Background: Oral squamous cell carcinoma (OSCC) is preceded by oral potentially malignant disorders (OPMD). Oral leukoplakia (OL) is a non-scrapable white plaque lesion on oral mucosa and one of OPMD. Lack of knowledge about signs and symptoms of OPMD and health care providers are responsible for delays in establishing the diagnosis. Objectives: to improve health care provider’s knowledge and the importance of detecting leukoplakia as an early sign of malignancy. Case: A 30-year-old woman came to the oral disease clinic at Hasan Sadikin Hospital complaining of canker sores and a lump on right tongue that had not been healed since 3 months ago. She had history of chronic irritation on tongue which was bitten by linguoversion tooth 46. Intraorally there was a solid mass, irregular edge in lateral right tongueat region tooth 46, 1.5 x 0.5 cm in size and a white plaque at region 46-48, 11 x 10 mm in size, could not be scrapped, unpainful on touched but felt sore when eating and drinking. Based on anamnesis, extraoral and intraoral examination, the working diagnosis was suspected fibroma region 46 and OL region 46-48. Case management: Pharmacological manage,ment was povidone-iodine mouthwash and referred to Oral Surgery Department. Well-differentiated OSCC was made as definitive diagnosis, based on histopathological examination and CT-Scan which showed bilateral lymph nodes enlargement. The patient was referred to Haemato-oncology Department for chemotherapy. Conclusion: it is important to do histopathological examination and CT-Scan as an effort for establishing OL as the early sign of malignancy. Pendahuluan: Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS) adalah reaksi obat yang jarang terjadi, tetapi menimbulkan reaksi klinis yang berat, berupa demam, erupsi kulit, dan keterlibatan organ internal. Terapi medikasi untuk pasien DRESS berupa pemberian kortikosteroid high potent memiliki faktor risiko terjadinya kandidiasis oral selain faktor gangguan imunitas secara sistemik akibat DRESS. Penulisan laporan kasus bertujuan untuk memaparkan faktor risiko dan tatalaksana kandidiasis oral pada pasien DRESS. Laporan kasus: Seorang laki-laki berusia 51 tahun dirawat selama 14 hari dengan diagnosis DRESS oleh bagian Dermatologi dan Venereologi Rumah Sakit Hasan Sadikin Bandung. Terapi intravena yang diberikan adalah Deksametason 20 gram/hari, Vitamin K 3x1 ampul/hari, Omeprazole 1x40 gram/hari, dan Siprofloksasin 2x400 gram/hari. Terapi per oral yang diberikan adalah: Cetirizine 1x10 gram/hari, Curcuma 3x1 kapsul/hari, Asam folat 1x1 tab/hari, Asetilsistein 2x2 mg/hari, serta Callos 1x500 mg/hari. Pasien kemudian dikonsulkan ke bagian Ilmu Penyakit Mulut karena memiliki keluhan sakit pada rongga mulut, tenggorokan, dan bibir sejak 3 hari di RS. Riwayat sariawan berulang disangkal. Pasien tidak pernah menyikat gigi selama rawat inap. Tatalaksana kandidiasis oral diberikan Nystatin in oral suspension 4x200.000 IU/hari, Chlorhexidine digluconate 0,12%, asam hialuronat, NaCl 0,9%, serta Mikonazol krim 2% dan racikan deksametason salep 0,002% diberikan terkait lesi pada bibir dan dioleskan pada sudut mulut. Simpulan: Faktor risiko kandidiasis oral pada pasien DRESS adalah penggunaan kortikosteroid secara sistemik, gangguan imun terkait DRESS, dan kebersihan rongga mulut yang buruk. Tatalaksana kandidiasis oral diberikan adalah antifungal dan antiseptik.Kata kunci: drug reaction with eosinophilia and systemic symptoms; faktor risiko; kandidiasis oral ABSTRACTIntroduction: Drug reaction with eosinophilia and systemic symptoms (DRESS) is a rare drug reaction but causes severe clinical reactions in the form of fever, skin eruptions, and involvement of internal organs. Medical therapy for DRESS patients in the form of high potent corticosteroids has a risk factor for oral candidiasis and systemic immune disorders. This case report aimed to describe the risk factors and management of oral candidiasis in DRESS patients. Case report: A 51-year-old male was treated for 14 days with a diagnosis of DRESS by the Department of Dermatology and Venereology of Hasan