A ferramenta de gerenciamento de riscos Bow-Tie consiste na elaboração de um diagrama que auxilie a análise de uma ocorrência descrevendo graficamente a relação das causas, consequências, barreiras e medidas para evitar e minimizar as consequências. O evento é posicionado no centro, as causas à esquerda, as consequências à direita, as barreiras entre a causa e o evento, bem como as medidas de reparação e mitigação para consequências são definidas, permitindo a visualização do sistema. É uma ferramenta chave por apresentar uma descrição didática, assim como uma visão geral e equilibrada do evento, assim auxiliando na tomada de decisão. Objetivo: Descrever a experiência da utilização da ferramenta para gerenciamento de risco de erros de medicação em unidade de terapia intensiva pediátrica.
em hospitais tem o objetivo de integrar colaboradores recém-admitidos, acolhendo e informando sobre os processos de trabalho de setores que proporcionam impacto direto na assistência ao paciente; todavia, não contempla atividades específicas de outras áreas. A Unidade de Terapia Intensiva Pediátrica têm como principal característica o atendimento de crianças críticas, que necessitam de profissionais especializados para o cuidado; diante da sua ampliação de 10 para 20 leitos, os gestores se viram com o desafio de capacitar um número expressivo de novos servidores em curto período, mantendo-se a qualidade do atendimento no Serviço. Este trabalho tem como objetivo relatar as etapas e os processos desenvolvidos na admissão de novos colaboradores. Metodologia: Um "brainstorming" dos gestores da Pediatria com a Equipe de Enfermagem definiu os temas mais relevantes para que se atendessem às especificidades da Unidade, montando-se um treinamento exclusivo dentro da área. Quatro tópicos foram levantados: Atendimento de Urgências, Materiais e Equipamentos, Anotação de Enfermagem da criança crítica, e Terapia Intravenosa; estes foram divididos em Estações, com treinamentos teóricos e práticos. Resultados: Com a ampliação dos leitos já confirmada para maio de 2019, foram convocados 8 Enfermeiros e 32 Técnicos de Enfermagem da lista de espera de um processo seletivo já em andamento na Instituição, não sendo possível o remanejamento entre setores desses colaboradores devido a regras administrativas. O Serviço de Educação Continuada da Instituição criou e aplicou para os recém-admitidos 4 estações de treinamento com um total de 6 horas, utilizando-se de abordagens teóricas e práticas com as principais características do setor. Contou-se com a participação da Equipe de Enfermagem da Unidade, que ministrou as aulas com envolvimento e compromisso, realizando intervenções educativas para compartilhar seus conhecimentos e experiências profissionais. Apenas 3 dos 40 colaboradores foram dispensados por não atenderem aos padrões da Unidade, e os indicadores de qualidade da Enfermagem se mantiveram dentro da média anterior a este projeto, quando o percentual de profissionais experientes era maior. Considerações finais: A implementação pelos Serviços de Enfermagem Pediátrica e de Educação Continuada de um treinamento específico para profissionais ingressantes na UTI Pediátrica proporcionou-lhes o conhecimento mínimo necessário ao desenvolvimento das suas atividades. Demonstrou-se, assim, que investir em treinamento para capacitação em áreas específicas é fundamental para garantir a qualidade da assistência, além de não onerar as Instituições com desligamentos desnecessários.
