La atresia de vías biliares (AVB) es una colangiopatía obstructiva neonatal que puede afectar a la vía biliar intra y extrahepática, como consecuencia de un proceso inflamatorio idiopático que determina una fibrosis y obliteración del tracto biliar, conduciendo al posterior desarrollo de cirrosis biliar secundaria. La metodología utilizada para el presente trabajo de investigación, se enfoca hacia una metodología orientada hacia la necesidad de indagar en forma precisa y coherente una situación. Enmarcada dentro de una revisión bibliográfica de tipo documental, ya que nos vamos a ocupar de temas planteados a nivel teórico como es Cirugía de Kasai. La técnica para la recolección de datos está constituida por materiales impresos, audiovisuales y electrónicos, estos últimos como Google Académico, PubMed, entre otros. La información aquí obtenida será revisada para su posterior análisis. La portoenterostomía de Kasai sigue siendo en la actualidad el procedimiento estándar para el abordaje de la atresia de las vías biliares, dejando atrás gracias al avance de la medicina los procedimientos invasivos totalmente abiertos para el empleo de técnicas menos invasivas por laparoscopia. Este procedimiento es eficaz en el restablecimiento del flujo biliar evitando complicaciones como una progresión a cirrosis biliar, así como es un tratamiento que puede evitar las mortalidades asociadas a esta patología, sin embargo, no hay que dejar de mencionar que, debido a la progresión de la enfermedad, se pueden presentar complicaciones como colangitis e hipertensión portal. Como cualquier otro tipo de patologías el temprano diagnóstico y abordaje, puede evitar complicaciones y muertes en edades tempranas y asegurar de esta manera una calidad de vida para el paciente. Luego de realizado el procedimiento quirúrgico, es necesario el seguimiento al paciente, ya que es vital mantener los niveles de bilirrubina adecuados. Siendo este es uno delos factores predictivos que están asociados a la supervivencia de los pacientes.
