Actualmente, con la era tecnológica, las pantallas juegan un papel fundamental dentro de la sociedad, incluyendo niños y adolescentes, quienes en la última década han aumentado de manera exponencial su uso a diario. La falta de información genera que padres y cuidadores permitan el uso de estos dispositivos desde edades muy tempranas, creyendo que programas educativos ayudarán en el desarrollo del niño. Se ha asociado el tiempo de pantalla excesivo con alteraciones en diversos ámbitos de la salud, tales como: sedentarismo, cambios en el peso, alimentación, sueño, desarrollo y bienestar psicológico; influyendo en el desarrollo de los mismos factores tanto internos como externos al ambiente familiar. Es por ello que se debe concientizar a la población sobre las implicaciones negativas que conlleva el uso excesivo de las pantallas, así como también la importancia de la interacción con el mundo real para un adecuado desarrollo del menor.
El vapeo o el uso de cigarrillos electrónicos es una tendencia que ha aumentado en los últimos años, afectando principalmente a jóvenes; produciendo una lesión pulmonar de la cual todavía no se tiene conocimiento exacto, sin embargo se ha correlacionado con un tipo de neumonitis aguda. Hasta la fecha, se desconoce cuál de todas las sustancias que contienen esos productos es la causante de la injuria; el acetato de vitamina E es hasta el momento la principal que ha sido asociada al daño, empero se ocuparán más estudios en las poblaciones afectadas para conocer más sobre la fisiopatología de esta enfermedad.
La colestasis intrahepática es la enfermedad hepática más común durante el embarazo. Su presentación se caracteriza por prurito asociado a alteración en pruebas de función hepática y ácidos biliares y frecuentemente se manifiesta en el segundo o tercer trimestre de embarazo. Está relacionada con una etiología multifactorial que abarca factores ambientales, hormonales y genéticos. Debido a la ausencia de guías estandarizadas, existen criterios divergentes en cuanto a su diagnóstico y manejo. Asimismo, los estudios realizados más recientemente muestran resultados que descalifican el conocimiento previo de su manejo. Las complicaciones más amenazadores son las perinatales que incluyen parto pretérmino, sufrimiento fetal agudo y muerte fetal intrauterina. Esta revisión tiene como objetivo incluir información actualizada acerca de la enfermedad, de manera que permita a los profesionales de salud crear un criterio clínico acerca del manejo integral de estas pacientes y lograr disminuir la morbilidad y mortalidad materno fetal.
La diabetes mellitus es una enfermedad de alta prevalencia en la población general, que puede presentar complicaciones agudas a quienes la padecen, siendo común que, en los servicios de emergencias, los médicos deban manejar urgencias diabéticas recurrentes como la cetoacidosis diabética, el estado hiperglicémico hiperosmolar, y la hipoglicemia. Las dos primeras entidades se presentan por un estado insulinopénico e hiperglicémico, con deshidratación severa y alteraciones del equilibrio ácido-base y electrolítico; y la hipoglicemia por su parte, es el efecto adverso más frecuente de la terapia para la diabetes, que requiere un manejo rápido para prevenir implicaciones clínicas severas para el paciente. Por la alta relevancia que tiene el manejo de las emergencias del paciente diabético, el abordaje del tema, en el presente artículo, parte de las definiciones de las complicaciones, de su fisiopatología, factores desencadenantes, presentación clínica, diagnóstico y finalmente una guía de manejo de estas complicaciones en el servicio de urgencias.
Las troponinas son parte de estructuras miocárdicas que se liberan durante el proceso infarction de daño tisular generado por los diferentes tipos de infarto o de patologías que pueden llegar a producir un efecto deletéreo en la estructura muscular. Han formado parte de los diferentes métodos de diagnóstico e incluso de seguimiento, aunque el nivel de concentración definido no ha sido estandarizado. Además, se intenta hacer una correlación entre las diferentes isoformas y su utilidad a corto y largo plazo. Es por esto que es importante mantener la perspectiva de que los marcadores de daño miocárdico orientan al clínico en la objetividad diagnóstica.