Sadikin Hospital, Bandung. Intravenous therapy administered was 20 grams/day of Dexamethasone, 3x1 ampoules/day of Vitamin K, 1x40 grams/day of Omeprazole, and 2x400 grams/day of Ciprofloxacin. Oral therapy was 1x10 grams/day of Cetirizine, 3x1 capsules/day of Curcuma, 1x1 tab/day of folic acid, 2x2 mg/day of Acetylcysteine, and 1x500 mg/day of Callos. The patient was then referred to the Department of Oral Medicine because of complaints of pain in the oral cavity, throat, and lips after hospitalisation for three days. History of recurrent thrush was denied. The patient never brushed his teeth during hospitalisation. The management of oral candidiasis was the administration of Nystatin in oral suspension with the dose of 4x200,000 IU/day, 0.12% Chlorhexidine digluconate, hyaluronic acid, 0.9% NaCl, and 2% topical Miconazole, and a mixture of 0.002% dexamethasone ointment, which was given related to lesions on the lips, and topically administered in the corner of the lips. Conclusion: The risk factors for oral candidiasis in DRESS patients are systemic use of corticosteroids, DRESS-related immune disorders, and poor oral hygiene. Pendahuluan: Terapi Human Immunodeficiency Virus/ Acquired Immunodeficiency Syndrome (HIV/AIDS) dengan antiretroviral (ARV) menghasilkan tingkat respon yang bervariasi terhadap rongga mulut. Beberapa penelitian menunjukkan respons yang baik tetapi dapat juga menimbulkan Immune Reconstitution Inflammatory Syndrome (IRIS) atau dikenal sebagai komplikasi terapi ARV. Lesi oral pada pasien HIV/AIDS yang terbentuk setelah menggunakan terapi ARV dapat dikategorikan sebagai IRIS. Ulasan sistemik ini bertujuan untuk mengetahui manifestasi oral yang berkaitan dengan IRIS atau terjadi setelah terapi ARV pada pasien HIV/AIDS. Tinjauan Pustaka: Ulasan sistematik dilakukan dengan metode PRISMA. Kriteria inklusinya adalah artikel dengan jenis clinical trial yang bersumber dari pencarian elektronik didapatkan full text melalui portal PubMed, Research Gate dan Google Scholar dengan menggunakan kata kunci: HIV/AIDS, IRIS, dan manifestasi oral dari tahun 2011 hingga Desember 2019. Hasil yang teridentifikasi sebanyak 6 artikel. Hasil penelitian menunjukkan lesi oral pada HIV/AIDS yang terkait dengan IRIS yaitu sarkoma kaposi 29,2%, OHL 16,9%, kandidiasis oral 11,5%, dan RAS 5,6%. Insiden terjadinya IRIS sekitar 10-27% pada pasien HIV/AIDS yang memulai terapi ARV, namun prevalensinya lebih tinggi pada pasien dengan jumlah CD4 rendah. Simpulan: Manifestasi oral yang terkait dengan IRIS pada pasien HIV/AIDS terbanyak adalah sarkoma kaposi, OHL, kandidiasis oral, dan RAS.
Pandemi Covid-19 menyebabkan gangguan psikosomatis berupa burning mouth syndrome (BMS), yaitu suatu kumpulan gejala rasa sakit dan panas pada satu atau beberapa struktur mulut dengan tampilan mukosa normal. Artikel ini membahas pengaruh gangguan psikosomatis terhadap nilai quality of life (QoL) pada pasien BMS di masa pandemi Covid-19 pada dua kasus. Kasus pertama, perempuan usia 74 tahun dalam masa menopaus, mengeluhkan rasa mulut kering, perih pada lidah, bibir atas, dan rasa pengecapan berubah sejak 3 tahun lalu. Terdapat riwayat hipertensi, mengonsumsi amlodipine sejak 5 tahun terakhir. Pemeriksa-an ekstra dan intra oral tidak ditemukan kelainan. Kasus kedua, laki-laki usia 36 tahun, keluhan nyeri yang tidak spesifik, rasa kebas pada ujung lidah sejak 1,5 tahun lalu, dan mulut terasa kering. Pasien mengalami kecemasan yang sulit dijelaskan. Kon-sultasi ke bagian psikiatri, penyakit dalam, dan saaraf namun tidak ditemukan kelainan. Kedua pasien terjadi penurunan berat badan. Berdasarkan anamnesis dan pemeriksaan klinis kedua kasus didiagnosis BMS. Penatalaksanaandari Ilmu Penyakit Mulut, diberikan obat kumur yang mengandung Aloe vera, sodium chloride, essential oils serta memberikan informasi, edukasi, dan dukungan moral terkait keluhannya. Disimpulkan bahwa gangguan psikosomatis berpengaruh terhadap QoL pada pasien BMS di masa pandemi Covid-19.Kedua pasien mengalami penurunan QoL. Pentingnya pendekatan medis dan psikologis memban-tu meningkatkan QoL pada pasien BMS.
