelines for cPTSD, and to specifically address the question as to whether a phase-based approach is needed. As reviewed in this paper, the research supporting the need for phase-based treatment for individuals with cPTSD is methodologically limited. Further, there is no rigorous research to support the views that: (1) a phase-based approach is necessary for positive treatment outcomes for adults with cPTSD, (2) front-line trauma-focused treatments have unacceptable risks or that adults with cPTSD do not respond to them, and (3) adults with cPTSD profit significantly more from trauma-focused treatments when preceded by a stabilization phase. The current treatment guidelines for cPTSD may therefore be too conservative, risking that patients are denied or delayed in receiving conventional evidence-based treatments from which they might profit.
A series of single-case experiments was used to evaluate the application of Eye Movement Desensitization and Reprocessing (EMDR) to traumatically induced dental phobia. Following two to three sessions of EMDR treatment, three of the four patients demonstrated substantially reduced self-reported and observer-rated anxiety, reduced credibility of dysfunctional beliefs concerning dental treatment, and significant behavior changes. These gains were maintained at six weeks follow-up. In all four cases, the clinical diagnosis present at pretreatment was not present at posttreatment at a clinical level. All patients actually underwent the dental treatment they feared most within three weeks following EMDR treatment. The findings support the notion that EMDR can be an effective treatment alternative for phobic conditions with a trauma-related etiology.
This article outlines a comprehensive model that helps to identify crucial target memories for EMDR treatment. The "Two Method Approach" can be used for conceptualization and treatment implementation for a broad spectrum of symptoms and problems, other than those related to PTSD per se. The model consists of two types of case conceptualizations. The First Method deals with symptoms whereby memories of the etiological and/or aggravating events can be meaningfully specifi ed on a time line. It is primarily aimed at the conceptualization and treatment of DSM-IV-TR Axis I disorders. The Second Method is used to identify memories that underlie patients' so-called dysfunctional core beliefs. This method is primarily used to treat more severe forms of pathology, such as severe social phobia, complex PTSD, and/or personality disorders. The two methods of case conceptualization are explained step by step in detail and are illustrated by case examples.
Eye Movement Desensitization and Reprocessing (EMDR ) is een relatief nieuwe procedure op het terrein van de psychotherapie. Ervaringen met EMDR geven aanleiding tot hoopvolle verwachtingen van de behandeling van diverse aan trauma gerelateerde angststoornissen, met name post-traumatische stress-stoornis (PTSS). Onderdeel van deze procedure is dat de therapeut bij de clie¨nt een aantal snelle en ritmische oogbewegingen uitlokt door te vragen zijn of haar vinger te volgen, terwijl de clie¨nt een beeld van de traumatische herinnering in gedachten houdt. In dit artikel worden de achtergronden en de principes van EMDR belicht en wordt de stapsgewijze procedure uitvoerig beschreven. Een gevalsbeschrijving van een clie¨nt met een paniekstoornis en een tandartsfobie laat zien dat EMDR kan leiden tot een langdurige vermindering van angstklachten. Tevens wordt ingegaan op de huidige stand van zaken van de wetenschappelijke ondersteuning van EMDR . Het toepassen van EMDR bij PTSSwordt door wetenschappelijk onderzoek gesteund, maar empirische ondersteuning voor de therapeutische effectiviteit van EMDR bij andere angststoornissen ontbreekt.
Eye movement desensitization and reprocessing (EMDR) has been shown to be a structured, noninvasive, time-limited, and evidence-based treatment for unprocessed memories and related conditions. This paper focuses on EMDR as a treatment for specifi c fears and phobias. For this purpose, the application of EMDR is conceptualized as the selection and the subsequent processing of a series of strategically important memories of earlier negative learning experiences concerning specifi c objects or situations. Firstly, the practical application and conceptualization of the treatment of phobias with EMDR is presented and compared with an exposure-based treatment approach. Next, specifi c attention is given to the assessment and selection of appropriate memories for processing. It is hypothesized that phobias with a nontraumatic background, or those in later stages of treatment after some reduction in anxiety has been achieved, would profi t more from the application of a gradual in vivo exposure, whereas trauma-based specifi c phobias and those with high initial levels of anxiety would respond most favorably to EMDR.
