In March, people living with HIV infection (PLWH) were included in the risk category of fragile people for severe COVID‐19 receiving priority access to vaccination with BNT162b2 vaccine. The aim of the study was to evaluate the immunogenicity and safety of the two doses regimen. The antibodies titer for severe acute respiratory syndrome‐related coronavirus‐2 (SARS‐CoV‐2) was evaluated after 21 days since the first administration (Time 1), 1 (Time 2), and 3 (Time 3) months post‐vaccination. Information regarding virological and immunological conditions at baseline, previous SARS‐CoV‐2 state of infection, other immunodeficiencies, current antiretroviral therapy (ART), comorbidities, and severe adverse events (SAE) to vaccination was collected. Six hundred and ninety‐seven patients were tested for quantitative anti‐spike antibodies at Time 1, 577 patients had a second detection at Time 2, and 491 patients had the third detection. Baseline characteristics of the study population are reported in Table 1. At the time of vaccine administration, all patients were on ART (except one long‐term nonprogressor); 632 (90.7%) patients had undetectable HIV‐RNA; 12 (1.7%) patients were immunosuppressed due to chemotherapy or other immunosuppressive drugs; 345 (49.5%) patients had at least one COVID‐19 related comorbidity and 155 (22.2%) had two or more comorbidities. No SAEs were reported. Final serological results are available for 694 patients after the first dose, 577 and 491 after the second and third ones, respectively; positive titer (values ≥ 50 AU/ml) was demonstrated in 653 (94.1%), 576 (99.8%), 484 (98.6%) patients, respectively. Only one patient was a nonresponder after completing vaccination, who was a newly diagnosed one for HIV infection. All vaccinations were well tolerated, with no SAEs. BNT162b2 mRNA vaccine was immunogenic and safe in PLWH.
Legionella spp. control is a critical issue in hospital with old water networks. Chemical disinfection methods are applied as a control measure over prolonged time periods, but Legionella may be resistant to chemical agents in pipeworks with low flow and frequent water stagnation. We evaluated Legionella spp. colonization in the hot water network of Italian hospitals after the installation of time flow taps (TFTs). In the period between 2017 and 2019, TFTs were installed in four hospital water networks. They were programmed in order to obtain a hot water flow of 192 L/day from each TFTs. A continuous chlorination system (chlorine dioxide) and a cold water pre-filtration device were applied in all the buildings. Before and after TFT installation, Legionella spp. was investigated at scheduled times. Before TFT installation, Legionella pneumophila was detected in all the hospitals with counts ranging from 2 × 102 to 1.4 × 105 CFU/L. After TFT installation, a loss in Legionella pneumophila culturability was always achieved in the period between 24 h and 15 days. Total chlorine concentration (Cl2) was detected in the range between 0.23 and 0.36 mg/L while temperature values were from 44.8 to 53.2 °C. TFTs together with chemical disinfection represent a method which improve water quality and disinfectant efficacy, reducing Legionella colonization in dead-end sections.
Surveillance of new cases of invasive pneumococcal disease (IPD) in Italy was started in 2007 by the Ministry of Health (MoH). In 2012, pneumococcal childhood vaccination was introduced at the national level and, in 2017, for citizens aged 65 years and over. We describe here IPD epidemiology in Italy over the past 10 years investigating the impact of the vaccine programme on disease burden. Reports of IPD cases, data on serotype and vaccination coverage (VC) data were obtained from MoH annual reports, for the period 2007–2017. IPD notification rate and proportion by year, region, age and serotype were calculated. In 2007, 525 cases were reported (rate 0.88/100 000), rising to 1703 cases (rate 2.82/100 000) in 2017. The distribution of IPD cases by age group over time registered the largest share among individuals aged 65 years and over. A decreasing trend in notification rate was observed among those aged 0–4 years. During the same period, the 24-month VC increased, ranging from 80.9% to 96.7% in 2017. Molecular data indicated re-emergence of PPSV23-specific serotypes and non-vaccine serotypes. We observed an increase in IPD notifications during 2007–2017, likely due to an improved surveillance system, at least in some regions, with the relative quota of IPD notifications decreasing among vaccinated children cohorts. Further strengthening of IPD surveillance system, including molecular and vaccine coverage data, would be needed to assess and inform pneumococcal vaccination strategies in Italy.
Adolescents engagement is fundamental to develop dedicated educational interventions. We piloted non-standard sociological methodology to assess risk perception, information sources and perceived educational needs of a group of Italian adolescents focusing on three infectious diseases. Three high-school classes students participated in a World Café event. A thematic analysis was performed. Participants showed lack of knowledge on diseases prevention. Family and school were key health information sources and social media considered unreliable. Future interventions preferences included interactive and informal sessions. We showed the utility of non-standard sociological methods to assess health knowledge among adolescents and enhance the design of dedicated interventions.
