ÖZETAmaç: Bu çalışmada acil servise başvuran ve medikal tedavi ile takip edilen sekiz tularemia hastasını sunmayı amaçladık. Materyal ve Metod: Ocak -Mayıs 2012 tarihleri arasında hastanemiz acil servisine boyunda şişlik, boğaz ağrısıve ateş şikayetleri ile başvuran hastalar dahil edildi. Tularemi tanısı mikro-aglütinasyon testi ile doğrulandı. Bulgular: Sekiz hastanın ortanca yaşı 58 yıl olup %50 si erkekti. Tularemi hastalarında en sık semptom %100 ile şiş servikal lenf nodu idi. Diğer semptomlar boğaz ağrısı, titreme, halsizlik ve ateş idi. Fizik muayanede 8 hastada lenfadenopati, bir hastada tonsillofaranjit ve yine bir hastada da ateş vardı. Hastalara tularemi tanısı ile antibiyotik tedavisi başlandı ve 3 hastaya cerrahi drenaj uygulandı. Hastaların muhtemel bulaş yolları değerlendirildiğinde kontamine kaynak suyu, kırsal alan ve ölü hayvandan bulaş olduğu değerlendirildi.Hastalar medikal tedavi ile takip edildi. Sonuç: Tularemi ülkemizde sık görülmekte olup erken tanı ve tedavi ile komplikasyonlar azaltılabilmektedir. Anahtar kelimeler: Tularemi, acil servis, enfeksiyon ABSTRACT Purpose: In this report, we describe eight tularemia cases who have been admitted to emergency department and treated with medical therapy. Material and Methods: Eight patients were admitted to our city hospital January -May 2012 with the complaints of swelling in the neck, sore throat and fever. Tularemia diagnosis was confirmed with the micro-agglutination test. Results: The median age of eight patients was 58 year and 50% of them were male. Swollen cervical lymph node (%100) was the most common symptom in the tularemia patients. Other symptoms were sore throat, chills, weakness and fever. At physical examination there were lymphadenopathy at 8 patients, tonsillopharyngytis at one patient and fever at one patient. Patients with a diagnosis of tularemia were started antibiotic and surgical drainage were applied to three patients. Possible source of contamination were source water, rural areas and dead animal. Patients have been followed with medical therapy. Conclusion: Tularemia is most common in our country and with early diagnosis and antibacterial therapy complications could be reduced.
Bu çalışmanın amacı künt toraks travmalı hasta sebebiyle travmatik asfiksi yönetimini gözden geçirmektir. Kırk yedi yaşında erkek hasta acil servise çalışırken maruz kaldığı kaza sonrası göğüs ve karın ağrısı şikayeti ile getirildi. Fizik muayenede hastanın yüz, boyun ve gövdenin meme başı hizasının üst tarafı ile üst ekstremite 1/3 proksimali siyanotikti ve her iki gözde subkonjonktival hemoraji mevcuttu. Yatak başı ekokardiyografide perikardiyal efüzyon yoktu. Akciğer tomografisinde pnömomediastinum, sternum kırığı ve pulmoner kontüzyon mevcuttu. Hasta entübe edilerek yoğunbakımda takip edildi. Travmatik asfiksi künt torakoabdominal travma ile oluşan klinik bir durumdur ve hastalar iyi travma yönetimi ile daha düşük mortalite ve morbidite ile taburcu edilebilirler.
ÖZETÖzofajit, özofagus mukozasının asit reflüsü, infeksiyon, sistemik hastalıklar, radyasyon, ilaçlar ve travma gibi birçok nedenle hasarlanmasıdır. İlaca bağlı özofajit (İBÖ) çeşitli ilaçların kullanımı ile özofagus mukozasında oluşan ülser ve ciddi hasar sonucu gelişen hastalıktır. En sık suçlananlar nonsteroidal anti inflamatuar ilaçlar (NSAİİ), klorid ve özellikle antibiyotiklerdir. Otuzaltı yaşında kadın olgu acil servise yutkunma esnasında ağrı (odinofaji) ve retrosternal ağrı şikayetleri ile başvurmuş, alınan öyküde 1 hafta önce terapotik küretaj nedeniyle doksisiklin 100mg. (2x1 po) reçete edildiği öğrenilmiştir. Hastanın ilacı kullanmaya başladığının üçüncü gününde şikayetlerinin başladığı ve dördüncü günde ilaç kullanımını şikayeleri nedeniyle kestiği ancak iki gündür ilaç kullanmamasına rağmen şikayetlerinin gerilemediği öğrenildi. Endoskopi randevusu alınarak, proton pompa inhibitör ve antiasit tedavisi başlanan olgu acil servisten taburcu edildi. Yapılan endoskopide özefagusta yaklaşık 30 cm'de 20 mm'lik segmente ülser görüldü. İBÖ'nün nadir olmamakla birlikte genelde ihmal edildiği düşünüldüğünden İBÖ tanısını acil servis çalışanlarına hatırlatmak ve hastanın tanı, tedavi ve takibini gözden geçirmek amacıyla olgu sunulmuştur. Anahtar Kelimeler: Özofajit, acil servis, doksisiklin ABSTRACTEsophagitis is a hazardous condition such as acid reflux of esophageal mucosa, infection, systemic diseases, radiation, drugs and trauma. Drug-induced esophagial injury (DIEI) is a disease with the use of variety of drugs that caused serious damage and ulcer in the mucosa of the esophagus. The most commonly implicated drugs are non-steroidal antiinflammatory drugs (NSAIDs), chloride and especially antibiotics. Thirty-six year-old female patient presented to the emergency department with odynophagia during swallowing and complaining of retrosternal pain. One week before 100 mg doxycycline (2x1 PO) for therapeutic abortion were prescribed. It was learned that in the third day of the initiation of medication, the patient's symptoms began and stopped using drug by the fourth day due to advers effect of drugs, but her symptoms didn't regressed although she didn't use them. Endoscopy appointment was taken, proton pump inhibitor and antiacid treatment was given, than patient was discharged from the emergency department. In the endoscopy, 20 mm segment esophageal ulcer was seen approximately in the 30.th cm of the esophagius. DIEI is a relatively common, although under-recognized, so this case was presented for remainding DIEI to emergency medicine personals and reweiving its diagnosis, treatment and follow-up.
