ResumenIntroducción: la Tuberculosis infantil representa del 6 al 10 % de los casos de TB, la OMS estimó para el 2017 unos 560 (rango: 380 -740) casos en menores de 15 años en República Dominicana 1 . De los 122 casos diagnosticados en RD, 53 fueron en nuestro hospital. Introducción: la TB infantil es considerada como un evento centinela de transmisión de la enfermedad y su control no ha sido una prioridad. República Dominicana es un país de elevada incidencia de TB y los datos reportados de TB infantil apuntan a una baja detección de casos y un sub diagnóstico. Objetivo: conocer las características clínico epidemiológicas de los casos de Tuberculosis infantil diagnosticados en el Hospital Infantil Dr. Robert Reid Cabral. 2007- 2012. Métodos: estudio descriptivo. Se revisaron los expedientes clínicos de los pacientes menores de 15 años de edad diagnosticados y seguidos en el programa de tuberculosis infantil del Hospital Infantil Robert Reid Cabral en los años 2007-2012. Resultados: se evaluaron 249 pacientes, 81% con diagnóstico de TB infantil y 19% con Infección Latente por Tuberculosis. 56.2% tenía menos de 5 años de edad y 61.4% tenían cicatriz de vacuna BCG. 63.5% de los casos refería el contacto con un caso de Tuberculosis, y en el 70.0% era intradomiciliario. La manifestación pulmonar representó el 63.4% de los casos TB y el 27.3% tenían baciloscopia positiva. Conclusión: existen diferencias importantes en el diagnóstico y manejo de la tuberculosis infantil y la TB en el adultos. Estas deben ser considerados en la implementación de estrategias para el control; con acciones integradas, un enfoque orientado no solo al niño, sino también a la familia y tomando en consideración los determinantes sociales
Se realizó un estudio descriptivo, retrospectivo, de corte transversal, para determinar la función pulmonar en falcémicos homocigotos de 5 a 15 años en la consulta de hematología del Hospital Materno Infantil San Lorenzo de Los Mina, durante el período agosto-octubre de 2015. De un total de 200 pacientes, 23 cumplieron con los criterios de inclusión. La edad más afectada es entre 10-15 años, equivalente a un 57%. El sexo masculino fue el más frecuente, con el 52%. La neumonía fue la enfermedad pulmonar previa más común, con un 48%. El 30% de los niños cursó con complicaciones, siendo el Síndrome Torácico Agudo el más reportado, con 6 casos (26%). Todos los pacientes tenían aplicada la vacuna antineumocócica. El 74% había sido transfundido. El tratamiento de sostén en el 70% de los casos era la hidroxiurea, y una combinación de hidroxiurea más terapia transfusional, en el 13% de los pacientes. Los niveles de hemoglobina más frecuentes correspondieron a la escala de 3-6 gramos y 18-27%, para un total de 11 casos. El patrón ventilatorio más frecuente fue el restrictivo en el 78% de los pacientes.
ResumenEn este trabajo se presenta la revisión de 95 casos de pacientes menores de 18 años, 61 de los cuales son menores de 5 años (64.2%). El 74% de ellos tuvo un tiempo de exposición mayor de un año a la tuberculosis antes de la evaluación; el 94.7% (90) refirió contacto drogoresistente, mientras el 92% (83) fue intradomiciliario.En cuanto a los resultados, hubo 5 casos de fracaso a tratamiento con drogas de primera línea, 20 pacientes sintomáticos respiratorios (tos, fiebre, dificultad respiratoria) (21%), así como con adenopatías cervicales y pérdida de peso. La prueba de tuberculina (PPD) aplicada a 71 pacientes (75%) reportó 44 resultados positivos (62%). De las radiografías de tórax al 100% de los pacientes, 14 de ellas fueron patológicas (14.7%). Hubo 9 baciloscopías positivas (26.4%) y 10 cultivos positivos (52%), 79 pacientes resultaron contactos sanos (83%) y 16 pacientes (17%) iniciaron su tratamiento con el diagnóstico de Tuberculosis Multidrogo Resistente (TB-MDR).De los casos de TB-MDR confirmadas o probables, aparecen 8 pacientes de 11 a 15 años (50%) y 2 menores de 1 año de edad; 9 pacientes (56.2%) TB-MDR nunca antes tratados (primarios) y 97 secundarios al uso de drogas de primera línea. Asimismo, PPD positiva en 10 pacientes y radiografía patológica en 15. La baciloscopía resultó positiva en 9 pacientes y el cultivo en 10. Resistencia a Isoniacida (H) y Rifampicina (R) observada en 10 pacientes (62%), patrones de resistencia similares al caso índice. Resumiendo, 14 casos fueron pulmonares (87.6%) y 2 ganglionares (12.5%). Los contactos sanos seguidos por dos años no desarrollaron la enfermedad; mientras que los pacientes con diagnóstico confirmado o presuntivo de TB-MDR recibieron tratamiento con el esquema estandarizado (inyectables, Pirazinamida, Etionamida, Cicloserina, Quinolonas) por 18 meses, mostrando una curación del 75% (12). Dos pacientes abandonaron el tratamiento antes de finalizar y solamente hubo un fracaso a tratamiento.A modo de conclusión, encontramos tardanza en la evaluación de los contactos; 17% de los pacientes evaluados fueron diagnosticados con TB-MDR; la curación se evidenció en el 75% de ellos; y, por último, los pacientes sanos seguidos sin tratamiento no desarrollaron la enfermedad, lo cual nos ubica dentro de lo esperado en manejo de TB-MDR a nivel mundial.Palabras clave: tuberculosis; drogoresistencia; tuberculosis; República Dominicana.
