This paper provides a critical review of the literature currently available relating to aphasic discourse. It outlines the major approaches which have been taken to analysis, differentiating the structuralist and functionalist frameworks in particular and discusses the resultant gap existing in aphasiology research between microstructural linguistic aspects of discourse and macrostructural/pragmatic aspects. Studies addressing lexical, syntactic, semantic, pragmatic and conversational aspects of discourse, as well as those focusing on specific aspects such as cohesion and text macrostructure are discussed and placed in a theoretical perspective. The different methodologies involved in the various studies are critically examined, with implications for using different elicitation techniques in particular discussed.

BackgroundWhiplash injuries are among the leading injuries related to car crashes and it is important to determine the prognostic factors that predict the outcome of patients with these injuries. This meta-review aims to identify factors that are associated with outcome after acute whiplash injury.Materials and methodsA systematic search for all systematic reviews on outcome prediction of acute whiplash injury was conducted across several electronic databases. The search was limited to publications in English, and there were no geographical or time of publication restrictions. Quality appraisal was conducted with A Measurement Tool to Assess Systematic Reviews.ResultsThe initial search yielded 207 abstracts; of these, 195 were subsequently excluded by topic or method. Twelve systematic reviews with moderate quality were subsequently included in the analysis. Post-injury pain and disability, whiplash grades, cold hyperalgesia, post-injury anxiety, catastrophizing, compensation and legal factors, and early healthcare use were associated with continuation of pain and disability in patients with whiplash injury. Post-injury magnetic resonance imaging or radiographic findings, motor dysfunctions, or factors related to the collision were not associated with continuation of pain and disability in patients with whiplash injury. Evidence on demographic and three psychological factors and prior pain was conflicting, and there is a shortage of evidence related to the significance of genetic factors.ConclusionsThis meta-review suggests an association between initial pain and anxiety and the outcome of acute whiplash injury, and less evidence for an association with physical factors.Level of evidenceLevel 1.Electronic supplementary materialThe online version of this article (doi:10.1007/s10195-016-0431-x) contains supplementary material, which is available to authorized users.

We examine how two sociological traditions account for the role of femininities in social domination. The masculinities tradition theorizes gender as an independent structure of domination; consequently, femininities that complement hegemonic masculinities are treated as passively compliant in the reproduction of gender. In contrast, Patricia Hill Collins views cultural ideals of hegemonic femininity as simultaneously raced, classed, and gendered. This intersectional perspective allows us to recognize women striving to approximate hegemonic cultural ideals of femininity as actively complicit in reproducing a matrix of domination. We argue that hegemonic femininities reference a powerful location in the matrix from which some women draw considerable individual benefits (i.e., a femininity premium) while shoring up collective benefits along other dimensions of advantage. In the process, they engage in intersectional domination of other women and even some men. Our analysis re-enforces the utility of analyzing femininities and masculinities from within an intersectional feminist framework.

Since its discovery almost 30 years ago, the fetal alcohol syndrome (FAS) has been characterized in the USA, as a major threat to public health. In part because FAS resonated with broader social concerns in the 1970s and 1980s about alcohol's deleterious effect on American society and about a perceived increase in child abuse and neglect, it quickly achieved prominence as a social problem. In this paper, we demonstrate that, as concern about this social problem escalated beyond the level warranted by the existing evidence, FAS took on the status of a moral panic. Through examples taken from both the biomedical literature and the media about drinking during pregnancy, we illustrate the evolution of this development, and we describe its implications, particularly how it has contributed to a vapid public policy response.
