BackgroundAlport syndrome is an inherited renal disorder characterized by glomerular basement membrane lesions with hematuria, proteinuria and frequent hearing defects and ocular abnormalities. The disease is associated with mutations in genes encoding α3, α4, or α5 chains of type IV collagen, namely COL4A3 and COL4A4 in chromosome 2 and COL4A5 in chromosome X. In contrast to the well-known X-linked and autosomal recessive phenotypes, there is very little information about the autosomal dominant. In view of the wide spectrum of phenotypes, an exact diagnosis is sometimes difficult to achieve.MethodsWe investigated a Spanish family with variable phenotype of autosomal dominant Alport syndrome using clinical, histological, and genetic analysis.ResultsMutational analysis of COL4A3 and COL4A4 genes showed a novel heterozygous mutation (c. 998G > A; p.G333E) in exon 18 of the COL4A3 gene. Among relatives carrying the novel mutation, the clinical phenotype was variable. Two additional COL4A3 mutations were found, a Pro-Leu substitution in exon 48 (p.P1461L) and a Ser-Cys substitution in exon 49 (p.S1492C), non-pathogenics alone.ConclusionCarriers of p.G333E and p.P1461L or p.S1492C mutations in COL4A3 gene appear to be more severely affected than carriers of only p.G333E mutation, and the clinical findings has an earlier onset. In this way, we could speculate on a synergistic effect of compound heterozygosity that could explain the different phenotype observed in this family.

Reporte de un caso Emphysematous pyelonephritis in a renal transplant recipient. A case report Sr. Director:La pielonefritis enfisematosa (PNE) es una enfermedad necrosante del riñón, rara, y aún más en pacientes trasplantados renales. Se caracteriza por la presencia de gas en el parén-quima renal, el espacio peri-renal o en las vías urinarias, como consecuencia de un proceso infeccioso, causado en la mayoría de los casos, por microorganismos productores de gas, como el Escherichia coli (más del 80% de los casos) o la Klebsiella pneumoniae, debido a la fermentación mixta de glucosa 1 . En más del 80% de los casos ha sido descrita en pacientes diabéticos. La obstrucción del tracto urinario es claramente un factor de riesgo para la PNE, y afecta más al sexo femenino, con un ratio de 4:1 2 . La mortalidad por PNE es primariamente atribuida a complicaciones sépticas, casi el 78% a finales de 1970, pero en las 2 últimas décadas, se ha reducido al 21% debido a las mejoras en las técnicas de manejo 3 .La presentación clínica más característica es la de una pielonefritis severa, que rápidamente progresa a sepsis con fallo orgánico múltiple. El gold standard para el diagnóstico y la estadificación es la imagen por TAC 1 .En los casos publicados de PNE en pacientes con trasplante renal, el intervalo entre la recepción del injerto y la aparición de la PNE varía desde 2 semanas hasta 15 años 4,5 . Al-Geizawi et al. 2 proponen un esquema de clasificación de la PNE en injertos renales basado en los hallazgos del TAC y, aunque necesita ser validada prospectivamente, es una alternativa a las clasificaciones previas que se realizaron para PNE en riñones nativos. Esta clasificación orientaría para realizar un tratamiento médico o quirúrgico, según indicación.
