Severe anaphylactic shock is a life-threatening situation that needs immediate treatment of progressive hemodynamic failure. We report two cases of severe anaphylactic shock treated with arginine-vasopressin (AVP): In a 42-year-old male patient anaphylactic shock was caused by the sting of a hornet. At the scene, he was found unconscious, cyanotic, with a heart rate of 130/min without measurable blood pressure. The patient was intubated and ventilated with 100% oxygen, intravenous epinephrine (1 mg over 2 min) had no effect on blood pressure. After injection of 10 IU vasopressin, followed by infusion of 40 IU vasopressin, hemodynamics could be stabilized at once. In the second case, a 47-year-old male patient was stung by a wasp. At the scene he was unconscious, blood pressure was not measurable, heart rate was 140/min. The patient was treated with 40 IU vasopressin followed by rapid infusion of 500 ml NaCl 0.9%. After injection of vasopressin, blood pressure raised to 80/50 mm Hg and heart rate decreased to 90/min. Both patients needed ventilator therapy for several days and recovered fully.
Zusammenfassung
Die schnelle Bestimmung von Laborparametern am Patientenbett (Point-of-Care[POC]-Labor) ist fester Bestandteil der klinischen Notfall- und Intensivmedizin. Insbesondere die arterielle Blutgasanalyse (BGA) sowie die Bestimmung von H?moglobin (Hb) und Elektrolyten k?nnen lebensrettende Ma?nahmen nach sich ziehen. Die vorliegende Studie untersucht den Stellenwert des POC-Labors in der pr?klinischen Notfallmedizin sowie die arterio-endexspiratorische CO2-Differenz bei beatmeten schwerkranken und schwerverletzten Patienten. In einer prospektiven Studie wurde nach Begutachtung durch die Ethikkommission pr?hospital bei kritisch kranken und verletzten Notfallpatienten (NACA???5) mittels arterieller Blutproben ein POC-Labor (BGA, Hb, Elektrolyte) am Notfallort und im Notarztwagen durchgef?hrt. Die Messungen erfolgten mit einem mobilen Analysator (Abbott i-STAT?). Zielgr??en waren die H?ufigkeit pathologischer Werte, daraus resultierende Notfalltherapiema?nahmen und die Bestimmung der arterio-endexpiratorischen Kohlendioxidpartialdruckdifferenz bei beatmeten Notfallpatienten. Bei 33 Patienten wurden insgesamt 58 POC-Labore (bis zu 3/Patient) durchgef?hrt mit insgesamt 682 Messwerten, hiervon waren 307 (45?%) pathologisch. Bei 22 (66,7?%) Patienten bestanden St?rungen des S?ure-Basen-Haushalts, bei 16 (48,5?%) Patienten bestand eine An?mie, bei 8 (24,2?%) Patienten eine Hypokali?mie und bei 6 (18,8?%) Patienten eine Hyperkali?mie. Insgesamt fand sich bei 32 (97?%) Patienten zumindest ein pathologischer Wert. Es erfolgten bei 18 (54,5?%) Patienten akute Interventionen aufgrund der Laborwerte. Die Differenz PaCO2-etCO2 bei 21 beatmeten Patienten betrug 17,2???12,9 (Mittelwert???SD), die maximale Abweichung 42,9?mmHg. Die bettseitige Bestimmung vitaler Laborparameter macht auch im Rettungsdienst wertvolle Informationen f?r die differenzierte Notfalltherapie zug?nglich. Die hohe und unvorhersagbare arterio-endexspiratorische Kohlendioxidpartialdruckdifferenz spricht gegen eine Respiratoreinstellung auf der Grundlage des etCO2 in der pr?klinischen Notfallmedizin.
