Bu çalışmada, doğuştan kalp hastalığı nedeniyle cerrahi uygulanacak olan düşük ağırlıklı infantlarda (5 kgʼnin altında) ultrason rehberliğinde santral ven kateterizasyonunun başarı ve komplikasyon oranlarına etkisi araştırıldı. Ça lış ma pla nı:Ekim 2014 -Ekim 2015 tarihleri arasında ultrason rehberliğinde internal jugüler venöz kateterizasyon uygulanan toplam 70 infant (38 erkek, 32 kız; hastaların ortalama yaşı <1 ay 16.4±9.5 gün [n=20; %28.6]; 1-7.5 ay 126.3±47.8 (n=50; %71.4]) retrospektif olarak incelendi. Tüm kateterizasyonlar, deneyimli iki anestezi uzmanı tarafından ultrason eşliğinde yapıldı. Hastaların demografik özellikleri, işlemin başarı oranı, kateterin takılma süresi, girişim sayısı ve komplikasyonlar dahil olmak üzere veriler kaydedildi. Bul gu lar: İşlemin başarı oranı %92.8 (n=65) idi. Hastaların %82'sinde (n=53) ilk girişimde işlem başarılı oldu. Kateterin ortalama takılma süresi (ilk ponksiyondan kateter yerleştirilmesine kadar geçen süre) 214±0.48 sn. idi. Beş hastada (%7.14) komplikasyon gelişti ve bu hastaların vücut ağırlığı 2500 g'nin altındaydı. Hastaların hiçbirinde arteriyel ponksiyon görülmedi. Bir hastada (%1.42) pnömotoraks ve dört hastada (%5.7) tekrarlayan girişimlere bağlı olarak hematom gelişti. So nuç: Çalışma sonuçlarımız ultrason rehberliğinde yapılan santral venöz kateterizasyonunun doğuştan kalp cerrahisi yapılacak 5 kg'nin altındaki pediyatrik hasta grubunda güvenli ve etkili bir teknik olduğunu göstermektedir. Objective: Regional anaesthesia is mostly preferred for elective caesarean delivery. This study aimed to compare general, epidural and spinal anaesthesia techniques in terms of their foetal and maternal effects.Methods: Parturients undergoing elective caesarean delivery were randomly divided into three groups: general anaesthesia (GA), epidural anaesthesia (EA), and spinal anaesthesia (SA). The APGAR score, foetal blood gas level, free O 2 requirement and positivepressure ventilation, phototherapy requirement, bilirubin level and weight loss were recorded. The time to the first breastfeeding, perioperative bleeding, crystalloid requirement, ephedrine consumption, first analgesic requirement, the time of defaecation and mobilization were also measured. Results:The APGAR scores and SpO 2 were lower in the GA group (p<0.05). Six newborns required O 2 supply and positive-pressure ventilation and one newborn from the GA group was intubated and transferred to the NICU. The time to first breastfeeding was prolonged in the GA group compared with the other groups (p<0.05). Postnatal 48-hour weight loss of the newborns was higher in the EA and SA groups than in the GA group (p<0.05). Peropertive Amaç: Sezaryen ile doğumlarda rejyonal anestezi çoğunlukla tercih edilir. Ayrıca ilk emzirme zamanı, perioperatif kanama, kristalloid gereksinimi, efedrin tüketimi, ilk analjezik gereksinimi, ilk defekasyon ve mobilizasyon süresi ölçüldü.Bulgular: APGAR skorları ve SpO 2 GA grubunda düşüktü (p<0,05). Altı yenidoğanda O 2 desteği ve pozitif basınçlı ventilasyon gerekti ve GA grubundan bir yenidoğan entübe edildi ve NICU'ya transfer edildi. İlk emzirme zamanı GA grubunda diğer gruplara göre uzadı (p<0,05). Yenidoğanların doğum sonrası 48 saatlik kilo kaybı EA ve SA gruplarında GA grubundan daha yüksekti (p <0,05). Peropertif kanama GA grubunda daha yüksekti. SA grubunda, efedrin ve sıvı gereksinimleri daha yüksekti ve mobilizasyon süresi daha kısa idi (p<0,05). İlk analjezik gereksinimi ve ilk defekasyon süresi EA grubunda daha kısaydı (p<0,05).
Objective: There is no consensus on the use of optimal analgesic method after pediatric circumcision, although, caudal block (CB), dorsal penile nerve block (DPNB), topical local anesthetic application and systemic analgesic administration are frequently used methods. In this study, we aimed to compare the postoperative analgesic effects and side effects, as well as parental satisfaction concerning these methods. Patients and Methods: Eighty children, aged 3-12 years, undergoing circumcision were randomized into four groups (n=20). Group Caudal Anesthesia (CA), Group Dorsal Penile Blok (DP), Group Topical Analgesia (TA), Group Systemic Analgesics (SA). The severity of pain was measured with NRS or CHEOPS scores. Time to awakening, first analgesic requirement, hospital discharge, side effects and parental satisfaction were recorded. Results: In groups CA and DP, pain scores during the first postoperative hour were lower than the pain scores of other groups and in group CA, the first analgesic requirement time was significantly longer compared to other groups (p<0.05). In the early postoperative hour, parental satisfaction was higher in group CA compared to other groups. Conclusion: In the early postoperative period, CB and DPNB reduce pain more effectively and provide more parental satisfaction than other applications, (in children who underwent) circumcision.
