✓ The authors document a rarely encountered aneurysm of the anterior inferior cerebellar-internal auditory artery. In the absence of subarachnoid hemorrhage, the aneurysm produced a typical picture of neurinoma of the acoustic nerve. Clipping of the aneurysm resulted in almost full recovery of the patient.
The authors analyze the results of total removal of acoustic neurinomas in 120 patients operated on between January, 1967, and January, 1976. The facial nerve was preserved after surgery in 65 patients (54.2%). The use of microsurgical techniques in the last 3 years of study permitted preservation of the seventh nerve in 26 of 35 operated patients (74.3%). Of 103 patients examined postoperatively, 96 retained their full or partial capacity for work (93.2%). Ten patients (8.3%) died after surgery. In the last 3 years, 35 patients were operated on without a single death.
✓ The authors present a method of temporary eyelid closure consisting of air inflation of the subcutaneous connective tissue. This method is effective for prevention and treatment of conjunctival inflammation due to facial paralysis developing after total removal of acoustic neurinomas.
A consecutive series of 137 patients with cerebral aneurysms operated on with microsurgical techniques is presented. Occlusion of the aneurysm was performed in 133 patients. Good results were obtained in 85.5% of the entire series and in 92.6% of the patients who were in satisfactory condition preoperatively. The overall mortality rate was 5.8%, and 3.7% for the patients in satisfactory condition. The preoperative and operative factors that influenced the results are discussed.
