Acute gastrointestinal illness (AGI) continues to be an important cause of morbidity and mortality among all ages. This study applied the principles of wastewater-based epidemiology for the preventive identification of potential outbreaks of acute viral gastroenteritis and hepatitis A by analyzing the presence of human enteric viruses in influents of small municipal wastewater treatment plants (WWTPs) handling domestic sewage, together with public health reports of acute diarrheal and hepatitis A disease in Costa Rica during 2013. Raw wastewater samples were collected during four seasonal periods with different rainfall levels. The presence of five human enteric viruses (rotavirus A, norovirus GI, norovirus GII, enterovirus, and hepatitis A virus) was studied by endpoint and real-time polymerase chain reaction in influents of five WWTPs. Cases of AGI were analyzed using historical public health reports of endemic levels and quartile ranges for each administrative and territorial area where the WWTPs are located and for its surrounding counties. A tendency for a higher rate of positive viral tests was present 1 week before an increase of AGI cases. Epidemiological weeks categorized as Outbreak (above the 75th percentile) and Success (below the 25th percentile) showed statistically significant differences in terms of positive viral test rates (Wilcoxon test, P = 0.05). Virological monitoring of wastewater in small WWTPs is an appropriate model for epidemiological surveillance of diarrheal and hepatitis A diseases in low- and middle-resource countries.
[Introducción]: El agua para consumo es un determinante de la salud humana; particularmente cuando se obtiene de fuentes superficiales puede ser afectada por contaminantes microbiológicos causantes de enfermedades, los cuales a su vez son influenciados por las condiciones ambientales; sin embargo, estos efectos han sido poco estudiados en Costa Rica. [Objetivo]: Determinar la influencia de variables ambientales en las concentraciones de microorganismos patógenos e indicadores fecales en un sistema de abastecimiento de agua para consumo humano. [Metodología]: Se realizó un muestreo dos veces al mes durante un año. Se midió la concentración de Cryptosporidium sp., Giardia sp. colifagos somáticos y Escherichia coli. Se analizaron las condiciones espaciales mediante el uso de sistemas de información geográfica, y las condiciones climáticas de manera descriptivas. Para el análisis de datos se utilizaron técnicas para datos censurados y un modelo de regresión de Poisson para evaluar la influencia de la intensidad de precipitación sobre las concentraciones de microorganismos. [Resultados]: Se encuentran diferencias estacionales para colifagos somáticos y E. coli. Cryptosporidium sp. y Giardia sp. son más frecuentes cuando la intensidad de precipitación es débil, mientras que colifagos somáticos y E. coli presentan mayores concentraciones conforme aumenta la intensidad de precipitación. [Conclusiones]: Las cuatro microcuencas presentan vulnerabilidad por contaminación con respecto a sus condiciones espaciales y se determina que tanto la estacionalidad climática y la intensidad de la precipitación 24 h antes afectan de manera diferente a cada uno de los microorganismos en estudio, por lo cual, se recomienda su evaluación periódica de forma independientemente.
This study describes the quality of drinking water sampled over 2 years (2018 and 2019) from 20 ASADAS (Spanish acronym for Administrative Associations for Water and Sewer Systems) in Costa Rica. The analysis included Rotavirus (RV), somatic coliphages, fecal coliforms, and Escherichia coli. The ASADAS were categorized into three regions as temperate rainy (region 1), tropical rainy (region 2), and tropical rainy and dry (region 3) according to biogeographic classification. The concentrations of fecal coliforms and E. coli were higher in samples from surface water sources from the ASADAS in region 3 compared to regions 1 and 2. RV-positive samples (24/296) were detected in drinking-water samples from regions 2 and 3 during dry and transition seasons, with higher concentrations more frequently in the dry season. In addition, somatic coliphages were detected in samples from the three regions, with higher concentrations in region 2. Furthermore, a statistically significant relationship was found between somatic coliphages and diarrheal cases, classified as outbreaks or alerts in the region. Thus, the results confirmed that somatic coliphages are a good indicator of the presence of diarrhea cases in a specific region.
We report the draft genome sequence of the multidrug-resistant Escherichia coli strain PTA A1517-5, isolated from a wastewater treatment plant in Costa Rica. The genome consists of 4,927,375 bp with a GC content of 50.57% and a total of 4,853 genes. RESUMEN. Introducción: La microcuenca del río Durazno en Costa Rica es fuente de agua para consumo humano, pero se sospecha de contaminación microbiológica por varios usos del suelo. Objetivo: Evaluar la calidad del agua de la microcuenca del Durazno desde el punto de vista fisicoquímico, microbiológico y antibiótico. Métodos: Utilizamos el índice holandés de calidad del agua; Métodos Estándar para el Examen de Agua y Aguas Residuales; y extracción en fase sólida y cromatografía líquida de ultra alta resolución con espectrometría de masas en tándem. Resultados: Si bien los parámetros fisicoquímicos indicaron una calidad de agua adecuada, microbiológicamente esta agua no es aceptable para el consumo humano. El agua tiene doxiciclina y cefotaxima, posiblemente de origen agrícola. Este es el primer reporte de Cefotaxima en agua de Costa Rica. El índice holandés no tiene en cuenta parámetros microbiológicos que también son importantes. Conclusión: El río Durazno no cumple con los estándares para consumo humano de agua y el índice holandés necesita ser complementado con parámetros microbiológicos.
Introducción: Los Chiles tiene uno de los índices de progreso social más bajos de Costa Rica. Los habitantes beben agua subterránea no tratada de un acuífero que atraviesa la frontera entre Nicaragua y Costa Rica. Objetivo: Caracterizar la calidad del agua y posibles asociaciones con el uso de la tierra. Métodos: Recolectamos seis muestras de agua de pozos durante las temporadas seca y lluviosa en 2019 y aplicamos pruebas estándar. Resultados: Los pastizales y el suelo descubierto cubren áreas más grandes que otros usos del suelo. El agua es del tipo calcio-bicarbonato, y sus valores físico-químicos son aceptables, no detectamos Escherichia coli en ninguna muestra, mientras que coliformes fecales sólo en La Trocha. Conclusión: Si se trata para eliminar los coliformes y se protege de otros efectos de la actividad humana, esta fuente será óptima para el consumo humano.
Giardia cysts stained with hot carbolfuchsin were used as internal controls in a concentration method for surface water samples. The morphological integrity of stained cysts and the stain's stability and intensity were tested with each of the chemical reagents used in the aluminum sulfate flocculation method. No alterations in morphology or color were noted. The stained cyst preparation has a low cost, high stability, and suitability for both light and immunofluorescent microscopy, making it affordable to researchers in low-and middle-income countries.
