En diciembre de 2019 en la ciudad de Wuhan se presentaron nuevos casos por un virus, esta enfermedad recibió el nombre de COVID-19. De acuerdo con Gattinoni la neumonía por COVID-19 tiene dos fenotipos diferentes, los cuales son distintos en cuanto a su fisiopatología que se puede diferenciar desde su ingreso. El fenotipo 1 se caracteriza por representar 70% de los casos, con una complianza pulmonar mayor de 50 mL/cmH 2 O. El fenotipo 2 representa 20% de los casos y una complianza menor de 50 mL/ cmH 2 O. Se insiste en que éstos deben ser identificados tan pronto como sea posible con tomografía computarizada, la complianza de igual forma se debe medir en cuanto se conecta al paciente a la ventilación mecánica. Y con esto iniciar acciones terapéuticas y de protección pulmonar. El fenotipo 1, entre otras cosas, se debe manejar con ciertos parámetros como volumen corriente de 6 mL/kg, presión positiva al final de la espiración entre 8-10 mmHg, mientras que el fenotipo 2 se trata de cierta forma como un síndrome de distrés respiratorio agudo severo. En este trabajo tratamos de mostrar de forma secuencial este pensamiento en forma de algoritmo, con la finalidad de que todos los médicos que en algún momento estén en contacto con este tipo de pacientes puedan seguir las directrices aquí mencionadas.
En los últimos 10 años, se han publicado diversos artículos relacionados con la microbiota humana, especialmente la intestinal. Este ecosistema de microorganismos, en su conjunto resguarda un genoma más amplio que el del ser humano, lo cual le confiere capacidad de regular procesos que convergen en la fisiología de diversas vías inmunológicas y en la fisiopatología de enfermedades intestinales y extraintestinales. Aplicar estos nuevos conceptos en la evaluación clínica, paraclínica y terapéutica de un paciente hoy en día representa un reto, ya que los factores implicados en la formación de la microbiota, en los cambios que presenta a lo largo de la vida y los factores que influyen en alterar y mantener su composición, diversidad y metabolismo, son muy variables. Algunos fuera de nuestro control como el tipo de nacimiento, pero otros como la dieta, consumo de fibra y probióticos, interfieren de manera importante con modificaciones a la microbiota y posteriormente a la salud en general del huésped. La siguiente revisión consiste en una síntesis de los aspectos funcionales más relevantes de la microbiota intestinal, esto con el objetivo de difundir el tema y motivar a nuevas investigaciones enfocadas a aplicaciones clínico-terapéuticas.
Los síndromes paraneoplásicos se definen como el conjunto de signos y síntomas que aparecen en sitios distantes a la neoplasia maligna primaria o sus metástasis en el paciente oncológico, secundario a la secreción de sustancias por parte del tumor o por el desarrollo de anticuerpos dirigidos contra antígenos tumorales, los cuales desarrollan una reacción cruzada con otros tejidos. Dentro de los cánceres que con mayor frecuencia desarrollan estos síndromes se encuentra el cáncer pulmonar. La presencia de algunos de estos síndromes paraneoplásicos puede orientarnos a conocer la estirpe histológica del cáncer pulmonar. De igual forma, muchos de estos síndromes tienden a presentarse antes del diagnóstico, e incluso, algunos pueden indicarnos el pronóstico de dicho cáncer.
La trombosis de la circulación esplácnica secundaria a la pancreatitis aguda es una entidad que poco a poco ha resultado más relevante para esta patología. La incidencia es variable estimando una prevalencia de 10.2 a 17.4% en las cohortes con búsqueda intencional. La presentación clínica es dependiente del segmento del sistema circulatorio afectado y para el diagnóstico contamos con dos líneas de abordaje: los métodos que no involucran estudios de imagen, en especial marcadores como dímero D y lactato, los cuales han demostrado tener un uso limitado; y los estudios de imagen: el ultrasonido Doppler y la tomografía contrastada, dependiendo del territorio vascular afectado. En relación al tratamiento farmacológico, el uso de anticoagulación a dosis terapéutica no ha demostrado mayor riesgo de sangrado ni una reducción significativa en la mortalidad. En conclusión, la trombosis de la circulación esplácnica relacionada con pancreatitis aguda es una entidad que es más frecuente de lo que se creía previamente. A pesar de que el beneficio del tratamiento se mantenga aún sin evidencia contundente en relación a su beneficio.