Choking phobia is a specific phobia characterized by fear and avoidance of swallowing foods and liquids. It often develops following an episode of choking on food. A prospective case study of a 30-yearold woman with a phobia of choking, acquired after a series of traumatic incidents 5 years previously, demonstrates the usefulness of an approach that is aimed at processing the disturbing memories of a traumatic event. Two therapy sessions of Eye Movement Desensitization and Reprocessing (EMDR) produced a lasting decrease in symptomatology. #
Gegeneraliseerde angststoornis: wat is het en wat kun je ertegen doen?Kees Korrelboom Sako Visser Erik ten Broeke Samenvatting Dit artikel bevat een overzicht van de aard en effectiviteit van psychologische behandelingen voor de gegeneraliseerde angststoornis. Ook gaat het uitvoerig in op interventies die gericht zijn op een van de kernsymptomen van deze stoornis: piekeren. Daarbij staan we stil bij een recente ontwikkeling binnen cognitieve therapie: interventies die niet zozeer zijn gericht op de inhoud van cognities, maar eerder op de denkstijl of het denkproces. Nadat we in dit kader 'mindfulness based cognitive therapy', 'acceptance and commitment therapy' en 'attention training technique' aan bod hebben laten komen, bespreken we een nieuwe, specifiek op piekeren gerichte interventie: anti-piekertraining. Deze interventie bestaat uit een stappenplan dat individueel en in groepsverband kan worden uitgevoerd. Dit stappenplan is gebaseerd op een moderne visie op de principes achter contraconditionering. Naar de effectiviteit van deze interventie wordt op het ogenblik onderzoek uitgevoerd. InleidingSchattingen van de 'lifetime prevalentie' van de generaliseerde angststoornis (GAS) varie¨ren tussen de twee en de zes procent. Volgens Amerikaans onderzoek komt het daarmee ongeveer even vaak voor als paniekstoornis en dwangstoornis (Craske, 1999). In Nederland ligt de situatie iets anders. Men vindt daar een lifetime prevalentie van 2,3 procent voor gegeneraliseerde angststoornis tegenover 3,8 procent voor paniekstoornis (Bijl, Van Zessen & Ravelli, 1997;Boeijen, Visser & Van Balkom, 2001). Bij ouderen ligt de prevalentie van gegeneraliseerde angststoornis waarschijnlijk hoger dan bij volwassenen (Craske, 1999). Hoewel gegeneraliseerde angststoornis frequent voorkomt, is betrekkelijk weinig bekend over de kenmerken en behandeling van deze stoornis. Die onbekendheid komt tot uitdrukking in de verschillende criteria waarmee gegeneraliseerde angststoornis is geclassificeerd in de diverse DSM-edities vanaf 1980(Craske, 1999. Volgens de huidige visie wordt gegeneraliseerde angststoornis vooral gekenmerkt door extreme angst en oncontroleerbaar gepieker in combinatie met spanningsklachten en motorische onrust (apa, 1994). De diagnose mag niet worden gesteld wanneer angst en onrust uitsluitend betrekking hebben op thema's die al beter door een andere as-I-diagnose worden verklaard. Omdat patie¨nten nogal eens de fysieke uitingsvormen (spanning, slapeloosheid, lichamelijk ongemak) van de stoornis op de voorgrond plaatsen en niet de psychische aspecten als angst en piekeren, vindt behandeling relatief vaak plaats in de somatische zorg, met name binnen de huisartspraktijk. Wanneer gegeneraliseerde angststoornis binnen de geestelijke gezondheidszorg wordt gezien, gaat het dikwijls om een aanvullende (comorbide) diagnose naast een andere angststoornis of een stemmingsstoornis. De behandeling richt zich dan niet zelden uitsluitend op de primair gestelde diagnose. Men gaat ervan uit dat de klachten die behoren bij de gegeneralis...
