RESUMOÉ descrito um envenenamento pela ingestão de vísceras de um baiacu-pintado (Sphoeroides testudineus) por uma criança de dois anos, que apresentou sudorese fria, fraqueza muscular progressiva, parada cardiorrespiratória e morte. ABSTRACTA case of poisoning resulting from ingestion of viscera from a spotted puffer fish (Sphoeroides testudineus) by a two-year-old child is described. The child presented cold sweating, progressive muscle weakness, cardiorespiratory arrest and death. The risks of consuming the meat and viscera of puffer fish, which is a common occurrence in certain regions of Brazil, are discussed. Key-words:Tetraodontidae. Sphoeroides. Puffer fish. Brazil.Os baiacus ou peixes-bola são peixes venenosos das famílias Tetraodontidae e Diodontidae, comuns na costa brasileira. São muito fáceis de identificar devido ao comportamento de aumentarem o volume corporal através da ingestão de ar ou água. Tal comportamento se justifica pelo fato do diâmetro do peixe aumentar e os predadores não poderem engoli-lo. Os baiacus mais importantes no Brasil são os baiacus-araras ou baiacus-lisos (Lagocephalus sp) e os baiacus-pintados (Sphoeroides sp). O primeiro gênero atinge até dois quilogramas de peso, mas os baiacuspintados são menores chegando a pesar até um quilograma. A carne destes peixes tem bom sabor e é apreciada em determinadas regiões do Brasil (V Haddad Jr: comunicação pessoal, 2009). Os gêneros acima pertencem à família Tetrodontidae e outros baiacus, que tem espículas recobrindo o corpo, são classificados na família Diodontidae.A tetrodotoxina (TTx) é a principal neurotoxina encontrada nos baiacus e pode ser isolada em maiores concentrações nas vísceras (especialmente gônadas, fígado e baço) e na pele do peixe 1 . O peixe não produz as toxinas, apenas as armazena em alguns órgãos corporais, usando-a como defesa contra predadores 2 . Outros animais terrestres e aquáticos também podem armazenar TTx, como alguns moluscos, salamandras, peixes e rãs das florestas tropicais (família Dendrobatidae) 1,2 .A TTx é uma toxina termo-estável, que não sofre ação pela cocção, lavagem ou congelamento. Seu nível é sazonal, e as maiores concentrações são encontradas nas fêmeas em época reprodutiva. Atua bloqueando os receptores de sódio voltagem-dependentes, impedindo a despolarização e a propagação do potencial de ação nas células nervosas. Esta ação ocorre nos nervos periféricos motores, sensoriais e autonômicos, tendo ainda ação depressora no centro respiratório e vasomotor do tronco encefálico 1,2,3 . As manifestações neurológicas surgem em algumas horas e cursam com parestesias periorais (sugestivas do acidente) e nas extremidades, fraqueza muscular, mialgias, vertigens, disartria, ataxia, dificuldade de marcha, distúrbios visuais e outros sintomas. Com o agravamento das manifestações neurológicas, surgem convulsões, dispnéia e parada cardiorres...
Objetivo: descrever as percepções sobre estresse ocupacional de enfermeiros que atuaram em Unidades de Terapia Intensiva com pacientes infectados pela COVID-19. Materiais e Métodos: estudo descritivo-exploratório, de abordagem qualitativa, com 29 enfermeiros atuantes em cinco Unidades de Terapia Intensiva públicas e privadas de referência para a COVID-19 em Pernambuco, Estado do Nordeste brasileiro. As entrevistas foram realizadas entre maio e setembro de 2020, sendo audiogravadas e, posteriormente, transcritas. A análise foi feita utilizando-se a condensação de significados. Resultados: a presença dos temas estresse, medo, insegurança, desestímulo, despreparo, falta de estrutura e cansaço foi constatada, com destaque para os fatores estresse e angústia. Não obstante, demonstrou-se, também, preocupação com a família, tanto pelo fato do isolamento quanto pelo risco de contaminação, além do medo do isolamento social. Considerações Finais: evidenciados resultados que demonstraram a necessidade de intensificar a oferta de tratamentos psicológicos destinados à prevenção e à promoção da saúde mental dos enfermeiros.
