Verbal and physical abuse were highly prevalent among this population of older Chinese with dementia. A higher level of caregiver stress is related to a higher level of verbal, but not physical abuse inflicted by the caregivers of these older persons with dementia.
The number of older victims of domestic violence is expected to increase drastically in Asia as many countries are experiencing rapid population aging. In 2012, 11% of the population in Asia were aged 60 years and over. This is expected to rise to 24% by 2050. This article discusses the unique features of Asian cultures that are relevant to the understanding of elder abuse and summarizes the existing literature looking at the prevalence and risk factors of such abuse in Asian populations.
This study examined the prevalence of elder abuse of Chinese in Hong Kong and also assessed its impact on their psychological functioning. A total of 355 (120 males, 235 females) elderly Chinese residing in Hong Kong responded to a questionnaire administered individually. Results indicated prevalence rates of 2% for physical abuse and 20.8% for verbal abuse. These rates were higher than those reported by a telephone survey of similar respondents. Rates did not differ significantly for men and women. Data from the General Health Questionnaire showed that abused elders, as compared with nonabused elders, scored significantly higher on overall psychological distress. Abused participants were also significantly more dependent on their caregivers than nonabused participants. This difference did not, however, predict the level of psychological distress after accounting for physical abuse and verbal abuse.
This study is the first to establish an association between care recipients' agitated behaviors, burnout, and abuse by family caregivers using prospective data. The present results confirmed that care recipients' agitated behaviors and caregivers' symptoms of burnout are associated with family caregivers' abusive behaviors. Caregiver burnout did not mediate the association between agitated behaviors and abuse. Proper management of care recipients' agitated behaviors and adequate support provided to family caregivers may help prevent the development of new cases of elder mistreatment.
More dialogue and education are needed about end of life issues in the early phase of dementia. Nurses should be aware of the cultural implications of surrogate decision-making for Chinese family caregivers.
