The contribution of interleukin (IL)-10 and interferon (IFN)-gamma to the regulation of type 1 and type 2 cytokine responses was investigated in Brazilians with different clinical forms of schistosomiasis mansoni. Cells from members of a family with acute intestinal schistosomiasis responded to schistosomal soluble egg antigen (SEA) or soluble adult worm antigen preparation (SWAP) with greater amounts of IFN-gamma than did cells from several patients with chronic intestinal schistosomiasis; IL-10 levels were similar. Neutralization of IL-10 had no effect on the SEA-specific IFN-gamma response in patients with acute infection, whereas SWAP-induced IFN-gamma was increased in both groups. Anti-IL-10 also up-regulated SEA-specific IFN-gamma protein and mRNA responses in most splenocyte cultures from hepatosplenic schistosomiasis patients but had no effect on antigen-specific IL-4 or IL-5 production. Neutralization of IFN-gamma resulted in a comparable increase in SWAP-specific IL-10 and IL-5, while IL-4 was not affected. These studies demonstrate that early disease in schistosomiasis is associated with a significant IFN-gamma response and that IL-10 contributes to the suppression of that response during both early and chronic infection.
PCR-based approaches targeting kinetoplast DNA were evaluated for the diagnosis of American cutaneous leishmaniasis (ACL) in regions of endemicity in northeastern Brazil. A total of 119 cutaneous biopsy specimens from patients with ACL and nonleishmaniasis cutaneous lesions were studied. Two PCR-based systems were used; one was specific for the subgenus Viannia, and the other was specific for the genus Leishmania. The PCR specific for the subgenus Viannia had a sensitivity of 95.4%, whereas the genus-specific PCR detected the target DNA in 88.2% of the samples tested. The specificities of the assays, determined with samples from a group with nonleishmaniasis cutaneous lesions, was 100%. The results of the conventional tests indicate that the sensitivities of the PCR-based methods were significantly higher than those of smear examination, histological staining, and isolation by culture (P < 0.05). Antibodies specific for Leishmania braziliensis were detected by indirect immunofluorescence in 82.9% of the patients tested. Parasites were isolated from 40 of 86 patients (46.5%). Sixty-seven percent of dermal scrapings and 66.2% of stained tissue sections were positive by microscopy. Amplified products from the subgenus-specific PCR hybridized with the Leishmania panamensis minicircle, confirming infection consistent with L. braziliensis. The evidence available at present incriminates L. braziliensis as the only causative agent of ACL in the state of Pernambuco in Brazil.

Foi realizado Inquérito epidemiológico com o objetivo de estimar o uso de substâncias psicoativas e a prevalência de alcoolismo. Tomou-se como referência a população de maiores de 13 anos de idade de região administrativa da cidade do Rio de Janeiro, RJ (Brasil), da qual se extraiu uma amostra aleatória de 1.459 indivíduos. Os resultados referentes ao uso de bebidas alcoólicas e alcoolismo, identificados através do teste CAGE, mostraram: prevalência de 51% para o consumo de álcool e de 3% para alcoolismo, sendo 4,9% em homens e 1,7% em mulheres; maior proporção de consumidores de álcool e de alcoolistas entre homens de 30 e 49 anos; abstinência mais freqüente entre os viúvos, protestantes e indivíduos com níveis de renda inferiores.Descritores: Alcoolismo, epidemiologia. Introduction: Physical rehabilitation is commonly used in patients with Parkinson's disease (PD) to improve their health and alleviate the symptoms. Objective: We compared the effects of three programs, strength training (ST), aerobic training (AT), and physiotherapy, on motor symptoms, functional capacity, and electroencephalographic (EEG) activity in PD patients. Methods: Twenty-two patients were recruited and randomized into three groups: AT (70% of maximum heart rate), ST (80% of one repetition maximum), and physiotherapy (in groups). Subjects participated in their respective interventions twice a week for 12 weeks. The assessments included measures of disease symptoms (Unified Parkinson's Disease Rating Scale [UPDRS]), functional capacity (Senior Fitness Test), and EEG before and after 12 weeks of intervention. Results: The PD motor symptoms (UPDRS-III) in the group of patients who performed ST and AT improved by 27.5% (effect size [ES]=1.25, confidence interval [CI]=-0.11, 2.25) and 35% (ES=1.34, CI=-0.16, 2.58), respectively, in contrast to the physiotherapy group, which showed a 2.9% improvement (ES=0.07, CI=-0.85, 0.99). Furthermore, the functional capacity of all three groups improved after the intervention. The mean frequency of the EEG analysis mainly showed the effect of the interventions on the groups (F=11.50, P=0.0001). Conclusion: ST and AT in patients with PD are associated with improved outcomes in disease symptoms and functional capacity.
