Background: Oral lesions represent one of the most important clinical symptoms of systemic lupus erythematosus (SLE), an autoimmune disease with a high degree of clinical variability rendering it difficult to arrive at a prompt and accurate diagnosis. There are many unknown causes and multiple organ systems involved, with the result that permanent organ damage may occur before treatment commences. Purpose: The purpose of this case report is to discuss the importance of recognizing the lesions related to SLE which may help dentists to make an early diagnosis. Case: A 17-year-old female patient was referred by the Internal Medicine Department with a suspected case of SLE. Prior to admittance to the hospital, the patient was diagnosed with tuberculosis. A subsequent extraoral examination revealed ulceration with a blackish crust on the upper lip. An intraoral examination showed similar ulceration covered with a blackish crust on the labial mucosa accompanied by central erythema in the hard palate. Blood tests indicated decreased levels of hemoglobin, hematocrit and platelets, but increased levels of leukocytes. A diagnosis of oral lesions associated with SLE and angioedema was formulated. Case management: The patient was given 1% hydrocortisone and vaseline album for extraoral lesions, while 0.2% chlorhexidine gluconate and 0.1% triamcinolone acetonide was used to treat intraoral lesions. An improvement in the oral lesions manifested itself after two weeks of treatment. Conclusion: Early detection of oral lesions plays a significant role in diagnosing SLE. It is important for the dentist to recognize the presentation of diseases that may be preceded by oral lesions. A multidisciplinary approach and appropriate referrals are necessary to ensure comprehensive medical and dental management of patients with SLE.
Dentists and dental students have an ethical responsibility to provide treatment for human immunodeficiency virus (HIV) patients, since oral lesions are common to occur in HIV patients. The purpose of this paper was to present a systematic review of current published literature regarding the basic medical knowledge of HIV/acquired immune deficiency syndrome (AIDS) among dentists and dental students. HIV infection has a high-rate of morbidity and mortality, and this disease is a devastating public health issue. A true understanding of HIV/AIDS is still lacking in developing countries. Among dentist 69.8% reported that absence of knowledge was their main concern in treating HIV patients. High-risk category for cross-contamination occurs in dentists and dental students. Inadequate knowledge of transmission process, diagnosis, treatment, and ignorance about the disease has led to a fear and HIV phobia. Negative attitude and willingness to treat HIV patients are influenced by lack of knowledge. The positive result in providing care for HIV patients has been proven through appropriate training of dentists in terms of HIV patient care in clinical experience. The knowledge and practice of dentists and dental students regarding the disease are strengthened by an innovative education. Knowledge of HIV/AIDS and positive attitude towards people with HIV/AIDS are crucial for dental students. There is a need for continued medical education for dentists and dental students on issues related to HIV infection.
Difteri kembali mewabah di Indonesia. Kementerian Kesehatan menetapkan status kejadian luar biasa dengan jumlah 907 kasus dari 29 provinsi hingga 25 Desember 2017. Jawa Barat merupakan provinsi dengan kasus difteri kedua terbanyak dengan jumlah 123 kasus dan jumlah kematian 13 kasus. Difteri merupakan penyakit infeksi akut disebabkan Corynebacterium diphtheriae menghasilkan eksotoksin penyebab kerusakan jaringan, pembentukan pseudomembran dan edema jalan nafas dapat berakhir pada kematian. Penurunan kondisi umum ini dapat menjadi faktor predisposisi mengubah Candida albicans komensal di rongga mulut menjadi patogen oportunistik sehingga mampu menyebabkan infeksi yang memperberat kondisi pasien. Tujuan laporan kasus ini untuk membahas manajemen perawatan multidisiplin ilmu pada pasien dengan manifestasi oral pasca difteri. Laporan kasus: Pasien laki-laki berusia 57 tahun dirujuk dari Departemen Ilmu Penyakit Dalam dengan diagnosis tonsilitis difteri disertai kandidiasis orofaringeal. Terdapat keluhan nyeri menelan, disertai timbulnya multipel ulser dan bercak putih pada mukosa oral, serta plak tebal putih kekuningan di posterior rongga mulut. Diagnosis ditegakkan sebagai lesi oral terkait difteri disertai kandidiasis orofaringeal. Sakit menelan menyebabkan pasien kekurangan asupan nutrisi. Medikasi yang diberikan meliputi nistatin oral suspensi, multivitamin, antiseptik klorheksidin glukonat 0,2%, serta diet cair tinggi kalori. Ulser mengalami penyembuhan, bercak putih menipis dan hilang dalam waktu 6 hari pemberian terapi. Manajemen perawatan lesi oral yang adekuat melalui terapi farmakologis, terapi penunjang dan edukasi yang komprehensif memiliki efikasi bagi pasien. Simpulan: Manajemen perawatan dengan manifestasi oral difteri berhasil dilakukan dengan melalui terapi farmakologis, terapi penunjang dan edukasi yang komprehensif. penekanan kebersihan mulut pasien, karena kebersihan gigi dan mulut dapat mencegah timbulnya infeksi oportunistik yang dapat memperparah keadaan rongga mulut.
ABSTRAKPendahuluan: Necrotizing ulcerative stomatitis (NUS) merupakan inflamasi akut yang ditandai destruksi, ulserasi serta nekrosis epitel, jaringan ikat dan papila. Umumnya terjadi pada pasien malnutrisi dan Human Immunodeficiency Virus (HIV) seropositif. Lesi awal berupa necrotizing gingivitis kemudian berkembang menjadi necrotizing periodontitis, selanjutnya menjadi NUS. Gambaran klinis kasus NUS kadang serupa dengan Erythema multiforme (EM), sehingga perlu dilakukan pemeriksaan penunjang yang dapat menegakkan diagnosa dengan tepat. Tujuan laporan kasus ini melaporkan cara menegakkan diagnosis NUS yang menyerupai EM secara tepat pada pasien dengan suspek infeksi HIV. Laporan kasus: Pasien laki-laki berusia 49 tahun dirujuk ke departemen Ilmu Penyakit Mulut dengan diagnosis EM disertai suspek terinfeksi HIV. Keluhan utama berupa rasa nyeri disertai sariawan pada lidah dan bibir bagian dalam. Pemeriksaan ekstraoral tidak ada kelainan, pada intraoral ditemukan ulser multipel dilapisi sloughing kekuningan pada mukosa labial, mukosa bukal, dorsal lidah, lateral lidah dan ventral lidah. Gingiva anterior rahang bawah terdapat lesi ulseratif disertai jaringan nekrosis. Pemeriksaan darah menunjukkan penurunan hematokrit, peningkatan leukosit, serta negatif pada pemeriksaan tes HIV. Actinomyces naeslundii ditemukan pada pemeriksaan mikrobiologi. Ditegakkan diagnosis NUS dan terapi yang diberikan amoxicillin 500mg, metronidazol 500mg, chlorhexidine gluconate 0,2% dan asam folat. Lesi oral mengalami perbaikan dalam 2 minggu. Simpulan: Lesi oral mengalami perbaikan secara signifikan setelah kontrol ketiga atau seminggu dari kunjungan pertama, pasien sudah dapat makan tanpa rasa nyeri. Gambaran klinis lesi mukosa oral yang khas, serta pemeriksaan penunjang mikrobiologi berperan dalam menunjang diagnosis NUS. Tes HIV perlu dilakukan untuk mengkonfirmasi status infeksi HIV pada pasien. Dokter gigi sebaiknya mengenali tanda dan gejala NUS, sehingga dapat menegakkan diagnosis dan memberikan terapi adekuat serta mencegah meluasnya kerusakan jaringan.
