RESUMOO presente estudo tem como objetivo identificar os determinantes históricos que influenciaram a enfermagem do HC-UFPR e os modelos gerenciais utilizados nos seus diferentes momentos históricos. Trata-se de uma pesquisa retrospectiva, cujos dados foram coletados por meio de entrevista com personagens que vivenciaram esses momentos. Estes dados foram categorizados por períodos históricos, assim como, os respectivos modelos de organização e, posteriormente, analisados à luz do paradigma materialista. Como resultado final, o trabalho apresenta as várias formas de organização da enfermagem do Hospital, segundo os determinantes de cada momento histórico e algumas considerações acerca da inserção da enfermagem no modelo atualmente implantado. ABSTRACT The present study aimed at identifying the historical determinants that influenced nursing from the HC-UFPR, and the managerial models used in its different historical moments. Como o capitalismo cria uma sociedade onde impera o interesse próprio e prevalece o contrato de trabalho entre empregado e empregador, a gerência torna-se um instrumento perfeito e sutil (1) . As abordagens teóricas que buscam explicar o gerenciamento das organizações de um modo geral e o gerenciamento em saúde e em enfermagem, mais especificamente, partem de dois paradigmas que coexistem na atualidade: o positivista e o materialista (2) . O paradigma positivista tem sido hegemônico na abordagem do trabalho em enfermagem e está fundamentado na Teoria Geral da Administração. Objectives: to understand the work process dimensions related to innovative actions developed by nurses in Primary Health Care. Methods: qualitative, descriptive study, developed in Primary Health Care in a city in the Southern Region of Brazil. Seventy-six nurses, who worked in management and assistance, participated in this study through semi-structured interviews. After data processing by IRAMUTEQ software, the textual analysis occurred by descending hierarchical classification. Results: forty-two innovative actions, considered by the participants as new ways of working, were identified. There was a predominance of innovative actions related to the Management and Assist dimensions; we noticed the fragility of the actions to contemplate all the nurse’s work process dimensions. Final Considerations: the predominance of innovative actions was related to users’ assistance and better conditions in the teamwork process, besides highlighting the role of nurses in the perspective of a new way of working in health services.
