Introdução: Mesmo com o progresso gradual na área fisiopatologia e dos fatores relacionados ao prognóstico em pacientes com COVID-19, contudo não se sabe ainda se a contagem de plaquetas no plasma nestes pacientes pode ter uma correlação com o pior desfecho clinico, como óbito durante a internação na fase aguda da doença. Objetivos: O objetivo desse estudo foi avaliar quais fatores de risco e marcadores laboratoriais correlacionados ao pior prognóstico clinico, o óbito e com a contagem de plaquetas do plasma pacientes hospitalizados por COVID-19 em hospital universitário Métodos: Estudo clínico prospectivo observacional em pacientes hospitalizados por COVID-19 com mais de 18 anos internados em hospital universitário. Todos os pacientes incluídos foram avaliados quanto a sintomas e exames laboratoriais nas primeiras 24 horas e fatores de risco para doença cardiovascular (DCV). Análise estatística foi realizada para avaliar quais fatores se associaram ao desfecho óbito durante a internação. Resultados: Foram avaliados 104 pacientes internados consecutivamente pela doença no período de agosto a dezembro de 2020, com idade média de 6218 anos, sem diferença entre sexo, média de 113 dias de internação, média de dias de sintomas no momento da internação de 103 sendo que 49% precisaram de UTI e 18% faleceram. A amostra apresentou 44% de hipertensos, 14,4% com DCV conhecida e 23% de diabéticos. Nenhum fator de risco ou sintoma na admissão se associaram a maior mortalidade. Em 67 pacientes que apresentaram todos os exames, os fatores que se associaram a maior mortalidade foram idade (p=0,001) e contagem de plaquetas (p=0,002) na admissão. Nem dímero D, troponina ou PCR foram associados à gravidade pela doença. O valor de corte de contagem de plaquetas 156000 mm 3 apresentou sensibilidade de 22% e especificidade de 82% para mortalidade, com área sobre a curva de 0,75 [IC: 0,63-0,87; p=0,002). Conclusão: Grande parte dos marcadores não teve relação com a alta taxa de mortalidade, contudo a contagem de plaqueta no limite inferior da normalidade no momento da internação apresentou especificidade de 82% para desfecho desfavorável: óbito. Além disso, o fator idade teve uma relação com o pior prognóstico em pacientes internados.
Objetivos: Essa pesquisa objetivou determinar a frequência de anormalidades cromossômicas em pacientes atendidos no Ambulatório de Genética Médica de São Carlos, São Paulo, Brasil, e analisar os motivos de encaminhamento destes pacientes.Métodos: Trata-se de estudo retrospectivo, de corte transversal e abordagem quantitativa. Foi realizada revisão dos mapas de atendimento de todos os 792 pacientes atendidos no Ambulatório entre junho de 2006 e dezembro de 2018, com identificação dos pacientes com anormalidades cromossômicas diagnosticadas pelo cariótipo convencional com bandeamento G.Resultados: Anormalidades cromossômicas foram identificadas em 77 pacientes (9,7%). Cromossomopatias numéricas de autossomos ou cromossomos sexuais representaram 75,3% de todas as alterações (58/77). Cromossomopatias estruturais desequilibradas foram identificadas em seis pacientes (6/77; 7,8%) e cromossomopatias estruturais equilibradas em outros seis pacientes (6/77; 7,8%). Heteromorfismos cromossômicos foram encontrados em sete pacientes (7/77; 9,1%).Conclusões: Nossos resultados apontam para frequência de cerca de 10% de pacientes com anormalidades cromossômicas identificadas pelo cariótipo convencional, no contexto de um ambulatório de genética médica geral. Os achados ressaltam a importância da investigação citogenética adequada para diagnóstico e aconselhamento genético familiar nas situações de defeitos congênitos, deficiência intelectual, baixa estatura patológica em meninas, amenorreia primária, infertilidade, abortamento de repetição e suspeita clínica de síndromes cromossômicas.
Introdução: O número de profissionais de saúde envolvidos na assistência especializada em genética é considerado insuficiente no Brasil, e supõe-se que a maior parte dos pacientes e famílias não recebe cuidado adequado. Além disso, a disponibilidade de recursos humano e material para atendimento em genética é muito desigual nas várias regiões do país. Objetivos: Descrever a trajetória de um serviço ambulatorial de genética médica no Brasil e ressaltar a importância de organizar a rede de atenção em genética médica no país. Metodologia: Estudo quantitativo, transversal e retrospectivo, no qual foram revisados os mapas de atendimento de todos os 792 pacientes avaliados no Ambulatório de Genética Médica de São Carlos, São Paulo, entre junho de 2006 e dezembro de 2018. Resultados: Houve grande diversidade de doenças, genéticas, que acometeram pessoas de diferentes faixas etárias. Diagnóstico genético foi definido em 290 pacientes (36,6%) e afastado em 153 pacientes (19,3%); 23 pacientes (2,9%) receberam aconselhamento genético para futura descendência e em 326 pacientes (41,2%) um diagnóstico genético não foi definido nem afastado. A frequência de casos não resolvidos explicita dificuldades inerentes ao diagnóstico clínico genético e barreiras para acesso aos testes genéticos moleculares. Conclusão: Experiências como a do Ambulatório de Genética Médica de São Carlos mostram os obstáculos de se prestar um serviço de saúde adequado à população, reforçando a necessidade de organizar a rede de atenção em genética no Sistema Único de Saúde, com base na Política Nacional de Atenção Integral às Pessoas com Doenças Raras.
