Ö ÖZ ZE ET T AAm ma aç ç: : Çalışmada yenidoğan işitme taraması ile erken dönemde işitme kaybı tanısı konan konjenital işitme kayıplı bebeklerin/çocukların gelişim alanları ile aynı yaş grubunda işitme kaybı olmayan bebeklerin/çocukların gelişim alanlarının karşılaştırılması ve yenidoğan işitme taramasının etkisinin belirlenmesi amaçlanmıştır. Her iki grupta da gelişimlerin değerlendirilmesinde Ankara Gelişim Tarama Envanteri (AGTE) ve Vineland Uyum Davranış Ölçeği uygulanmıştır. B Bu ul lg gu ul la ar r: : İşitme kayıplı gruptaki bebeklerde/çocuklarda, normal işiten gruptaki bebeklere/çocuklara göre AGTEDil Bilişsel (DB) ve AGTE-Genel Gelişim (GG) puanları ile V-İletişim, V-Sosyalleşme ve V-Uyum Düzeyi test puanları anlamlı derecede düşük elde edilmiştir (p<0,05). Yenidoğan işitme taraması hedeflerine göre erken tanı (0-6 ay) konan bebekler/çocuklar ile geç tanı (7 ay ve üzeri) konan bebeklerin/çocukların karşılaştırmasında, erken tanı konanların AGTE-DB ve AGTE-GG test puanları ile V-İletişim ve V-Uyum Düzeyi test puanları anlamlı derecede daha yüksek elde edilmiştir (p<0,05). İşitme kaybı derecelerine göre yapılan karşılaştırmada, çok ileri derecede işitme kaybı olan bebekler/çocuklar normal işiten çocuklara göre AGTE-DB, V-Alıcı Dil, V-İfade Edici Dil ve VSosyalleşme testlerinde düşük puan almışlardır (p<0,05). Erken (3-6 ay) ve geç (7 ay ve üzeri) cihaz kullanan bebeklerde AGTE ve VUDÖ test puanlarında anlamlı farklılık elde edilmemiştir(p>0,05) S So on nu uç ç: : Yenidoğan işitme taraması, tanı, işitme cihazı kullanma ve işitsel eğitime başlama yaşlarını düşürmenin yanı sıra, işitme kayıplı bebeklere kortikal algısal gelişim yönünden önemli olan kritik dönem içinde müdahale edilebilmesini de sağlamaktadır. İşitme kayıplı bebekler/çocuklar dil gelişimi ve dile dayalı gelişim alanlarında normal işiten yaşıtlarından geri olmakla birlikte, erken tanı konan işitme kayıplı bebekler/çocuklar, geç tanı konanlara göre dil gelişimi ve genel gelişim alanlarında daha iyi olmaktadırlar. ÖzPurpose: Swallowing is a complex neuromuscular process requiring the integrity of many muscles and nerves. The natural aging process can result in deterioration of the swallowing structures and result in functional changes in healthy older adults. The Test of Masticating and Swallowing Solids is the only non-instrumental test that provides quantitative measures for solid food ingestion. This study aimed to establish normative data for the Test of Masticating and Swallowing Solids in healthy Turkish adults to potentially serve as a standard in dysphagia evaluation. Materials and Methods: A total of 144 healthy Turkish adults (ages 20-80+ years) grouped by age and gender participated in this study. The test required participants to eat a regional cracker. Per the protocol, the number of masticatory cycles, number of swallows, number of bites, and total duration to eat the cracker were recorded. Results: Normative data were obtained for the Test of Masticating and Swallowing Solids from 144 healthy adults, stratified by age and gender. Younger participants took fewer bites (2.00 versus 2.65), chewed less (34.50 versus 51.02), swallowed less (2.06 versus 3.39), and swallowed quicker (30.14 versus 49.20) than older participants. In terms of gender, males took fewer bites (1.94 versus 2.59), chewed less (40.98 versus 45.83), and swallowed quicker (36.72 versus 41.23) than females. Conclusion: The Test of Masticating and SwallowingSolids is a quick, sensitive, and objective measure of solid bolus ingestion that may be useful for identifying dysphagic patients at bedside and for tracking the efficiency of the oral phase of swallowing.
Amaç: Bu çalışmada yaş faktörünün koklear implant uygulamasının uzun dönem başarısına etkisi incelendi. Hastalar ve Yöntemler:Ocak 2001 -Aralık 2008 tarihleri arasında kliniğimizde iki taraflı ileri/çok ileri düzeyde sensörinöral işitme kaybı ön tanısıyla koklear implant uygulanan hastaların elektronik kayıtları retrospektif olarak incelendi. Hastalar koklear implant uygulandığın-da dört yaş altında olanlar (grup 1) ve dört yaş üstünde olanlar (grup 2) şeklinde iki gruba ayrıldı. Gruplar ameliyat sonrası üçüncü yılda demografik özellikler, işitme performans kategorisi (CAP) ve anlaşılabilir konuşma oranı (SIR) skorları açısından değerlendirildi ve karşı-laştırıldı. Bulgular:Toplam 37 hastaya koklear implant uygulaması yapıldı. Bu hastaların 18'i (%48.6) dört yaş altında, 19'u (%51.4) dört yaş üstünde ameliyat edildi. Gruplarda hastaların cinsiyetleri, anne ve babanın eğitim düzeyleri ve meslekleri, implantın uygulandığı taraf ve implant kullanım süreleri açısından anlamlı farklılık yoktu. İşitme performans kategorisi ve SIR kategorileri esas alındığında, grup 1'deki hastalarda CAP ve SIR kategorileri istatistiksel olarak anlamlı düzeyde daha yüksek idi (sırasıyla p=0.009 ve p<0.001).Sonuç: Çalışma bulgularımıza göre, koklear implantın uygulandığı yaş, uzun dönem işitme ve konuşma yetisi üzerine anlamlı olarak etki etmektedir. Erken yaşlarda yapılan implant uygulamaları uzun dönem etkinlik açısından daha başarılı sonuçlar elde edilmesini sağlamaktadır.Anahtar Sözcükler: Koklear implantlar; İşitme kaybı; uzun dönem etkiler; sensorinöral; tedavi.Objectives: This study aims to investigate the effects of age factor on the long-term efficacy of cochlear implantation. Patients and Methods:The electronic records of patients, who underwent cochlear implantation with the initial diagnosis of bilateral severe to profound sensorineural hearing loss in our clinic between January 2001 and December 2008, were analyzed retrospectively. The patients were divided into two groups according to their age at the time of cochlear implantation, including group 1 with the patients less than four years of age and group 2 with the patients more than four years of age. Groups were evaluated and compared for the in terms of demographic characteristics, Categories of Auditory Performance (CAP) and Speech Intelligibility Rating (SIR) at three years following surgery.Results: A total of 37 patients underwent cochlear implantation. Of them, 18 patients (48.6%) were operated at less than four years of age, while 19 patients (51.4%) were operated at more than four years of age. No significant difference was found between the groups in terms of sex, educational level and profession of the parents, operated side and duration of implant use. According to CAP and SIR categories, group 1 patients had statistically significantly higher level of the CAP and SIR categories (p=0.009 and p<0.001 respectively).
