Os linfomas de tecido linfoide associado à mucosa (MALT) são linfomas não Hodgkin de células B que podem surgir em diversas áreas extra nodais, sendo mais frequentes nos estômago. A apresentação primária em reto é particularmente rara, havendo poucos relatos de casos. Por sua raridade, não há consenso sobre a melhor forma de tratamento. Objetivo: Descrever um caso de linfoma MALT em reto, em uma paciente do sexo feminino e a sua resposta ao tratamento quimioterápico R-CHOP (rituximab, ciclofosfamida, doxorrubicina, vincristina e prednisona). Métodos: Estudo descritivo na forma de relato de casos e revisão de literatura. Relato de caso: Mulher de 55 anos iniciou há 4 anos, quadro clínico de hematoquezia e tenesmo, associada a distensão e sensação de plenitude abdominal. Evoluiu com queixas de astenia e dor do tipo cólica de fraca intensidade em hipocôndrio esquerdo. Apresentava histó- rico de câncer de mama tratada cirurgicamente e com quimioterapia há 7 anos. Após investigação foi realizado o diagnóstico de linfoma MALT de reto, classificado com estádio II pela classificação de Ann Arbor. Paciente recebeu 8 ciclos de quimioterapia do regime R-CHOP e obteve remissão completa do tumor. Conclusões: Tendo em vista a raridade da apresentação do linfoma MALT primário de reto e da ausência de consenso sobre o tratamento ideal, observamos que, através do presente caso, o tratamento quimioterápico com o protocolo R-CHOP possibilita atingir remissão completa de forma eficaz e segura.
A doença de Crohn é uma inflamação crônica intestinal que pode envolver qualquer local do trato digestivo. Sua etiopatogenia ainda não é conhecida. Em apenas 4% dos pacientes com DC, o duodeno é afetado. Os dois maiores desafios da doença de Crohn são o diagnóstico e o tratamento.O prognóstico da DC é geralmente bom independente do tipo de tratamento utilizado. Essas evidências nos despertaram o interesse em relatar, um caso de doença de Crohn duodenal em um paciente do sexo masculino, de 35 anos, não tabagista, com diagnóstico de doença de Crohn duodenal há 2 anos, com o objetivo de alertar sobre a importância de um diagnóstico precoce. Este paciente apresentou-se com dor abdominal e diarreia frequente não mucosanguinolenta. O paciente permanece em tratamento com a doença em remissão. De fato, o conhecimento sobre esta doença, sua detecção precoce, seu tratamento adequado são determinantes para o melhor prognóstico. Introdução: O infarto omental é uma rara causa de abdome agudo inflamatório, sendo a torção omental uma de suas principais etiologias. A clínica do infarto omental se assemelha com as outras patologias do abdome agudo inflamatório com queixas inespecíficas. O diagnóstico é normalmente realizado durante exame de imagem ou procedimento cirúrgico. Objetivo: Descrever um caso de infarto omental que evoluiu com formação de abscesso. Relato de caso: Paciente masculino, 65 anos, procurou atendimento devido a dor abdominal difusa. Foram solicitados exames laboratoriais e TC abdominal. A TC abdominal apresentou imagem sugestiva de infarto segmentar do omento maior com áreas de necrose de permeio. Foi indicado o tratamento conservador com antibioticoterapia oral ambulatorial, analgesia e orientações para retorno. Uma semana após, retornou com relato de piora do quadro, evoluindo com aumento da dor abdominal, com maior intensidade em região abdominal esquerda e febre. Foi realizada nova TC abdominal, exames laboratoriais e EAS, para avaliar evolução da lesão. O estudo comparativo com a TC realizada 7 dias antes mostra discreto aumento da reação inflamatória da estrutura lesional acima descrita e acrescido ao espessamento da parede abdominal adjacente. Foi indicada a omentectomia da lesão e foi enviada para análise histológica. Paciente foi internado com antibioticoterapia intravenosa e após 3 dias recebeu alta hospitalar. Conclusões: Em pacientes com abdome agudo apresentando uma clínica inespecífica, é importante pensar no infarto omental, pois, apesar de ser uma patologia rara, pode gerar complicações caso ocorra erro diagnóstico ou atraso do mesmo.