Summary Bariatric surgery has been recognized as the gold standard treatment for severe obesity. Although postbariatric surgery patients usually achieve and maintain substantial weight loss, a group of individuals may exhibit weight regain. Several factors are proposed to weight regain, including psychiatric comorbidity. The objective of the study is to conduct a systematic review and meta‐analysis of studies investigating the relationship between psychiatric comorbidity and weight regain. A systematic review through PubMed, Web of Science, Cochrane Library, Scopus, and PsycINFO was performed, following Preferred Reporting Items for Systematic Reviews and Meta‐Analyses (PRISMA). After a stepwise selection, 13 articles were included in the qualitative analysis and 5 were included for a meta‐analysis. Women was majority in most of the studies (87.6%), and a bypass procedure was the bariatric intervention most evaluated (66.8%), followed by gastric banding (32.1%) and sleeve (1.1%). Higher rates of postbariatric surgery eating psychopathology were reported in patients with weight regain. However, the association between general psychopathology and weight regain was not consistent across the studies. In the meta‐analysis, the odds of eating psychopathology in the weight regain group was higher compared with the nonweight regain group (OR = 2.2, 95% CI 1.54‐3.15). Postbariatric surgery eating psychopathology seems to play an important role in weight regain.
INTRODUÇÃO: Estima-se que 50% das quedas em idosos resultem em algum tipo de lesão. O objetivo deste estudo é estimar a prevalência e identificar fatores associados a quedas em idosos em um município no Rio de Janeiro. METODOLOGIA: Um inquérito foi conduzido em residentes com mais de 60 anos capazes de prestar informações por si, através de um questionário de avaliação multidimensional, aplicado por agentes comunitários treinados. As análises incluíram cálculos de proporções e respectivos intervalos de confiança de 95%. RESULTADOS: Foram entrevistados 1.064 idosos, em sua maioria mulheres (57%), com idade média de 71,4 anos. No ano anterior à entrevista, 322 participantes (30,3% - IC95% 27,6-33,2) caíram, dos quais 148 (13,9% - IC95% 11,9-16,2) o fizeram pelo menos duas vezes. Algumas associações se evidenciaram: sexo feminino, idade avançada, ser divorciado, morar só, assim como as variáveis indicadoras de más condições de saúde, capacidade funcional e satisfação com a vida. CONCLUSÃO: Este estudo ratifica fatores associados a quedas em idosos já conhecidos. O grande desafio deste conhecimento é a proposição de intervenções nos fatores modificáveis e a identificação ativa dos idosos sob risco, objetivando a reabilitação preventiva.
In 1988, a cross-sectional survey was conducted to estimate the prevalence of alcoholism and alcohol consumption in the 20th Administrative Region of the city of Rio de Janeiro. The interview protocol included questions about consumption of psychotropics, coffee, and cigarettes. This paper presents data from psychotropic consumption in the month. The sample of the population older than 13 years old included 1,459 subjects. Overall consumption of psychotropic drugs was 5.2%. Rate of psychotropic use was higher for females (6.7%) than for males (3.1%). Users were predominantly women, elderly, (the highest rate was for those between 60-69 years), divorced or widows, and with lower family income. The association of each socio-demographic variable was evaluated by the odds ratio adjusted by logistic regression. Anxiolytics (benzodiazepines) predominated (85.23%) among the reported psychotropics, followed by antiepileptics (5.68%) and hypnotics and sedatives (4.54%). Physicians not specializing in neurology or psychiatry lead prescriptions (65.8%). 80.26% of the drugs were obtained in pharmacies, while 13.16% were obtained in governmental institutions, most of them by controlled prescriptions. The results are discussed and new lines of research are suggested.
