INTRODUÇÃOO câncer de mama tem sido motivo crescente de preocupação, com uma tendência mundial de aumento da sua incidência [1][2][3][4] . Para a região Centro-oeste, estima-se 36 novos casos de câncer de mama para 100.000 mulheres no ano de 2001, com uma taxa de mortalidade de 9/100.000. Assim, devem ter sido diagnosticados, na região Centro-Oeste, 880 novos casos de câncer de mama para o mesmo ano, sendo 150 desses na cidade de Goiânia 2,4 . O diagnóstico precoce do câncer de mama é um importante fator na escolha do tratamento adequado, na mortalidade e na expectativa de vida da paciente [5][6][7][8][9] . Mesmo detendo esse conhecimento, em alguns locais do país, aproximadamente dois terços dos casos de cânceres da mama são diagnosticados nos estádios avançados da doença [10][11][12] e a principal causa deste quadro parece ser a desinformação da população frente ao problema 8 . Nos países desenvolvidos, programas populacionais de detecção precoce do câncer de mama (programas de rastreamento), com mamógrafos de alta resolução, têm oferecido alguns pontos importantes: aparelhamento de diversos centros, com educação médica continuada, onde se pode oferecer mamografia adequada; maior número de diagnósticos da doença nos estádios precoces e, influenciando na redução da taxa de mortalidade para mulheres acima de 50 anos, em aproximadamente 30%. Nesses locais, a taxa de diagnósticos de lesões avançadas tem sido inferior a 10% do total dos casos de câncer de mama 7,13 . No Brasil, geralmente, o atendimento inicial da paciente com queixas mamárias é feito pelo médico ginecologista, o qual passa a ter um papel fundamental nas etapas para o diagnóstico da neoplasia. Entretanto, não se tem estabelecido através de estudo qual o conhecimento desses profissionais em relação ao diagnóstico do câncer de mama.
The BIA-ALCL is a rare type of T-cell lymphoma CD30+ e AKL−, occurring more common in women with Allergantextured implants. It presents most frequently as a late-onset accumulation of seroma fluid between the implant and less frequently as a palpable tumor mass, with malignant cells infiltrating through the capsule and surrounding tissue with potential lymph node and systemic involvement. This article describes a case report of a 65-year-old female patient with BIA-ALCL complaining of erythema in her right breast for almost 7 months. She agreed no family history of cancer and no fever. The patient was diabetic type 2, dyslipidemic, and postmenopausal taking estrogen therapy. She had undergone a breast augmentation with 215 mL polyurethane-coated implants 15 years ago. Imaging revealed right axillary lymph node enlargement, thickening of the breast parenchyma, and minimal periprosthetic seroma. The initial suspicion was infection wherefore she was submitted removal of the implants, partial capsulectomy on the right side, and total contralateral capsulectomy. Immunochemistry confirmed BIA-ALCL CD30+ e AKL− on the right and no disease on the left. Bacterioscopy was negative. A complementary surgical procedure involving removal of all the right capsules, resection of axillary palpable nodes, and reconstruction was necessary to achieve a bilateral oncoplastic mastopexy. The final diagnosis was BIA-ALCL confined capsule with negative margins and none axillary lymph nodes involvement, staging IB. No adjuvant treatment was necessary. The patient remained symptom free during follow-up examinations, and she desires a new breast augmentation.
